DiepgenAbstract:Summary Background Hand eczema (HE) is a common Occupational Skin Disease. Tobacco smoking is known to be associated with adverse cutaneous effects. However, its influence on the prognosis of Occupational HE has not yet been studied. Objectives To evaluate relations between smoking status, severity and prognosis of Occupational HE in patients taking part in an interdisciplinary tertiary individual prevention programme (TIP). Methods In a prospective, multicentre, cohort study 1608 patients with Occupational HE taking part in a TIP were recruited and followed up for 3 years. The clinical and self-reported outcome data of smokers and nonsmokers were compared. Results Nonsmokers and smokers were equally distributed. During the TIP, the average self-reported daily cigarette consumption and the severity of HE decreased significantly (P < 0·01). However, at all time points HE was significantly more severe in smokers than in nonsmokers. This association was not dependent on the self-reported number of cigarettes smoked daily. Smokers had significantly more days of absence from work due to Occupational HE than nonsmokers in the year before the TIP (P < 0·01) and in the following year (P = 0·02). After the TIP, smokers reported significantly more often that they had to give up their occupation (P = 0·02) than nonsmokers. Conclusions The severity of Occupational HE is increased in smokers. Tobacco smoking is associated with a higher number of days of absence from work and with not staying in the workforce owing to Occupational HE. Thus, smoking confers a worse prognosis and interferes with the outcome of prevention programmes.

DiepgenAbstract:Summary Invasive squamous cell carcinoma (SCC) as a “quasi Occupational Disease” according to §9 Section 2 of the German Social Code Book (SGB) VII typically develops on chronically UV-damaged Skin from actinic keratoses. After the Medical Scientific Committee of the Federal Ministry of Labor and Social Affairs has confirmed the legal criteria for acknowledging UV-induced SCC as an Occupational Disease, it is expected that the condition will be added to the official list of Occupational Diseases issued by the Federal Government in the near future. The Social Accident Insurance is required by law (§3 Occupational Disease Regulation) to prevent these tumors by “all appropriate means“. There are excellent therapeutic and preventive measures for the management of actinic keratoses to avoid the development of SCC. The “Dermatologist's Procedure“ according to §§ 41–43 of the agreement between the Social Accident Insurance and the Federal Medical Association was established in Germany in 1972 to take preventive measures in insured persons with Skin lesions possibly developing into an Occupational Disease, or worsening it, or leading to a recurrence of it This procedure proved to be very successful in the prevention of severe and/or recurring Skin Diseases forcing a worker to leave his job. On the basis of this agreement, the Social Accident Insurance has the instruments to independently provide preventive measures for the new Occupational Skin Disease SCC induced by natural UV light according to §9 Section 2 of the German Social Code Book (SGB) VII.

DiepgenAbstract:Background. Occupational Skin Disease (OSD) is common, and imposes a considerable personal and public burden. To tackle OSD, the German stepwise procedure of handling OSD was set up. It contains an interdisciplinary, integrated inpatient rehabilitation measure [tertiary individual prevention (TIP)] [dermatological treatment and diagnostic procedures, and patient education (health and psychological)]. The primary aims of the TIP are reduction of the severity of OSD, reduction in the use of corticosteroids, facilitation of return to work, decreased absence from work, and enhanced quality of life (QoL). It was positively evaluated for a period of 4 weeks after return to work. Objectives. To investigate whether the observed short-term effects remain significant and meaningful over a period of 12 months after discharge from the TIP. Methods. A prospective design was used to compare clinical and patient-reported outcome data between admission to a 3-week inpatient TIP and 12 months after discharge (12-month follow-up). Results. Of 1788 individuals admitted to the TIP, data from 1617 individuals were available for analysis. We observed a significant reduction in the severity of OSD, the use of topical corticosteroids, and days of absence from work because of OSD. QoL was significantly improved, and 87.4% were able to return to work and remain in the workforce. Conclusions. A randomized controlled trial would have been desirable, but was not possible, for legal and other reasons. However, the long-term 12-month follow-up shows that the TIP is associated with sustained improvements in terms of ability to work, QoL, and prognosis, and reductions in days of absence from work because of Skin conditions and topical corticosteroid application. These results indicate that the TIP provided a reduction in the personal and public burden of OSD.

