    SUMMARY Skeletal maturation was studied in 60 Caucasian Western-Canadian children (45 boys, 15 girls) with Minimal Brain Dysfunction (MBD), the majority aged from six to 12 years. Four boys and two girls–i.e. 10 per cent of the group–had a bone age more than 2SD below the normal mean for their chronological age (CA) according to the norms established by Greulich and Pyle, and only one boy had a bone age that was significantly advanced. 38 children had bone ages below the mean line for their chronological age, and 13 had bone ages above the mean line. The features of MBD in the six children with significantly delayed bone age did not differ notably from those of the other MBD children, with the exception of one child whose stature and head circumference were both significantly small. The bone age of 58 of the children was then reassessed blindly by the same radiologist without knowledge of the children's chronological age. The mean bone age was found to be more than eight months lower than the mean CA. A control group of 58 children with trauma or idiopathic scoliosis was assessed with Greulich-Pyle norms and was found to have a mean bone age more than three months lower than the the mean CA. The delay in bone age thus tended to be greater in children with Minimal Brain Dysfunction than in the control children, but the difference was not statistically significant. Further analyses of the data showed that MBD children exhibited a significantly greater variance of bone age than the control children. It is therefore possible that a subgroup of MBD children with significant delay of bone age may prove to have special characteristics which are worthy of study, as they may shed some light on the control of skeletal maturation.     UMMARY Children with ‘Minimal Brain Dysfunction’ and learning disabilities were found to have significantly more minor neurological signs than control children. Many of these signs become less obvious or disappear by the age of 11 years; therefore older cases are more similar to controls, whereas younger cases show lags or deficits at the highest levels of central nervous system functioning—language, fine motor co-ordination and cross-modality integrations.     Abstract:

    Background Before 1980, attention-deficit/hyperactivity disorder (ADHD) was called Minimal Brain Dysfunction and included emotional symptoms now listed as "associated features" in DSM-IV. Data from two multicenter, placebo-controlled studies with 536 patients were reexamined to assess: 1) the pervasiveness of these symptoms in samples of adults with ADHD; 2) the response of these symptoms to atomoxetine; and 3) their association with depressive/anxiety symptoms. Methods The Wender-Reimherr Adult Attention Deficit Disorder Scale (WRAADDS) was used to assess temper, affective lability, and emotional overreactivity, thus identifying patients exhibiting "emotional dysregulation." Other DSM-IV Axis I diagnoses were exclusionary. Outcome measures were the Conners' Adult ADHD Rating Scale (CAARS) and the WRAADDS. Results Thirty-two percent of the sample met post hoc criteria for emotional dysregulation and had higher baseline scores on ADHD measures, a lower response to placebo, and greater response to atomoxetine ( p = .048). Symptoms of emotional dysregulation had a treatment effect ( p p = .002) and the total WRAADDS ( p = .001). Emotional dysregulation was present in the absence of anxiety or depressive diagnosis. Conclusions Symptoms of emotional dysregulation were present in many patients with ADHD and showed a treatment response similar to other ADHD symptoms.

    Abstract:

    After referral to a pediatric neurologist for problems in behavior and learning, 61 children aged from three to seven years were assigned to one of four diagnostic groups: (1) Minimal Brain Dysfunction (MBD); (2) hyperactivity; (3) learning disability; and (4) other criteria. Their temperament profiles were determined by the Behavioral Style Questionnaire. The disproportionately large number of children with more difficult temperament diagnoses in the referred population indicates that teachers and physicians may have mininterpreted a less adaptive behavioral style as evidence of neurological Dysfunction. Those diagnosed clinically as having MBD were less adaptable, less persistent, more active and more negative than the control population. This suggests that MBD overlaps with difficult temperament. Children in the other three groups were temperamentally similar to the MBD group, which raises doubt about the advisability of diagnosing MBD on the basis of behavior alone. A comprehensive neurobehavioral profile is necessary to separate clearly the various factors contributing to problems in school performance.
