    Abstract:

    The progress of science in search of new techniques of the nerve regeneration and the functional repair in reinnervated Muscle has been the target of many researchers around the world. Consequently, nerves and Muscles in different body segments asked for more enlightenment of their morphology, their interrelation with other anatomic structures and their peculiarities. One of the most significant areas that need deeper studies is the region of the head and neck, since they are often affected by important pathologies. In order to offer the researcher`s community a morphological myoneural interaction model, this study elected the levator labii superioris Muscle and its motor nerve, the buccal branch of the facial nerve (VII pair) not only for its special characteristics, but also its value on the facial expression. The rat was chosen for this investigation for being easy to obtain, to keep, to manipulate and to compare this experiment with many others studies previously published. The techniques used were Mesoscopic (dissection), histoenzymologic and morphometric ones. In the results the Muscle proved to have a predominance of fast twich fibers (FG and FOG) and superficial location, with a proximal bone and a distal cutaneous insertion. Its motor nerve, the buccal branch of the facial nerve (VII pair), breaks through the Muscle belly into its deep face, and comprised a heterogeneous group of myelinic nerve fibers disposed in a regular form in all fascicle. Near the motor point, the nerve showed to be composed of two fascicles with different sizes. Due to the small nerve dimensions, the nerve fibers have a smaller diameter if compared to the motor nerve of Pectineus Muscle of the cat. Further studies with neural tracers have already had a start in order to provide more information about the distribution and the architecture of these fibers.FAPES

    Abstract:

    Background and Objectives: This study was conducted to provide a thorough description of the variability in the obturator nerve branching pattern in the inguinal region. Methods: The anatomic variability of obturator nerve branching among 84 dissected embalmed cadavers was investigated. Ultrasound examination of the inguinal region was undertaken in 20 cases and the location of the obturator nerve was identified. Results: The point of division for the obturator nerve into the anterior and posterior branches was intrapelvic (23.22%), within the obturator canal (51.78%), or in the thigh (25%). Most commonly, the anterior branch was divided among 3 major muscular branches (66.66%) that innervated the adductor longus, adductor brevis, and gracilis Muscles. Four, and 2 subdivisions of the anterior branches were observed, in 4.76% and 28.57% of cases, respectively. The posterior branch predominantly separated into 2 divisions (60.11%), which provided innervation to the adductor brevis and adductor magnus Muscles. In addition, either 1 (13.69%), 3 (19.04%), or 4 (7.14%) muscular divisions of the posterior branch were observed. The articular branch of the obturator nerve showed 9 different branching patterns, which most frequently arose from the common obturator nerve. The fascias medial to the femoral vessels and deep to the Pectineus Muscle were clearly visualized (100%) by ultrasound imaging. This region was used as an "imaging" landmark for localization (success rate of 80%) of the common obturator nerve. Conclusions: High anatomic variability in the obturator nerve9s divisions and subdivisions does exist, and explains the difficulty frequently encountered in the application of regional anesthetic techniques.

    Abstract:

    In regenerative medicine, new approaches are required for the creation of tissue substitutes, and the interplay between different research areas, such as tissue engineering, microsurgery and gene therapy, is mandatory. In this article, we report a modification of a published model of tissue engineering, based on an arterio-venous loop enveloped in a cross-linked collagen-glycosaminoglycan template, which acts as an isolated chamber for angiogenesis and new tissue formation. In order to foster tissue formation within the chamber, which entails on the development of new vessels, we wondered whether we might combine tissue engineering with a gene therapy approach. Based on the well-described tropism of adeno-associated viral vectors for post-mitotic tissues, a muscular flap was harvested from the Pectineus Muscle, inserted into the chamber and transduced by either AAV vector encoding human VEGF165 or AAV vector expressing the reporter gene \u3b2-galactosidase, as a control. Histological analysis of the specimens showed that Muscle transduction by AAV vector encoding human VEGF165 resulted in enhanced tissue formation, with a significant increase in the number of arterioles within the chamber in comparison with the previously published model. Pectineus muscular flap, transduced by adeno-associated viral vectors, acted as a source of the proangiogenic factor vascular endothelial growth factor, thus inducing a consistent enhancement of vessel growth into the newly formed tissue within the chamber. In conclusion, our present findings combine three different research fields such as microsurgery, tissue engineering and gene therapy, suggesting and showing the feasibility of a mixed approach for regenerative medicine