Little is known about the cognitive representations individuals with Occupational Skin Disease have towards measures of personal Skin protection and Occupational safety and whether they change during an intervention. We aimed to evaluate whether social cognitions as embodied by the theory of planned behaviour become more favourable during a tertiary inpatient individual prevention programme (TIP) and whether the model’s predictions hold in a setting to which the model has not been applied. We used a longitudinal design. A questionnaire, assessing the theory of planned behaviour variables attitude, subjective and descriptive norm, perceived behavioural control and behavioural intention was developed and administered to 101 patients before (at admission) and after (at discharge) a 3-week inpatient TIP. The scales showed good internal consistency. Before the TIP, patients had favourable cognitions towards Skin protection measures and these improved during the TIP. Attitude, perceived behavioural control and intention to perform Skin protection significantly increased during TIP. Attitude and perceived behavioural control were significant predictors of behavioural intention in multiple regression analyses with perceived behavioural control being the strongest predictor in the equations. Descriptive norm was a significant predictor of intention only at admission but not at discharge. This is the first study attempting to explain the motivation to perform Skin protection measures in patients with Occupational Skin Disease by applying an otherwise well established health-behaviour theory. The results emphasise the importance of health-educational and psychological interventions for patients with Occupational Skin Disease. Promoting personal control over and attitudes towards Skin protection measures may enhance the Occupational health of individuals with Occupational Skin Disease.

Occupational Skin Diseases among dental nurses

A structured questionnaire was used to inquire about Skin, respiratory symptoms, atopy, work history and methods, and exposure at work. The 328 nurses, who reported work-related dermatitis on their hands, forearms or face, were invited to an interview by an Occupational physician; 245 nurses participated. 31 nurses had previously been diagnosed with an Occupational Skin Disease (OSD). 133 nurses with a suspected OSD were selected for further clinical examinations with prick and patch testing. Among the 107 nurses examined, 22 new cases of OSD were diagnosed. There were altogether 29 cases of allergic contact dermatitis, 15 of contact urticaria, 12 of irritant contact dermatitis, and 1 case of onychomycosis. Rubber chemicals and natural rubber latex (NRL) in protective gloves, as well as dental-restorative plastic materials [(meth)acrylates], were the most common causes of allergy. 42% of the OSD cases in the studied population had been missing from the statistics (Finnish Register of Occupational Diseases). Plastic gloves or NRL gloves with a low-protein content are recommended for dental work. Skin exposure to (meth)acrylates should be avoided.

Good Occupational Skin Disease statistics exist in few countries. Questionnaire studies are needed to get more data on the epidemiology of Occupational Skin Diseases. The Nordic Occupational Skin Questionnaire Group has developed a new questionnaire tool - Nordic Occupational Skin Questionnaire (NOSQ-2002) - for surveys on work-related Skin Disease and exposures to environmental factors. The 2 NOSQ-2002 questionnaires have been compiled by using existing questionnaires and experience. NOSQ-2002/SHORT is a ready-to-use 4-page questionnaire for screening and monitoring Occupational Skin Diseases, e.g. in a population or workplace. All the questions in the short questionnaire (NOSQ-2002/SHORT) are included in the long version, NOSQ-2002/LONG, which contains a pool of questions to be chosen according to research needs and tailored to specific populations. The NOSQ-2002 report includes, in addition to the questionnaires, a comprehensive manual for researchers on planning and conducting a questionnaire survey on hand eczema and relevant exposures. NOSQ-2002 questionnaires have been compiled in English and translated into Danish, Swedish, Finnish and Icelandic. The use of NOSQ-2002 will benefit research on Occupational Skin Diseases by providing more standardized data, which can be compared between studies and countries.

The workers had been exposed to many chemicals. Those working in the GRP industry had also been exposed to glass fibre and to dust produced by finishing work. 94% used protective gloves. 22 workers, all employed in the GRP industry, had contracted Occupational Skin disorders. 6 had allergic and 12 irritant contact dermatitis. 4 workers had an accidental injury caused by a peroxide catalyst, fire, hot air and constant mechanical friction. Allergic dermatoses were due to natural rubber (latex) (4 cases) in protective gloves, phenol-formaldehyde resin (1 case) and cobalt naphthenate (1 case). Irritant hand dermatoses (5 cases) were caused by the combined hazardous effect of unsaturated polyester or vinyl ester resins, organic solvents, glass fibre and dust from finishing work on the Skin. Other cases of irritant dermatoses (7 cases) were due to the dust, promoted by mechanical friction of clothes. Skin disorders in the GRP industry were common (26%) but the symptoms were mild and only 3 patients had been on sick leave because of Occupational Skin Disease.

