Permanent Tooth agenesis in individuals with non syndromic robin sequence a systematic review and meta analysis

The outcomes desired were prevalence rates and patterns of Permanent Tooth agenesis. The final search was carried out in January 2017. RESULTS: Six articles were selected for inclusion, with a total of 448 individuals with ns-RS. Data available in each study varied which allowed conducting further specific meta-analyses only on sample sizes of 339 or 378 individuals from three or four of the six included articles. The overall estimated prevalence rate of Permanent Tooth agenesis was 42% (95% CI: 35%-48%), excluding third molars, with no sex predilection. In individuals with Tooth agenesis, approximately 30% had one missing Tooth while 40% had two missing teeth. Tooth agenesis was more commonly bilateral (Relative Risk 2) and found in the mandible (Relative Risk 1.6). The highest prevalence of Permanent Tooth agenesis was found for mandibular second premolars (26%) followed by maxillary second premolars (14%). The most common Tooth agenesis patterns were the agenesis of both mandibular second premolars, followed by the agenesis of all second premolars. CONCLUSION: Individuals with ns-RS have high prevalence rates of Permanent Tooth agenesis. The most prevalent Tooth agenesis patterns are bilateral symmetric Tooth agenesis, most commonly agenesis of both mandibular second premolars.

The aims of this study were to characterize Permanent Tooth agenesis patterns and to determine their prevalence in patients with nonsyndromic PRS. Methods Radiographs and clinic charts of 146 children with nonsyndromic PRS were examined for Permanent Tooth agenesis, excluding third molars, and the Tooth agenesis patterns were identified with the Tooth agenesis code. Results The overall prevalence of Permanent Tooth agenesis was 32.9% (48 of 146 patients), with about two thirds having bilateral Tooth agenesis. No sex or racial predilection was found for Tooth agenesis. The most common Tooth agenesis pattern was the absence of both mandibular second premolars. Among the children with Tooth agenesis, almost half had agenesis patterns involving both mandibular second premolars. Other common patterns of Permanent Tooth agenesis involved the maxillary lateral incisors and the maxillary premolars. Conclusions Symmetric agenesis of individual Tooth types occurs much more frequently in the mandible than in the maxilla in patients with nonsyndromic PRS. Bilateral absence of the mandibular second premolars is the predominant pattern of dental agenesis.

Two independent investigators carried out a literature search to locate articles pertaining to Permanent Tooth agenesis in individuals with Down syndrome. The outcomes of interest were the prevalence and pattern of Permanent Tooth agenesis. Thirteen articles were selected for inclusion. The estimated overall prevalence of Permanent Tooth agenesis, excluding third molars, was 54.6% (95% CI: 44-66%). Considerable heterogeneity was present across the studies. Among subjects with Tooth agenesis, approximately half had three or more missing teeth. The Permanent teeth with the highest prevalence of agenesis were the maxillary lateral incisor (27%), mandibular second premolar (21%) and maxillary second premolar (18%). The single most commonly absent Tooth, however, was the mandibular left second mandibular premolar (19.9%), followed by the maxillary left lateral incisor (19.4%). Little data describing specific Tooth-agenesis patterns were available. In conclusion, individuals with Down syndrome display high prevalence rates and severity of agenesis of Permanent teeth. Proper and timely diagnosis of missing Permanent teeth is thus necessary, to allow a more comprehensive long-term treatment plan and a more favorable prognosis in these individuals.

Effect of exogenous stem cells from apical papillae in the pulp revascularization treatment for the immature Permanent Tooth with periapical periodontitis
Chinese journal of stomatology, 2018Co-Authors: Jie Yang, Yuming Zhao, Wenjun Wang, Lihong GeAbstract:Objective To evaluate the effect of exogenous stem cells from apical papillae (SCAP) in the pulp revascularization treatment for the immature Permanent Tooth with periapical periodontitis in animal model. Methods After the SCAP were isolated and cultured from the Beagle dogs, stem cell properties of these cells were characterized by analyzing their colony-forming ability, the expression of mesenchymal stem cell markers and the multidifferentiation characteristics including osteogenic, adipogenic, and chondrogenic potentials. Models of young Permanent Tooth with periapical periodontitis were established in dogs and the infection in each of the model Tooth was eliminated by root canal irrigation and intracanal medication. After that, all of the model teeth were randomly divided into 4 groups: Group 1: normal developing teeth with no treatment applied;Group 2: teeth that periapical tissues were irritated to induce blood flowing into the root canals;Group 3: teeth that peripheral blood was delivered into the root canals;Group 4: teeth that SCAP were resuspended in peripheral blood and delivered into the root canals. In Group 2-4, firm coronal seal was performed after revascularization procedure and radiographs were taken periodically in order to observe the development of roots. After a 12-week-period, alveolar samples were collected and observed histologically. Results The isolated SCAP showed clonogenic ability and multilineage differentiation ability including osteogenic, adipogenic and chondrogenic potentials. These cells also expressed the mesenchymal stem cell markers such as STRO-1 and CD146, while no cytokeratin was detected. The thickening of canal wall was observed radiographically 12 weeks after procedures of infection control and revascularization. Histologically, the newly formed tissues on the inner canal wall were found bone lacuna like structure in Group 2 and 3, and the new tissue formed in the Group 3 seemed easy to separate from the canal wall. The newly formed tissues in Group 4 were much thicker compare to those in the Group 2 and 3, and the dentine tubule like structure instead of bone lacuna was noticed although the orientation of these tubules were various. Conclusions SCAP seem to play an important role in the tissue regeneration procedure when infection is well controlled in young Permanent teeth with periapical periodontitis. It is difficult to achieve real tissue regeneration due to the lack of endogenous SCAP in apical area, therefore delivering adequate exogenous SCAP isolated and cultured in vitro could be a promising approach to overcome the challenge. The role of pulp in the root resorption of primary teeth without Permanent Tooth germs

