UROLOGICAL ONCOLOGY: BLADDER, PENIS AND URETHRAL CANCER, AND BASIC PRINCIPLES OF ONCOLOGY Detection of Occult Tumor Cellsin Lymph NodesFrom Bladder Cancer Patientsby MUC7
2020Co-Authors: M Retz, M. StAbstract:Objective: Systemic progression is the prevalent form of bladder tumor recurrence after radical cystectomy. The detection of occult bladder tumor cells in histopathologically normal lymph nodes could be of prognostic value. We examined the possibility that Mucin 7 (MUC7) RNA might reflect the presence of occult tumor cells in lymph nodes from bladder cancer patients. We used the polymerase chain reaction (RT-PCR), a highly sensitive assay, to monitor MUC7 RNA. Methods: We collected 240 pelvic lymph nodes from 25 bladder cancer patients undergoing radical cystectomy. We also obtained 20 lymph nodes from patients with prostate cancer and interstitial cystitis to use as negative controls. Each lymph node was divided in two parts to provide tissue for both histopathological and PCR analysis. Results: 166/240 lymph nodes from bladder cancer patients were usable for MUC7 RT-PCR. By conventional histopathology, six of these nodes contained metastases. MUC7 RT-PCR analysis was positive for five of the six histologically proven lymph node metastases. Histopathological reevaluation of the sixth node revealed tumor in an adjacent vein, not in the lymph node, itself. In contrast, 46/160 (29%) histologically classified normal lymph nodes (pN0) from 17 bladder cancer patients were positive for MUC7. All 20 lymph nodes from control patients were MUC7-negative. Conclusion: MUC7 RT-PCR is a specific and sensitive method for the detection of occult tumor cells in lymph nodes from bladder cancer patients. Long-term observation will be necessary to evaluate the clinical value of MUC7 as a prognostic indicator of lymph node metastasis and disease progression.

Gschwend, M RetzAbstract:Detection of metastases in lymph nodes is an important prognostic factor for progression-free survival in bladder cancer patients. Patients undergoing radical cystectomy with pelvic lymphadenectomy are randomized in the LEA study (AUO AB 25/02) into two groups receiving standard (obturator and external nodes) or extended lymphadenectomy (complete pelvic nodes up to the inferior mesenteric artery). The aim of this study is the detection of lymph node metastases that are not identified with classic pathological methods using RT-PCR as a highly sensitive and specific method. For detection of occult disseminated tumor cells we analyze the expression of the tumor markers cytokeratin 20 (CK-20), uroplakin II (UP II), Mucin 2 (MUC2), and Mucin 7 (MUC7). We examined 315 lymph nodes from 19 cystectomy patients for the expression of CK-20. In 93 lymph nodes CK-20 expression was detected whereas only 18 lymph nodes were histopathologically positive. More than one third of CK-20-positive lymph node metastases were located outside the standard lymphadenectomy field. We did not detect any skip lesions. Follow-up data will validate if there is a correlation between detection of occult disseminated tumor cells and progression-free survival. Die Detektion von Lymphknotenmetastasen ist ein wichtiger prognostischer Faktor für das progressionsfreie Überleben von Harnblasenkarzinompatienten. Die Zystektomiepatienten werden im Rahmen der LEA-Studie (AUO AB 25/02) randomisiert und erhalten entweder eine eingeschränkte (obturatorische und externe iliakale Lymphknoten) oder eine ausgedehnte Lymphadenektomie (Lymphknoten zwischen Beckenboden und Abgang der A. mesenterica inferior). Ziel unserer Untersuchung ist eine sensitive und spezifische Detektion von Lymphknotenmetastasen, die durch die klassische Histopathologie nicht erkannt werden. Methodisch analysieren wir die Expression einer Genmarkerkombination von Cytokeratin-20 (CK-20), Uroplakin-II (UP-II), Muzin 2 (MUC2) und Muzin 7 (MUC7) mittels RT-PCR (reverse transcriptase-polymerase chain reaction), wodurch wir eine zuverlässige Aussage über das Auftreten von okkulten disseminierten Tumorzellen erhalten. Von 19 Zystektomiepatienten wurden bisher 315 Lymphknoten molekularbiologisch auf CK-20 untersucht. In 93 Lymphknoten konnte eine Expression von CK-20 nachgewiesen werden. Hiervon waren lediglich 18 Lymphknoten in der Histopathologie positiv. Über ein Drittel der CK-20-positiven Lymphknotenmetastasen befanden sich außerhalb des eingeschränkten Lymphadenektomiefeldes. „Skip lesions“ wurden nicht nachgewiesen. Die Nachsorgedaten werden zeigen, ob ein Zusammenhang zwischen dem Nachweis der von uns getesteten molekularbiologischen Epithelmarker und dem progressionsfreien Überleben besteht.

