    Abstract:

    Blunt fractures of the Pancreas are rare and serious lesions. An isolated injury to the Pancreas is uncommon. Physical signs and laboratory parameters are often inaccurate, and missing the diagnosis can cause serious clinical problems. This is a case report of blunt pancreatic trauma in whom computed tomography revealed a fracture through the body  of the Pancreas. It was complicated by Pseudocyst formation.   • The diagnosis and treatment of blunt pancreatic injuries:an analysis of 32 cases

    Abstract:

    Objective:To explore the diagnosis and treatment of blunt pancreatic injury.Methods:The clinical data of 32 cases with blunt pancreatic injury were retrospectively analyzed from Jan 2005 to Jul 2011.Among 32 cases,17 and 15 cases were diagnosed throngh operation and preoperative examination of B-ultrasonography and multi-layer CT respectively.Five cases with nonoperative treatment,5 cases with the tail of Pancreas and spleen resecting,15 cases with pancreatic debridement drainage,2 cases with duodenal diverticulization,4 cases with pancreatic head end embedding and far end jejunum Roux-en-Y anastomosis and 1 case with pancreaticoduodenal resection were implemented.Results:Thirty cases were cured and the cure rate was 93.75%.Two cases died and the fatality rate was 6.25%,which were caused by hemorrhage,infection,multiple organ failure and important organ injury.Three cases with pancreatic leakage were found and two cases cured by conservative treatment and 1 case cured by operating.Two cases of 4 cases with Pancreas Pseudocyst were self-healing,Its cured by puncturing drainage and operation.Conclusions:The multi-layer CT examination and exploratory laparotomy are the important method for diagnosis of pancreatic injury.The site and degrees of pancreatic injury and pancreatic duct damage are the important basis of treatment and the proper perioperative management can reduce the complications and fatality rate.

    Abstract:

    Introduction. Pancreatic Pseudocyst presented as pseudoaneurysm of the splenic artery is a potential serious complication in patients with chronic pancreatitis. Case report. A 42-year-old male patient with a long-standing evolution of chronic pancreatitis and 8-year long evolution of Pancreas Pseudocyst was referred to the Military Medical Academy, Belgrade due to worsening of the general condition. At admission, the patient was cachectic, febrile, and had the increased values of amylases in urine and sedimentation (SE). After clinical and diagnostic examination: laboratory assessment, esophagogastroduodenoscopy (EGDS), ultrasonography (US), endoscopic ultrasonography (EUS), multislice computed scanner (MSCT) angiography, pseudoaneurysm was found caused by the conversion of Pseudocyst on the basis of chronic pancreatitis. The patient was operated on after founding pancreatic Pseudocyst, which caused erosion of the splenic artery and their mutual communication. Postoperative course was duly preceded without complications with one year follow-up. Conclusion. Angiography is the most reliable and the safest method for diagnosing hemorrhagic Pseudocysts when they clinically present as pseudoaneurysms. A potentially dangerous complication in the presented case was treated surgically with excellent postoperative results.

    Abstract:

    A major complication of L-asparaginase used in the treatment of paediatric malignancies in children is pancreatitis (2%-16%). However, only seven paediatric cases of pancreatic Pseudocyst caused by the utilization of the agent have been reported in literature. We present the case of a 5-year old girl who had abdominal pain and epigastric dullness after the third course of BMF-95 protocol with a diagnosis of ALL. A pancreatic Pseudocyst of 10 x 10 cm size was found by abdominal tomography. The cyst was treated by percutaneous external drainage, total parenteral nutrition (TPN), administration of octreotide and antibiotherapy for one month. Percutaneous external drainage has proven to be an effective, noninvasive method in this special case with a systemic disorder and the high risk of mortality should a surgical intervention have been performed.