Methods The animal model without Permanent Tooth germs was established by surgery in Beagle dog. The root resorption was observed by taking periapical radiographs periodically. The samples of mandibular bone and pulp at different resorption stages were collected. The distribution of odontoclasts and the activating factor was analyzed by histological staining and semiquantitative reverse transcriptase polymerase chain reaction(RT-PCR). The role of pulp in the root resorption of primary teeth was tested by early pulpectomy. Results In the root resorption of primary molars without Permanent teeth germs, a large number of odontoclasts were present on the pulpal surface of the root canal. Semiquantification RT-PCR showed that the ratios of the expression of receptor activator of NF-κB ligand (RANKL) mRNA and β-actin in the pulp of Permanent teeth and primary teeth without Permanent teeth germ during different periods of root resorption are 0. 1314, 0. 1901, 0. 2111 and 0. 6058 ( P > 0. 05 ). The root resorption of primary teeth without Permanent teeth germs in test groups was about 5 weeks later than that of control group. Conclusions The pulp of primary Tooth played an important role in the root resorption of primary Tooth without Permanent Tooth germ. Establishment of Permanent Tooth germ missing animal model and study on root resorption of the corresponding deciduous teeth

Methods: The Permanent Tooth germ missing animal model was established by surgical removal of the Permanent Tooth buds in a male 11-week-old Beagle dog. Root resorption of the deciduous teeth without Permanent successors was observed by taking periapical films periodically,and compared with physiological root resorption. Once the sign of root resorption of the deciduous teeth without Permanent successors was detected on radiographic films, the animal was sacrificed and the mandibular bone was collected for histological study. Results: Root resorption of the deciduous teeth with the presence of Permanent Tooth germ started at 20 weeks after birth, while root resorption of deciduous teeth without Permanent Tooth germ started 26-27 weeks which was significantly delayed. Histological studies showed that a large number of multinucleated giant cells were present on the pulpal surface of the root, while only few of them were seen on the outer surface. Conclusion: The Permanent Tooth germ missing animal model was successfully established in this study which simulated the case of congenital absence of Permanent Tooth germ in human. Root resorption of deciduous Tooth without Permanent Tooth germ was significantly delayed than the deciduous Tooth with Permanent Tooth germ.

Permanent Tooth development in children with cleft lip and palate
Pediatric Dentistry, 2008Co-Authors: Adena F Borodkin, Robert J Feigal, Soraya Beiraghi, Karlind T Moller, James S HodgesAbstract:PURPOSE: The purposes of this study were to: (1) quantify and compare Permanent Tooth development of cleft lip and palate (CLP) patients to age- and gender-matched controls; (2) relate these findings to cleft type and severity; and (3) examine delays in individual Permanent maxillary teeth related to their proximity to the cleft. METHODS: Standardized methods using panoramic radiographs were employed to stage dental development and dental age for 49 children with clefts and 49 matched controls. Data were analyzed with a mixed linear model. RESULTS: Analyses indicated a correlation between delayed Permanent Tooth development and CLP with an overall delay of 0.52 years (P = .02) and with boys accounting for all the delay. No differences were found between subjects with unilateral or bilateral clefts. A nonsignificant trend was noted for greater delay in subjects with clefts of the primary and secondary palates vs primary palate alone. Teeth most often affected by the delay were maxillary first and second premolars and maxillary second molars. CONCLUSIONS: While Permanent Tooth development is delayed in cleft lip and palate patients, this delay is: found in boys only; is independent of the cleft type and severity; and is not correlated with proximity to the cleft.