Total RNA was extracted from exfoliated cells collected from 200 ml. voided urine. Clinical significance of combined detection of Survivin and MUC7 in urine in the detection of bladder cancer

Methods Seventy-five patients with bladder cancer, 38 patients with benign diseases of urinary system and 20 healthy volunteers were enrolled, and three urine specimens were collected in all patients. The clinical value of three methods in the diagnosis of bladder cancer was analyzed and compared by detecting the sensitivity, specificity, positive predictive value, negative predictive value and accuracy of Survivin, MUC7 and cytology in urine respectively. Results The sensitivity and specificity of Survivin in the diagnosis of bladder cancer were 93.3% and 98.3%, respectively. The sensitivity and specificity of MUC7 in the diagnosis of bladder cancer were 80% and 100%, respectively. The sensitivity and specificity of cytology in the diagnosis of bladder cancer were 66.7% and 100%, respectively. The sensitivity and specificity of Survivin combined with MUC7 were 98.7% and 100%, respectively. Anatomical stability of human fungiform papillae and relationship with oral perception measured by salivary response and intensity rating

Literature is inconclusive as to whether taste perception correlates with fungiform papillae density (FPD). Gustatory reflexes modulate the amount and composition of saliva subsequently produced, and thus may be a more physiologically objective measure of tastant-receptor interactions. Taste perception fluctuates with time but the stability of individual fungiform papillae is unclear. This study followed ten healthy volunteers longitudinally at baseline, one and six months. FPD, diameter and position were measured and participants rated intensity perception of sucrose, caffeine, menthol and capsaicin solutions. Salivary flow rate, protein concentration and relative changes in protein composition were measured following each tastant. FPD, diameter and position were unchanged at six months. FPD did not correlate with intensity rating for any taste. FPD did correlate with changes in salivary protein output following sucrose (ρ = 0.72, p = 0.02) and changes in levels of proline-rich protein and Mucin 7 following capsaicin (ρ = 0.71, p = 0.02, ρ = 0.68, p = 0.04, respectively). These results suggest that over six months fungiform papillae are anatomically stable, playing a greater role in mediating the physiological salivary response to stimuli rather than determining the perceived intensity of taste.

This loss of salivary function leads to oral dryness, impaired swallowing and speech, and increased infection and is associated with other autoimmune diseases and an increased risk of certain cancers. Despite the implications of this prevalent disease, diagnosis currently takes years, partly due to the diversity in patient presentation. Saliva is a complicated biological fluid with major constituents, including heavily glycosylated Mucins MUC5B and MUC7, important for its viscoelastic and hydrating and lubricating properties. This study investigated Sjogren's patient's perception of dryness (bother index questionnaires) along with the rheological, protein composition, and glycan analysis of whole mouth saliva and the saliva on the mucosal surface (residual mucosal saliva) to understand the properties that most affect patient wellbeing. Sjogren's patients exhibited a statistically significant reduction in residual mucosal saliva, salivary flow rate, and extensional rheology, spinnbarkeit (stringiness). Although the concentration of Mucins MUC5B and MUC7 were similar between patients and controls, a comparison of protein Western blotting and glycan staining identified a reduction in Mucin glycosylation in Sjogren's, particularly on MUC7. LC-MS/MS analysis of O-glycans released from MUC7 by β-elimination revealed that although patients had an increase in core 1 sulfation, the even larger reduction in sialylation resulted in a global decline of charged glycans. This was primarily due to the loss of the extended core 2 disialylated structure, with and without fucosylation. A decrease in the extended, fucosylated core 2 disialylated structure on MUC7, residual mucosal wetness, and whole mouth saliva flow rate appeared to have a negative and cumulative effect on the perception of oral dryness. The observed changes in MUC7 glycosylation could be a potential diagnostic tool for saliva quality and taken into consideration for future therapies for this multifactorial syndrome.
