    Children with perinatally acquired parenchymal haemorrhage are thought to have better visual Perceptual skills than those with leukomalacia. We examined seven prematurely born children with parenchymal haemorrhage and 14 with grades 2-4 leukomalacia, at the age of 5 years. Clinical and magnetic resonance imaging parameters were related to visual Perceptual performance assessed with the L94, using performance age. Belonging to the leukomalacia group, the inability to walk, a diminished peritrigonal white matter, a high degree of gliosis and cortical damage were associated with poorer visuo-Perceptual skills. Enlarged lateral ventricles, confirming the findings of Melhelm (Radiology 214 (2000) 199), were associated with both cognitive, Perceptual and motor problems and probably reflect the considerable extent of the brain damage. Specific factors protecting against visual Perceptual Impairment were a preserved volume of the right optical radiation and of the splenium of the corpus callosum. Children with leukomalacia are at considerable risk of visual Perceptual Impairment. Children with right-sided parenchymal haemorrhages also appear to be at risk although they function much better due to better motor and cognitive skills.

    Reduced non-verbal compared to verbal intelligence is used in many outcome studies of perinatal complications as an indication of visual Perceptual Impairment. To investigate whether this is justified, we re-examined data sets from two previous studies, both of which used the visual Perceptual battery L94. The first study comprised 47 children at risk for cerebral visual Impairment due to prematurity or birth asphyxia, who had been administered the McCarthy Scales of Children's abilities. The second study evaluated visual Perceptual abilities in 82 children with a physical disability. These children's intellectual ability had been assessed with the Wechsler Intelligence Scale for Children-Revised and/or Wechsler Pre-school and Primary Scale of Intelligence-Revised. No significant association was found between visual Perceptual Impairment and (1) reduced non-verbal to verbal intelligence; (2) increased non-verbal subtest scatter; or (3) non-verbal subtest profile deviation, for any of the intelligence scales. This result suggests that non-verbal intelligence subtests assess a complex of cognitive skills that are distinct from visual Perceptual abilities, and that this assessment is not hampered by deficits in Perceptual abilities as manifested in these children.

Relative to their performance level on non-verbal intelligence subtests, 37.5% of the children were impaired on at least one task, and 18.7% on two or more tasks. No child was impaired on the visuoconstruction tasks. Visual-Perceptual Impairment was highest among six children with brain malformation (67%), followed by spastic CP (40%), and brain damage acquired after the first year of life (38%). There was no difference in visual-Perceptual Impairment between the subtypes of spastic CP. Results are not secondary to visual acuity deficits, as only one L94 task was significantly correlated with acuity Impairment. We conclude that visual-Perceptual Impairment is frequent in children with physical disabilities, and not restricted to children with CP of hypoxic-ischaemic origin.

    To study the selectivity of visual Perceptual Impairment in children with early brain injury, eight visual Perceptual tasks (L94), were administered to congenitally disabled children both with and without risk for cerebral visual Impairment (CVI). The battery comprised six object-recognition and two visuoconstructive tasks. Seven tasks were newly designed. For these normative data are presented (age 2.75-6.50 years). Because the recognition tasks required object naming, each item included a canonical control drawing and visual Perceptual ability was evaluated relative to the non-verbal intelligence level, instead of chronological age. In 22 multiple disabled children with no indications of CVI, the frequency of Impairment did not exceed that in the reference sample for any L94 task. In contrast, in 57 5-year-old children who were at risk for CVI due to pre-maturity or birth asphyxia, a significant increase in the frequency of Impairment was seen on six L94 tasks (range 12-38%). However, only five children had more than two Impairments, indicating that the deficits were selective, not pervasive. We conclude that early brain lesions interfere with the functioning of particular visual subsystems, yet leave other subsystems intact and functioning within the normal range.

    Previous studies have shown impaired memory for faces following restricted sleep. However, it is not known whether lack of sleep impairs performance on face identification tasks that do not rely on recognition memory, despite these tasks being more prevalent in security and forensic professions—for example, in photo-ID checks at national borders. Here we tested whether poor sleep affects accuracy on a standard test of face-matching ability that does not place demands on memory: the Glasgow Face-Matching Task (GFMT). In Experiment 1, participants who reported sleep disturbance consistent with insomnia disorder show impaired accuracy on the GFMT when compared with participants reporting normal sleep behaviour. In Experiment 2, we then used a sleep diary method to compare GFMT accuracy in a control group to participants reporting poor sleep on three consecutive nights—and again found lower accuracy scores in the short sleep group. In both experiments, reduced face-matching accuracy in those with poorer sleep was not associated with lower confidence in their decisions, carrying implications for occupational settings where identification errors made with high confidence can have serious outcomes. These results suggest that sleep-related Impairments in face memory reflect difficulties in Perceptual encoding of identity, and point towards metacognitive Impairment in face matching following poor sleep.

    We set out to define visuo-Perceptual Impairment related to periventricular leukomalacia (PVL) using the Developmental Test of Visual Perception (DTVP). Correlations were sought between visual–Perceptual deficits and DTVP profile and neuroradiological and neurophthalmological findings. The DTVP was administered to 20 children (m/f: 10/10), aged between 5 and 8 years (mean: 6.95 years), presenting with: spastic diplegia; PVL documented by brain MRI; normal or mildly impaired visual acuity; mild-moderate upper limb functional Impairment. The mean General Visual–Perceptual Quotient was impaired, showing a great variability among the patients. Despite this, an uneven DTPV profile, characterised by a significant difference between the VMIQ and the Non-Motor Visual–Perceptual Quotient (P<0.001), and a poor result on the Closure subtest (identification of whole figures from incomplete visual information) was observed in all the subjects. This profile reflects a deficit in eye–hand coordination and in praxic-constructional abilities and could be the expression of malfunctioning of the occipital–parietal pathway of visual integration, the so-called ‘dorsal stream,’ a hypothesis reinforced by the emergence of a statistically significant correlation between the neuroradiological data and the presence of visual–Perceptual Impairment.

    Aim Visual perception is one of the cognitive functions often impaired in children with cerebral palsy (CP). The aim of this systematic literature review was to assess the frequency of visual–Perceptual Impairment (VPI) and its relationship with patient characteristics. Method Eligible studies were relevant papers assessing visual perception with five common standardized assessment instruments in children with CP published from January 1990 to August 2011. Results Of the 84 studies selected, 15 were retained. In children with CP, the proportion of VPI ranged from 40% to 50% and the mean visual perception quotient from 70 to 90. None of the studies reported a significant influence of CP subtype, IQ level, side of motor Impairment, neuro-ophthalmological outcomes, or seizures. The severity of neuroradiological lesions seemed associated with VPI. The influence of prematurity was controversial, but a lower gestational age was more often associated with lower visual motor skills than with decreased visual–Perceptual abilities. Interpretation The Impairment of visual perception in children with CP should be considered a core disorder within the CP syndrome. Further research, including a more systematic approach to neuropsychological testing, is needed to explore the specific impact of CP subgroups and of neuroradiological features on visual–Perceptual development.
