Archives of Otolaryngology-head & Neck Surgery, 1996Co-Authors: Robert S ThorntonAbstract:Background: Lateralization of the middle turbinate with scarring and obstruction of the middle Meatus after endoscopic ethmoidectomy has accounted for a high percentage of postoperative complications. Objective: To evaluate a suture stabilization technique of the middle turbinates in an attempt to solve this problem and preserve the middle turbinate. Design: Retrospective chart review of 31 consecutive patients undergoing endoscopic sinus surgery with suture stabilization of the middle turbinate. Results: With the technique of suture stabilization of the middle turbinate in 60 operated-on sides of 31 patients, 59 sides showed the middle Meatus to be patent without synechia or maxillary sinus ostium obstruction postoperatively. Conclusion: The complication of lateralization of the middle turbinate with scarring and obstruction of the middle Meatus after endoscopic ethmoidectomy can be prevented with suture stabilization of the middle turbinate, and the middle turbinate can be preserved. Impact Assessment of Urethral Meatus Morphology and Penile Biometry in Transurethral Prostate and Bladder Surgery

Sampaio, Luciano A. FavoritoAbstract:Objective. To analyze the penile and urethral Meatus biometry and its correlation with meatoplasty during endoscopic resections. We also propose a new classification for urethral Meatus morphology. Materials and Methods. We prospectively studied 105 patients who underwent prostate and bladder transurethral resections. We performed standardized measurement of penile and urethral Meatus biometry followed by penile photo in the front position. The need to perform meatoplasty or dilatation during resectoscope introduction was registered. Data were analyzed comparing the correlation between two groups: without intervention (Group A) and with intervention (Group B). Results. We observed in Group A and Group B, respectively, the average length of urethral Meatus of 1.07 cm versus 0.75 cm () and average width of urethral Meatus of 0.59 cm versus 0.38 cm (). Considering the morphology of the urethral Meatus, we propose a new classification, in the following groups: (a) typical; (b) slit; (c) point-like; (d) horseshoe; and (e) megaMeatus. The point-like Meatus was the one that most needed intervention, followed by the slit and the typical Meatus (). Conclusions. Point-like and slit-shaped urethral Meatus, as well as reduced length and width of the urethral Meatus, are the determining factors.

Development, 1996Co-Authors: Moises Mallo, Thomas GridleyAbstract:The tympanic membrane in mammals is a trilaminar structure formed by the apposition of two epithelial cell layers, along with an intervening layer of cells derived from pharyngeal arch mesenchyme. One epithelial layer is contributed by the external acoustic Meatus, a derivative of the first pharyngeal cleft. The other epithelial layer is contributed by the tubotympanic recess, a derivative of the first pharyngeal pouch. We demonstrate here an absolute correlation between formation of the external acoustic Meatus and formation of the tympanic ring, a first arch-derived membrane bone that anchors the tympanic membrane. Experimental loss of the tympanic ring by retinoic acid treatment, or duplication of the ring in Hoxa-2 null mutant embryos, resulted in corresponding alterations in formation of the external acoustic Meatus. We suggest that the tympanic ring primordium induces formation and morphogenesis of the external acoustic Meatus, and that expression of the Hoxa-2 and goosecoid genes may be involved in regulating the formation and morphogenesis of these structures.

Excision and Circumferential Buccal Graft for Severe Meatus and Fossa Navicularis Strictures.

FiglerAbstract:Objective To demonstrate our technique for glans sparing trans-urethral excision and circumferential buccal graft for severe strictures of the Meatus and fossa navicularis. Materials and methods Key steps of the procedure are use of a fixed retractor and stay sutures to retract the glans skin laterally, pre-placing urethral sutures to improve visualization, and dividing the buccal graft in half so that dorsal and ventral portions of the anastomosis can be performed separately. Results As demonstrated in the video, this approach allows for effective treatment of challenging Meatus and fossa navicularis strictures while avoiding post-operative glans dehiscence and minimizing disruption of glans vasculature and sensation. Conclusion Glans sparing trans-urethral excision and circumferential buccal graft is an effective technique for severe strictures of the Meatus and fossa navicularis.

Preoperative treatment with human chorionic gonadotropin in infancy decreases the severity of proximal hypospadias and chordee.

JayanthiAbstract:ABSTRACTPurpose: We determined whether human chorionic gonadotropin (HCG) pretreatment of severe proximal penoscrotal hypospadias and chordee causes sufficient penile shaft or skin enlargement to enhance surgical repair and improve patient outcome.Materials and Methods: A total of 12 boys 6 to 12 months old with proximal hypospadias and severe chordee received a course of HCG for 5 weeks immediately preceding hypospadias repair.Results: Chordee decreased and penile length increased in all cases (mean increase 94%). Penile length gain was disproportional. Most of the increase in length was proximal to the urethral Meatus, which moved the Meatus distally an average of 11.4 mm. (range 6.0 to 19.0), producing a mean increase of 586% in the distance between the penoscrotal junction and Meatus. In contrast, there was no statistically significant increase in penile shaft length distal to the urethral Meatus. Surgical treatment was facilitated by HCG pretreatment.