Essential oils are also increasingly used to promote wellness. In previous studies, the sensitization potential of some EOs has been identified; however, the current prevalence of sensitivity is largely unknown. Objectives The aim of this study was to determine frequency of positive patch-test reactions to EOs tested in the baseline series, along with 3 fragrance markers (FMs) (fragrance mix I, fragrance mix II, and Myroxylon pereirae), in consecutive patients in the US/Canadian North American Contact Dermatitis Group (NACDG) (2009-2014) and the central European, trinational Information Network of Departments of Dermatology (IVDK) (2010-2014). Methods This study used a retrospective analysis of patch-test results and relevant demographic/clinical data collected electronically by the networks, obtained with Santalum album 10% petrolatum (pet) (IVDK only); Cananga odorata 2% (NACDG) and 10% (IVDK) pet; Jasminum species 2% (NACDG) and 5% (IVDK) pet; Mentha piperita 2% pet; Melaleuca alternifolia, oxidized (tea tree oil), 5% pet; and Lavandula angustifolia 2% pet (latter 3 NACDG only). Results Overall, 62,354 patients were tested to 3 FMs and EOs (NACDG, 13,398; IVDK, 48,956); 11,568 (18.6%) reacted to at least 1 FM or EO, whereas 857 (1.4%) reacted to 1 or more EOs but none of the 3 FMs. For both the NACDG and IVDK populations, individuals who were positive to 1 or more of the 9 study allergens were significantly less likely to be male, have Occupational Skin Disease, or have hand involvement and significantly more likely to have leg dermatitis and be 40 years and older (P's ≤ 0.005). Prevalence rates for EOs were as follows: S. album, 1.4% IVDK; C. odorata, 1.1% NACDG and 2.4% IVDK; Jasminum species, 0.7% NACDG and 1.4% IVDK; M. piperita, 0.9% NACDG; L. angustifolia, 0.3% NACDG; and M. alternifolia, 0.3% NACDG. Of the 140 NACDG patients who reacted to 1 or more of the 5 NACDG EOs but none of the FMs, M. alternifolia yielded most positive reactions (45%); half of these reactions were strong (++ or +++, 50.8%) and of definite/probable clinical relevance (52.4%). Of the 717 IVDK patients who reacted to 1 or more of the 3 IVDK EOs but none of the 3 FMs, 38% were positive to C. odorata, 38% to S. album and 36% to Jasminum species.' Conclusions Testing to EOs may be important for detecting sensitivity not detected by FMs alone. In North America, M. alternifolia is an important and clinically relevant sensitizer often not detected by FM. In Europe, as well as in North America, clinical relevance is often difficult to evaluate because (1) labeling of EOs when used as fragrance is not mandatory, and (2) these mixtures may indicate sensitization to 1 or more of their individual constituents from other sources, including synthetic fragrances.

Patch testing is an important diagnostic tool for the determination of responsible allergens. OBJECTIVE: This study reports the results of patch testing by the North American Contact Dermatitis Group from January 1, 2003, to December 31, 2004. METHODS: At 13 centers in North America, patients were tested with the same screening series of 65 allergens, with a standardized patch-testing technique. Data were recorded on standardized forms and manually verified and entered. Descriptive frequencies were calculated, and trends were analyzed with chi-square statistics. RESULTS: A total of 5,148 patients were tested. Of these, 3,432 (66.7%) had at least one positive reaction, 2,284 (44.4%) were ultimately determined to have primary allergic contact dermatitis, and 676 (13.1%) had occupation-related Skin Disease. There were 9,762 positive allergic reactions. Compared to the previous reporting period (2001-2002), allergies to nickel, budesonide, mercaptobenzothiazole, and paraben mix were at least 1.12 times more common (all p values < .03). Compared with the previous 8 years (1994-2002), only the prevalence rates of allergies to nickel and budesonide were statistically significantly higher (p values < .003). CONCLUSION: Allergic contact dermatitis from nickel and budesonide may be increasing in North America. These results again underscore the value of patch-testing with many allergens.

