    Abstract:

    OBJECTIVE. The purpose of this article is to show radiologists how to readily recognize Nonpuerperal subareolar abscess and its complications in order to help reduce the time to definitive therapy and improve patient care. To achieve this purpose, the various theories of pathogenesis and the associated histopathologic features are reviewed; the typical clinical characteristics are detailed in contrast to those seen in lactational abscess and inflammatory breast cancer; the common imaging findings are described with emphasis on the sonographic features; correlative pathologic findings are presented to reinforce the imaging findings as they pertain to disease origins; and the various treatment options are reviewed. CONCLUSION. Nonpuerperal subareolar Mastitis and abscess is a benign breast entity often associated with prolonged morbidity.     Abstract:

    Objective:To investigate the diagnosis and treatment of the Nonpuerperal Mastitis. Methods:120 cases of Nonpuerperal Mastitis from 2001 to 2005 were analyzed retrospectively. Results:94 cases of Nonpuerperal Mastitis were diagnosed correctly,17 cases were misdiagnosed as breast cancer,5 as fibroma and 4 as cyst. The misdiagnosis rate was 21.7%. Conclusion:Retention of secretions, ductal ectasy, galactophoritis and chronic granulomatous Mastitis are steps of pathophysiological processs of different intensity and the causes of occult or symptomatic nipple secretion.

    Abstract:

    BACKGROUND Nonpuerperal Mastitis (NPM) is a nonspecific chronic inflammation in the breast. The two most common pathologic subtypes are idiopathic granulomatous Mastitis (IGM) and plasma cell Mastitis (PCM). The cause of NPM is unknown, and the recurrence rate is high; however, the sequential development of two different pathologic subtypes of NPM in contralateral breasts of the same patient is rare. CASE SUMMARY Two women who were in their 30 s and with a history of childbearing had an inflammatory mass in one breast, which was identified as PCM by core needle biopsy. However, 1 year after treatment, IGM occurred in the contralateral breast. Histopathologic results after the surgery confirmed both diagnoses. New-onset PCM in both patients was successfully treated with local extended surgical resection and antibiotics combined with incision and drainage. The IGM that developed in the contralateral breast after 1 year was treated with local extended surgical resection and oral steroids. No signs of NPM recurrence were observed in either breast after a follow-up period of more than 1 year. CONCLUSION Because recurrence after NPM treatment often occurs in the affected breast, the contralateral breast, which does not often exhibit morbidity factors, is often ignored during treatment follow-up. However, our cases suggest that patients with the occurrence of NPM in one breast should undergo long-term follow-up of the contralateral breast by close monitoring. NPM recurrence should also be considered when there is pain, redness, and swelling in the contralateral breast.

    Abstract:

    Objective To explore and summarize the clinical features and treatmentof the Nonpuerperal Mastitis.Methods Eighty-two cases of nonpuer-peral Mastitis from 2003 to 2013 were analyzed retrospectively.All patients were divided into surgery alone group (11 cases)and medication combined surgery group (71 cases),which PDM patients with triple anti-mycobacterial drugs therapy,GM patients with cortical hormone therapy.Results All patients were diagnosed by biopsy.The total duration of simple drug group (triple or hormone) is (11.92 ±6.50) weeks and remission rate after 4 weeks was (89.06 ± 2.25) %.Drugs plus surgery group,the total duration of the group is (6.57 ± 2.30) weeks,and no recurrence.The total duration of the simple surgery group is (17.09 ± 13.11) weeks and remission rate after 4 weeks was (63.64 ± 1.10) %.Comparison of the total duration of the three groups and the cumulative response rate curves were significantly different(P < 0.05).Conclusions Nonpuerperal Mastitis requires a combination of clinical and pathological diagnosis.Surgery is not the only treatment option,triple anti-mycobacterial or cortisol hormones for different types of pathology conservative treatment can achieve a satisfactory therapeutic effect.Drug therapy combined with surgery can shorten the duration and reduce the relapse rate,it is an effective treatment for Nonpuerperal Mastitis.
