  • case control study on open reduction and internal fixation for the treatment of humeral surgical neck fracture in aged patients

    Abstract:

    OBJECTIVE To summarize clinical experiences of open reduction and internal fixation for the treatment of humeral surgical neck fractures. METHODS From May 2008 to May 2013, 295 patients with humeral surgical neck fractures were divided into two Groups: Operation Group and small splint Group. In the Operation Group: there were 139 cases including 58 males and 81 females, treated with open reduction and locking plate fixation. The mean age of the patients was (70.3 ± 6.5) years old (ranged from 60 to 91 years old). The interval from injury to hospital ranged from 30 min to 15 days, with an average of (19.7 ± 18.4) hours. In the small splint Group: there were 156 cases including 75 males and 81 females, treated with manipulative reduction and small splint external fixation. The mean age of the patients was (70.6 ± 7.0) years old (ranged from 60 to 98 years old). The interval from injury to hospital ranged from 1 hour to 15 days, with an average of (20.2 ± 20.1) hours. The therapeutic effects were evaluated with shoulder pain, function, activity, anatomical indicators and total score according to Neer Score after treatment. RESULTS All the patients were followed up, and the duration ranged from 5 to 20 months, with an average of 13.2 months. According to Neer Score, the total score of patients in the Operation Group was 91.48 ± 7.46; while in the small splint Group, the score was 85.62 ± 7.61; the score of patients in the Operation Group was higher than that of small splint Group. CONCLUSION Compared with small splint external fixation, the method of open reduction and internal fixation for the treatment of humeral surgical neck fracture in aged patients has several advantages such as reducing pain, improving functional recovery, getting a better anatomical position and better clinical effects.

    Objective To investigate the effect of magnetic stimulation treatment to rats sufferring from sciatic nerve crush injury on motor conduction velocity(MCV) and the expression of growth associated protein(GAP-43) of the corresponding motor neurons.Methods Selected 60 SD rats,then divided into experimental Group(24 rats),control Group(24 rats) and sham Operation Group(12 rats) randomly.Crushed sciatic nerve to make models of peripheral nerve injury in the first two Groups.The experimental Group had magnetic stimulation on the wounds of sciatic nerve after crush.The control Group had invalid stimulation after crush.The sham Operation Group only exposed the unilateral sciatic nerve.The sciatic nerve of the three Groups were observed by means of electrophysiological at 2,4,8,12 weeks after opertation to study the effect of magnetic stimulation treatment on the expression of GAP-43 of L_4~L_5 motor neurons,and immunohistochemical staining was used at 12 weeks after opertation to study the MCV.Results The expression of GAP-43 in L_4~L_5 motor neurons of the first two Groups was higher than that of the control Group at the same time point after Operation(P0.05).The regenerated nerve of the experimental Group had faster conduction velocity,higher amplitude and shorter latency time than the control Group 12 months after the Operation(P0.05).Conclusion The magnetic stimulation can promote the recovery of MCV,the mechanism may up-regulate the GAP-43 expression of the motor neurons in the damage spinal cord.

    BACKGROUND: Anal fissure is a chronic condition characterized by painful defecation and rectal bleeding. The aim of this study was to compare injection of botulinum toxin versus surgical sphincterotomy for treatment of chronic anal fissure. PATIENTS AND METHODS: In a quasi-experimental trial in a university hospital in Kerman, 50 patients diagnosed with chronic anal fissure received 20 units botulinum toxin (n=25) or underwent lateral internal sphincterotomy (n=25). All patients were evaluated for pain, bleeding and healing of the fissure from one to six months later by another surgeon. The data was analyzed by SPSS software with the Mann-Whitney and Fisher’s exact tests. RESULTS: One month after treatment, the rate of healing and bleeding in the Operation Group was better than in the toxin Group (P<0.05), while pain was equal. After two months, none of the patients in either Group had complications. After six months follow-up, bleeding, pain and healing were better in the Operation Group. CONCLUSION: In the clinical evaluation, botulinum toxin is an effective alternative nonsurgical modality for the treatment of chronic anal fissure. We recommend botulinum toxin as the first step in treatment because of the 60% chance of cure with an easily performed treatment.

  • Effects of erythropoietin on the expression of 5-bromo-2-deoxyuridine in subgranular zone of neonatal rats with hypoxic- ischemic brain damage

    Objective To investigate the effect of erythropoietin( EPO)on the expression of 5- bromo- 2- deoxyuridine( Brd U) in subgranular zone( SGZ) of neonatal rats with hypoxic- ischemic brain damage( HIBD). Methods Animal HIBD model of seven- day- old newborn Wistar rat was made,and then the model rats were randomly divided into two Groups: the model Group of HIBD and the EPO trial Group( EPO 5 U·g- 1·d- 1for 14 d). The expressions of Brd U in dentate gyrus were examined with immunohistochemical staining and image quantitative analysis in fourteen days,twenty- first days and twenty- eight days after the Operation. Results The expression of Brd U in SGZ: The number of Brd U- positive cell in SGZ of hippocampal region was gradually decreasing accompany with the increasing age in three Groups,from 14 days to 28 days after Operation. The Brd U- positive cell were a significantly difference in the each Groups( P 0. 01). The number of Brd U- positive cell of the EPO trial Group was maximum,the model Group was second,the sham Operation Group was least. In fourteen days,twenty- first days after Operation,the number of Brd U- positive cell in SGZ of hippocampal region of the sham Operation Group,the model Groupand the EPO trial Group were 8. 08 ± 1. 62,25. 71 ± 4. 18,32. 21 ± 8. 63,respectively. By pairwise comparison,there is a significant difference in each Groups( P 0. 01) in twenty- first days. In twenty- eight days after the Operation,there was no significant difference between the model Group and the sham Operation Group( P 0. 05),but the significant differences were found between the model Group of HIBD and the EPO trial Group( P 0. 05). Conclusion Exogenous EPO could increase the expression of Brd U in SGZ of hippocampal region of neonatal rats with HIBD,it suggest that EPO could promote the neurogenesis in subgranular zone of hippocampal region of neonatal rats with HIBD,it may be involved in the regeneration of neurocytes and play an important role of neuroprotective effects in neonatal rats with HIBD.

    【Abstract】Objective: To investigate the clinical features and treatment strategy of dissymmetric bilateral frontal contusion, and to summarize our experience in treat-ing these patients by minimally invasive surgery. Methods: Over the past 3 years, we have treated a total of 31 patients with dissymmetric bilateral frontal contusion using endoscopy-assisted unilateral cerebral falx incision. Other 30 patients treated by routine bilateral approaches within the same period were taken as control. Results: Seventeen cases (54.8%) in the unilateral Operation Group survived and were in good condition, 8 (25.8%) had moderate disability, 4 (12.9%) had severe disability, 1 (3.2%) was in vegetative state, and 1 (3.2%) died. Compared with the control Group, the Glasgow Out-come Scale score was not significantly different in the uni-lateral Operation Group, but the Operation time, blood trans-fusion volume, the length of hospital stay, the incidences of mental disorder and olfactory nerve injury were greatly reduced in the unilateral Operation Group. Conclusions: Endoscopy-assisted unilateral cerebral falx incision can shorten the Operation time, reduce surgical trauma and complications in treatment of patients with dis-symmetric bilateral frontal contusion. It can obviously di-minish the chance of delayed intracerebral hematoma and subsequently minimize the incidences of subfalcial and centrencephalic herniation.