Recent reports suggest otherwise. Objectives: The purposes of this study were to determine the prevalence of allergic and irritant forms of Occupational contact dermatitis in the population seen at a Midwestern dermatologic referral clinic, report professions commonly affected by Occupational Skin Disease, and indicate which substances are frequent allergens. Methods: A retrospective analysis of patch test data collected at the University of Kansas between 1994 and 1999 was performed. Results: Of 537 patients who underwent patch testing, 135 (25%) had Occupational Skin Disease. Allergic contact dermatitis affected 81 (60%) patients, and irritant contact dermatitis was found in 46 (34%). Health care professionals, machinists, and construction workers accounted for nearly half of all patients with Occupational Skin Disease. Nickel sulfate, glutaraldehyde, and thiuram mix were the most common allergens. Conclusion: Patch tests with a wider array of allergens than those currently available in the United States are needed to prevent misdiagnosis of Occupational contact dermatitis. Diepgen, Stefanie Soder, Elke WeisshaarAbstract:Background: Specific instruction courses as a tool for secondary prevention of Occupational Skin Disease (secondary individual prevention, SIP courses) affecting individuals have been introduced in Germany. Little is yet known about the outcome of Occupational Disease and related behavioural patterns following these courses. Objectives: To investigate the outcome of Occupational Skin Disease in health care workers (HCW) 1 year after attendance of a SIP course. Patients/methods: Two hundred and fifty-three HCW participated in SIP courses and were contacted by telephone 1 year after attendance. Patients were interviewed about Skin lesions, medical treatment, sick leave, change of occupation, Skin protection, Skin care and Skin cleansing, and the impact of Skin Disease. Results: The follow-up rate was 81%. The proportion of participants reporting Skin lesions decreased significantly (68% at follow-up compared with 77% at baseline; P = 0.02). Seventy-two per cent reported that their Skin lesions had improved. Nine per cent reported having left their occupation due to their Skin Disease. Skin care and Skin protection had improved, while the frequency of reported hand washing was reduced. Twenty-seven per cent said their quality of life was impaired due to the Skin Disease, compared with 54% at baseline (P < 0.001). Conclusions: Outcome parameters, including behavioural parameters, indicate a positive impact of courses for SIP on Skin health of HCW 1 year after attendance.

Prevention including improving individual Skin care and Skin protection behavior, health education, optimizing diagnostics and therapy as well as avoidance of Occupational Skin Disease (BK 5101) is important. Patients and methods: Participants in the courses were patients suspected of having an Occupational Skin Disease. Besides socio-demographic and Diseaserelated data, health-related quality of life (QL) was measured using the SF-36 and Skindex-29. One year later all participants were interviewed by telephone about the course of their Skin Disease. Results: Out of 212 participants, 84.0 % were female. The mean age was 41.6 (SD = 10.8) years.168 patients (79.2 %) suffered from hand dermatitis,with irritant contact dermatitis being the predominant diagnosis (46.2 %,n = 98).One year later 65.4 % (n = 85) of the patients interviewed still suffered from hand der-matitis.9.2 % (n = 12) had meanwhile quit their job due to the Skin Disease.QL was impaired in all age groups being lower with increasing age of the patients. Conclusions: The follow-up confirmed the positive impact of the Skin protection courses on patients' Skin Disease and well-being. Occupational Skin Diseases impair health-related quality of life in these professions but Disease severity does not seem to play a key role.

DiepgenAbstract:Due to increased Occupational dermatoses in certain professions such as health care workers (HCW), cleaning and kitchen employees (CKE), measures of prevention and Skin protection are of high significance. Skin protection courses focus on educational aims (e.g. improving Skin care habits, influencing the participants’ attitudes towards health) and medical aims (e.g. optimising diagnostic procedures, complementing individual therapy). Participants are patients who have been suspected to suffer from an Occupational Skin Disease and are insured with the German Accident Prevention & Insurance Association (BGW). Teaching units of the Skin protection courses focus on basics of Skin functioning and important aspects of Occupational Skin Diseases. Practical parts include information and instructions about the correct implementation of Skin protection, Skin care and Skin cleansing. Every participant is seen by a dermatologist obtaining a precise patient’s history and performing a Skin examination. All this results in working out individually adapted and professional Skin protection strategies. In total, 791 participants (93 men and 698 women) completed the Skin protections courses. Six hundred and sixty-seven patients (mean age: 36.9 years, SD = 11.6) were HCW and 124 patients (mean age: 40.4 years, SD = 10.8) were CKE. In HCW 82.5% (n = 550) and in CKE 86.3% (n = 107) suffered from hand eczema of the atopic, irritant or allergic type. Irritant contact dermatitis as a single diagnosis was the most frequent one in both groups (HCW: 34.5.%, n = 230; CKE: 49.2% n = 61). According to regularly performed evaluations, the participants rated the course as good to excellent. Nearly 80% of the participants had Skin lesions while attending the course, in 27% the dermatosis was severe. There is a high need for health education, advisory services, diagnostics and additional therapy in Occupational dermatology. Prevention of Occupational Skin Diseases and maintenance of health through educational programmes are important complementary measures for dermatological care but still missing in endangered professions.
