MATERIALS AND METHODS: Gray-scale and color Doppler US examinations were performed of scrotal masses in eight patients. The masses were evaluated for size, location, echogenicity, sound attenuation, and vascularity. RESULTS: Seventeen intratesticular masses and one extratesticular mass were examined. All were hypoechoic except for one intratesticular mass that contained hyperechoic areas. Six masses demonstrated sound attenuation. The Mediastinum Testis was found in the center of 11 of the 17 intratesticular masses. At color Doppler US, six masses were hypervascular, seven were isovascular, and five were hypovascular relative to the normal testicle. All intratesticular masses contained vascular structures that entered them from the normal Testis without change in course or caliber. Eleven masses showed a spoke-like pattern of converging vessels. Testicular adrenal rest tumours in postpubertal males with congenital adrenal hyperplasia: sonographic and MR features

JagerAbstract:The purpose of this study was to investigate the prevalence of testicular adrenal rest tumours in patients with congenital adrenal hyperplasia (CAH), and to describe sonographic and MR features of these lesions. Seventeen postpubertal male CAH patients underwent scrotal sonography, with colour Doppler, and in 16 of them pre- and postcontrast enhanced T1- and T2-weighted MR images of the testes were obtained. Ultrasound revealed lesions in 16 of 17 patients (94%), bilateral in 10 patients and unilateral in 6 patients. The lesions were typically located adjacent to the Mediastinum Testis. The maximal diameter of the lesions varied from 2 to 40 mm. Margins were blurred in 11 of 31 lesions. Seventeen of the 20 lesions smaller than 2 cm in diameter were hypoechoic, whereas all 11 lesions larger than 2 cm showed hyperechoic reflections. On MR all lesions were isointense on T1- and hypointense on T2-weighted images and lesion margins were clearly defined. Enhancement of the lesions after intravenous contrast was seen in 13 of 15 patients. In our series the prevalence of testicular adrenal rest tumours in postpubertal CAH patients is much higher than in other reported studies. The lesions may develop from some small, hypoechoic, and multifocal nodules and coalesce to large hypoechoic lesions with hyperechoic reflections on ultrasound. As our results suggest that ultrasonography and MR show the lesions equally well, ultrasonography should be the method of first choice for detection and follow-up of these lesions, because it is the cheapest and quickest imaging technique. In case of a partial orchiectomy, MR is recommended because it shows lesion margins optimally.

Morphological variations of intra-testicular arterial vasculature in bovine Testis - a corrosion casting study

The article offers the first classification of the bovine intra-testicular arteries. A corrosive study focused on the intra-testicular arterial vasculature was performed on 40 bovine testes. Result In bulls, the centripetal arteries tended to run straight to the mediastinal region, where they form knot-like vascular structures. Those structures are the origin for centrifugal recurrent branches, running peripherally. However, three basic types of intra-testicular arterial vasculature were noted. Type I had centrifugal, recurrent branches, running peripherally towards the surface of the Testis but did not reach the tunica albuginea. Type II exhibited centrifugal, recurrent branches running more horizontally than type I. Type III is the most heterogeneous type, composed of other variform types of arteries not classified as type I or type II. Type II was most commonly observed as a vascular conglomerate of intra-testicular arteries within the arterial network of the Mediastinum Testis. In type III, artery diameter was significantly smaller than observed in types I and II ( p 0.05). Conclusion Variations of the intra-testicular arterial vasculature in bovine Testis may suggest that particular types of vessels play different physiological roles. The most common type of intra-testicular artery comprising the arterial network of the Mediastinum Testis was type II.

Arterial supply of human and bovine testes: a topographic and morphometric comparison study.

The study was made on two extremely different types of location of the Mediastinum Testis. The study was made on 100 (50 human and 50 bovine) corrosive casts of the testicular, cremasteric, and deference duct arteries. The differences between the species included different courses of the testicular artery inside the spermatic duct, the relative size of the three arterial diameters, and the morphology of the anastomoses of the arteries. In human testicular arteries, the course inside the spermatic course was more variable than in that of bulls. The artery was straighter and in 80% of the cases did not form the loops which were present in 100% of the bovine specimens. The bovine testicular artery was significantly wider in relation to the cremasteric and deferens duct arteries than the human one. This finding suggests that collateral blood flow to the Testis was less effective in bulls than in men. The human testicular artery directly connected the other two with its terminal branches. The bovine testicular artery connected with the cremasteric and deferens duct arteries indirectly by means of its deferens duct branch. (Folia Morphol 2010; 69, 4: 225-231)

Topographic and morphometric comparison study of the terminal part of human and bovine testicular arteries.

The study was made on two extremely different types of location of the Mediastinum Testis. The investigation was carried out on 80 (40 human and 40 bovine) corrosive casts of the testicular arteries. The differences between the species, including the different course of the testicular artery inside the spermatic cord and in the posterior margin of the gonads, were observed. The division of the testicular artery into terminal branches was located in men on the level of the Mediastinum Testis, and in bulls close to the inferior end of the gonad. The types of terminal division were similar in both groups. In men, the testicular artery course inside the spermatic cord was more variable than in bulls. The artery was straighter, and in 75% of the cases it did not form the loops which were present in 100% of the bovine specimens. The bovine testicular artery in the posterior margin of the Testis was longer and had a more variable course than in men.

Comparison of the blood vessel complexes of the human and bovine male gonads.

BolanowskiAbstract:The aim of the study was to compare the blood vessel network of the human and bovine male gonads, paying special attention to differences resulting from Mediastinum Testis localisation. Two groups of specimens were compared. Each group consisted of 50 corrosive die casts of the testicular and spermatic cord vessels of bovine and human gonads respectively. In each group 20 gonads had only the arterial vessels injected, another 20 only the veins, while in 10 gonads both arteries and veins were injected. The die casts were examined with a stereoscopic binocular as well as an optic microscope. The different testicular vascular patterns in humans and bulls were observed. In humans the intratesticular arteries branch off the arterial network of the tunica albuginea and Mediastinum Testis. The former runs centripetally, the latter centrifugally. Analogically, the intratesticular veins emptying into the tunica albuginea venous network run centrifugally and those emptying into the Mediastinum Testis plexus run centripetally. In bulls the arterial vessels run centripetally, later forming a helical, screw-like layer to give off centrifugal branches. Venous vessels run centrifugally and empty into the venous plexus of the tunica albuginea.

Human testicular arterial supply: gross anatomy, corrosion cast, and radiologic study.

Design Prospective. Setting Academic setting. Patient(s) Forty fresh male cadavers. Intervention(s) Gross anatomy of 20 cadavers, corrosion casting of 10, and radiography of 10. Main Outcome Measure(s) Testicular vascular supply, course, branching, and anastomosis. Result(s) The testicular artery descends bilaterally in a straight course (85%) and in a convoluted course (15%). There were three sites of terminations: along the upper pole of the Mediastinum Testis (78.8%), giving terminal branches (16.2%) or descends without division (5%). Four patterns of termination were found, but in the majority (69.7%) it terminates as upper and lower polar branches. The cremasteric artery arises from the inferior epigastric artery and terminates close to the lower end of the Testis, anastomosing with the lower polar branch of the testicular artery. The artery of the vas arises from the inferior vesical artery, terminates by several capsular branches close to the Mediastinum Testis, anastomosing with branches of the testicular artery along the Mediastinum Testis. Conclusion(s) The Testis gets its arterial supply mainly from the testicular artery supplemented with the cremastric artery and the artery of the vas. The Testis has rich vascular areas in the upper polar, Mediastinum Testis, and posterolateral segments.
