    Abstract:

    [Objective] To find the better indicator in pre-stage renal injury in blood cadmium and urinary cadmium by the survey of the persons who were exposed to cadmium pollution around one factory in Huangshi City. [Methods] Collected blood and urine samples from 649 participators, and calculated areas under the curves of blood cadmium and urine cadmium by SPSS software. Besides, compared standardized Partial Regression Coefficient by Logistic Regression model. [Results] Among 649 samples, area under the ROC curves of blood cadmium (0.569) was not as large as that of urinary cadmium (0.725). And urine cadmium’s standardized Partial Regression Coefficient (0.5036) was 23.75 times than blood cadmium’s (-0.021 2) in logistic Regression model. [Conclusion] Urine cadmium is superior to blood cadmium in reflecting cadmium-reduced renal dysfunction.

    In normal weight subjects (body mass index G) is associated with the risk of NAFLD and/or renal dysfunction; however, the influence of the weight status on the associations remains unknown. We aimed to clarify the associations of the PNPLA3 polymorphism with the risk of NAFLD and/or renal dysfunction, while also paying careful attention to the weight status of the subjects. Cross-sectional and retrospective longitudinal studies with 5.5 ± 1.1 years of follow-up were conducted in 740 and 393 Japanese participants (61.2 ± 10.5 and 67.5 ± 6.0 years), respectively, during a health screening program. Among 591 subjects who did not have a habitual alcohol intake and/or hepatitis B or C virus infections, the PNPLA3 G/G genotype was associated with the risk for NAFLD in normal weight subjects [odds ratio (95% CI): 3.06 (1.11–8.43), P < 0.05]. Among all subjects, carriers of the PNPLA3 G/G genotype with a normal weight had a lower eGFR than those of the C/C genotype [Partial Regression Coefficient (SE): -3.26 (1.48), P < 0.05]. These associations were replicated in the longitudinal analyses. Among the overweight subjects, none of the genotypes were significantly associated in the cross-sectional and longitudinal analyses; however, the power of the analyses was small, especially in the analyses among overweight subjects. The findings of this study suggest that carriers of the PNPLA3 G/G genotype with a normal weight status should nevertheless be carefully monitored for the presence of NAFLD and/or renal dysfunction.

  • Development of the Path Model in Road Traffic Noise Annoyance of Dhanbad Township

    Abstract:

    A path model not only estimates the direct effect of traffic noise on nearby residents but also estimates the indirect effect via other variables. This paper highlights the development of a path model to determine the traffic noise annoyance around seven strategic locations in Dhanbad. The study considers only eight variables but it can be applicable to any number of variables. At the beginning of the analysis a priori path model was constructed and on the basis of the Partial Regression Coefficient value for the different paths, the revised path model was developed. The standardised Partial Regression Coefficients called path Coefficients, which show the strength of the linkage among variables, were determined to know the variables strongly related to traffic noise annoyance. Each path in the model was tested for statistical significance at 5% significance level.

    This paper elucidates the basic approach of determining the path Coefficients and its significance in the road traffic noise annoyance. Path model not only outline the direct effect of the traffic noise on the nearby residents but also indicate the indirect effect via other variables. In this study seven variables were considered for determining road traffic noise annoyance. However the same would be equally applicable for other situations like aircraft noise, rail noise, and industry noise with the different variables. At the outset a priori path model was designed and then on the basis of the Partial Regression Coefficient values for the different paths, the revised path model was developed. The standardized Partial Regression Coefficients known as path Coefficients, determine the strength of the linkage among variables. Some of the paths in the model were not statistically significant. Revised path models were developed by deleting the insignificant paths whose values were found above 5% level. In the revised path model, thus the direct and indirect effect due to a particular variable causing the road traffic noise annoyance could be observed.

    To clarify measures for making clearer instructions on how to read the results of general health checkups in a workplace by surveying the views of workers on these instructions, an anonymous self-administered questionnaire survey was conducted at a Japanese manufacturing plant. The responses of 984 male workers were analyzed. The average age of the subjects was 32.1 yr (range, 19-60 yr). Regarding the instructions, 4.1% of the subjects answered "definitely sufficient," 58.9% "somewhat sufficient," 28.0% "neither sufficient nor insufficient," 8.1% "somewhat insufficient" and 0.8% "definitely insufficient." Multiple Regression analysis showed that the degree of examinees' satisfaction significantly correlated with the health condition (standard Partial Regression Coefficient =0.189, p<0.001), knowledge of health management (standard Partial Regression Coefficient =0.095, p=0.014), and fear of lifestyle-related diseases (standard Partial Regression Coefficient =0.095, p=0.009). The adjusted R square value of the multiple Regression analysis was 0.064. It is necessary to provide more detailed information to those with poor health. Because the instructions contain many difficult medical terms, health professionals must give instructions that are easy to understand. Those feeling fear of lifestyle-related diseases may already know how to read checkup results. It is necessary to promote awareness of lifestyle-related diseases targeting people unfamiliar with such diseases.

    Method We analyzed 20 consecutive patients (14 male; age 64 ± 12 years) with first acute inferior MI due to proximal right coronary lesion. All patients were assessed with cardiac MRI after primary percutaneous coronary intervention and followed up for a median (25, 75 percentiles) of 6 (5, 6) months. We assessed the association of RV function with length of cardiac care unit (CCU) treatment as an index of short-term prognosis by using multiple Regression analysis, and used Kaplan-Meier analysis to demonstrate the association of RV dysfunction with occurrence of heart failure (HF) after discharge as an index of long-term prognosis. Results RV involvement was diagnosed with delayed enhancement MRI in 11 patients (55%). Patients with RV involvement had lower RV ejection fraction (EF) (32.4 ± 8.6 % vs 43.2 ± 7.7 %; p = 0.009) and longer duration of CCU treatment (5.3 ± 2.3 days vs 3.5 ± 1.9 days; p = 0.038) than those without RV involvement, although there were no significant differences regarding clinical, angiographic and the other MRI characteristics such as left ventricular parameters. By multiple Regression analysis, RVEF was an independent predictor of length of CCU treatment (standardized Partial Regression Coefficient -0.799, Partial Regression Coefficient -0.127; 95% confidence interval -0.222 to -0.031; p = 0.017).
