    Abstract:

    With the growth of basic science studies, clinical research and updated diagnostic and imaging modalities, the subspecialty of Neuro-Ophthalmology has continued to evolve. Clinical updates include novel treatment approaches, new disease entities and improved understanding of the underlying pathophysiology for many well-known neuro-ophthalmic diagnoses. This chapter will review these updates while focusing on clinical aspects in an effort to provide clinicians with knowledge that can be applied to everyday practice.

    This is the first volume of a biennial publication that reviews pertinent neuro-ophthalmologic literature of the preceding 2 years. However, this initial volume contains work from 1980 to 1986. This volume is not intended as a textbook, but rather as a reference of specific neuro-ophthalmologic topics. Each review is written by a clinician or scientist who is considered to be an authority in his or her field. "... provides numerous exhaustive but succinct reviews of neuroophthalmologic topics." The book is divided into six subjects. Part 1 describes the visual sensory system, from diseases of the optic nerve to disorders of higher visual sensory function. Emphasis has been placed on optic neuritis and its differential diagnosis, tumors of the optic nerve, and diseases of the optic chiasm.   • Diagnostic Error of Neuro-ophthalmologic Conditions: State of the Science.

    BACKGROUND Diagnostic error is prevalent and costly, occurring in up to 15% of US medical encounters and affecting up to 5% of the US population. One-third of malpractice payments are related to diagnostic error. A complex and specialized diagnostic process makes neuro-ophthalmologic conditions particularly vulnerable to diagnostic error. EVIDENCE ACQUISITION English-language literature on diagnostic errors in Neuro-Ophthalmology and neurology was identified through electronic search of PubMed and Google Scholar and hand search. RESULTS Studies investigating diagnostic error of neuro-ophthalmologic conditions have revealed misdiagnosis rates as high as 60%-70% before evaluation by a Neuro-Ophthalmology specialist, resulting in unnecessary tests and treatments. Correct performance and interpretation of the physical examination, appropriate ordering and interpretation of neuroimaging tests, and generation of a differential diagnosis were identified as pitfalls in the diagnostic process. Most studies did not directly assess patient harms or financial costs of diagnostic error. CONCLUSIONS As an emerging field, diagnostic error in Neuro-Ophthalmology offers rich opportunities for further research and improvement of quality of care.

    Vascular Neuro-Ophthalmology includes visual symptoms and signs found in stroke patients as well as numerous primary vascular disorders involving the eye and the optic nerves. Cerebrovascular diseases are commonly associated with neuro-ophthalmologic symptoms or signs, which mostly depend on the type, size, and location of the vessels involved, and the mechanism of the vascular lesion. Funduscopic examination allows direct visualization of the retinal circulation, which shares many common characteristics with the cerebral microcirculation, and can be used as a marker of vascular disease.

    PURPOSE OF REVIEW Radiologic imaging is indispensible for the diagnosis and management of many neuro-ophthalmologic conditions. Advances in the radioimaging of neuro-ophthalmologic disorders may evolve from the clinical or the radiological side, meaning there is a constant stream of new information for the clinician. RECENT FINDINGS Functional MRI, diffusion tensor MRI, magnetization transfer imaging, and magnetic resonance spectroscopy are examples of nonstandard radiographic techniques, which have expanded the knowledge of neuro-ophthalmologic conditions. Studies using conventional MRI have also led to advances in understanding optic neuropathies, the ocular motor system, pseudotumor cerebri, posterior reversible encephalopathy syndrome and migraine. SUMMARY This article discusses recent radiologic advances relevant to Neuro-Ophthalmology.
