2005Co-Authors: Klilah Hershko, Ido Weinberg, Arieh IngberAbstract:A relationship between sensitivity to poison oak or poison ivy and mango Dermatitis has been suggested by previous publications. The observation that acute allergic contact Dermatitis can arise on first exposure to mango in patients who have been sensitized beforehand by contact with other urushiol-containing Plants has been documented previously. We report 17 American patients employed in mango picking at a summer camp in Israel, who developed a rash of varying severity. All patients were either in contact with poison ivy/oak in the past or lived in areas where these Plants are endemic. None recalled previous contact with mango. In contrast, none of their Israeli companions who had never been exposed to poison ivy/oak developed mango Dermatitis. These observations suggest that individuals with known history of poison ivy/oak allergy, or those residing in area where these Plants are common, may develop allergic contact Dermatitis from mango on first exposure. We hypothesize that previous oral exposure to urushiol in the local Israeli population might establish immune tolerance to these Plants.

comparison of patch test contact sensitivity to acetone and aqueous extracts of parthenium hysterophorus in patients with airborne contact Dermatitis
2004Co-Authors: Vinod K Sharma, Gomathy Sethuraman, Trilokraj TejasviAbstract:Our aim was to compare the degree of patch test positivity to acetone and aqueous extracts of Parthenium hysterophorus in patients with airborne contact Dermatitis. We performed patch testing with the Indian standard series (which includes aqueous extracts of parthenium, xanthium and chrysanthemum), and with 1 : 100 and 1 : 200 dilutions of an acetone extract of parthenium in 72 patients with airborne contact Dermatitis. All patients showed contact sensitivity to the 1 : 100 dilution and 67 patients had positive allergic reactions to the 1 : 200 dilution of the acetone extract, whereas only 45 patients showed a positive reaction to the aqueous extract of P. hysterophorus. Our results confirm that parthenium allergens are more soluble in acetone than in water, and that the acetone extract is significantly better in detecting contact sensitivity to parthenium in patients with suspected Plant Dermatitis. Hence, the acetone extract is recommended for routine patch testing.

Parthenium Dermatitis in India: Past, present and future
2012Co-Authors: Vinod K Sharma, Parul VermaAbstract:Parthenium Dermatitis is an immuno-inflammatory disease caused by Parthenium hysterophorus and is the commonest cause of Plant Dermatitis in India. It is caused by airborne dry and friable Plant particles including trichomes, and the most important allergens responsible for allergic contact Dermatitis are sesquiterpene lactones. The combined type IV and type I hypersensitivity to parthenium has been recently postulated. In sensitized individuals, it can cause a spectrum of clinical patterns, such as classical airborne pattern, chronic actinic Dermatitis-like presentation, mixed pattern Dermatitis, exfoliative Dermatitis, widespread Dermatitis, and other rare patterns. There is definite trend towards change from airborne pattern to chronic actinic pattern in natural history of parthenium Dermatitis. Contact sensitivity to parthenium is everlasting, and hence the disease runs a chronic course with exacerbation during summers. Patch testing with acetone or aqueous Plant extract is the simplest way of confirming parthenium contact allergy. Management includes avoiding contact with allergen, managing Dermatitis with topical corticosteroids/tacrolimus, and other immunosupressives like azathioprine. In future, we expect parthenium Dermatitis to become less prevalent due to rapid urbanization and possible development of new biological methods to eradicate the parthenium.
