Análisis de costo-efectividad y costo-beneficio Programa de Detección Temprana de Cáncer Gástrico en Costa Rica

In the second part, a stated-Preference Method was employed, using surveys, which allows us to compare costs and benefits of this programme. We took the willingness to pay of the participants in this programme of the Caja Costarricense de Seguro Social as the basis for this analysis. If a comparison of the costs to save one life and the value of a statistical life saved were taken as a criterion for evaluating this programme, we would find a strong justification for this programme, if the productivity and effectiveness of the screening are sufficient. In order to reach this level of productivity and effectiveness, in terms number of members of high risk groups screened per day, several improvements in the organization of the programme are required. In case these improvements are not carried out, a weaker justification in terms of benefits for society for this programme exists. In this study, EOs from Tagetes lucida , Lepechinia betonicifolia , Lippia alba , Cananga odorata , and Rosmarinus officinalis , species grown in Colombia, were analyzed by gas chromatography-mass spectrometry. These oils as well as several of their constituents were tested for repellent activity against Tribolium castaneum , using the area Preference Method. The main components (>10%) found in EOs were methylchavicol, limonene/α-pinene, carvone/limonene, benzyl acetate/linalool/benzyl benzoate, and α-pinene, for T. lucida, L. betonicifolia, L. alba, C. odorata, and R. officinalis, respectively. All EOs were repellent, followed a dose-response relationship, and had bioactivity similar to or better than that of commercial compound IR3535. EOs from C. odorata and L. alba were the most active. Compounds from EOs, such benzyl benzoate, β-myrcene, and carvone, showed good repellent properties. In short, EOs from plants cultivated in Colombia are sources of repellents against T. castaneum.

Most oil components were oxygenated monoterpenoids or phenolic compounds. Six oils were repellent, with Lippia origanoides the most active. Eucalyptus citriodora and Tagetes lucida were also repellant at doses between 0.063 and 0.503 μL/cm 2 .

However, the FPR suffers from an intrinsic limitation of consistency in decision making. In this regard, many researchers proposed the consistent fuzzy Preference relation (CFPR) as a decision-making approach. Nevertheless, most CFPR Methods involve a traditional aggregation process which does not identify the interrelationship between the criteria of decision problems. In addition, the information provided by individual experts is indeed related to that provided by other experts. Therefore, the interrelationship of information on criteria should be dealt with. Based on this motivation, we propose a modified approach of CFPR with Geometric Bonferroni Mean (GBM) operator. The proposed Method introduces the GBM as an operator to aggregate information. The proposed Method is applied to a case study of assessing the quality of life among the population in Setiu Wetlands. It is shown that the best option derived by the proposed Method is consistent with that obtained from the other Methods, despite the difference in aggregation operators.

Frontiers in Pharmacology, 2019Co-Authors: Eline Van Overbeeke, Chiara Whichello, Rosanne Janssens, Karin Scholin Bywall, Jenny Sharpe, Nikoletta Nikolenko, B S Phillips, Paolo Guiddi, Gabriella PravettoniAbstract:Objectives: To investigate stakeholder perspectives on how patient Preference studies (PPS) should be designed and conducted to allow for inclusion of patient Preferences in decision-making along the medical product life cycle (MPLC), and how patient Preferences can be used in such decision-making. Methods: Two literature reviews and semi-structured interviews (n = 143) with healthcare stakeholders in Europe and the US were conducted; results of these informed the design of focus group guides. Eight focus groups were conducted with European patients, industry representatives and regulators, and with US regulators and European/Canadian health technology assessment (HTA) representatives. Focus groups were analyzed thematically using NVivo. Results: Stakeholder perspectives on how PPS should be designed and conducted were as follows: 1) study design should be informed by the research questions and patient population; 2) preferred treatment attributes and levels, as well as trade-offs among attributes and levels should be investigated; 3) the patient sample and Method should match the MPLC phase; 4) different stakeholders should collaborate; and 5) results from PPS should be shared with relevant stakeholders. The value of patient Preferences in decision-making was found to increase with the level of patient Preference sensitivity of decisions on medical products. Stakeholders mentioned that patient Preferences are hardly used in current decision-making. Potential applications for patient Preferences across industry, regulatory and HTA processes were identified. Four applications seemed most promising for systematic integration of patient Preferences: 1) benefit-risk assessment by industry and regulators at the marketing-authorization phase; 2) assessment of major contribution to patient care by European regulators; 3) cost-effectiveness analysis; and 4) multi criteria decision analysis in HTA. Conclusions: The value of patient Preferences for decision-making depends on the level of collaboration across stakeholders; the match between the research question, MPLC phase, sample, and Preference Method used in PPS; and the sensitivity of the decision regarding a medical product to patient Preferences. Promising applications for patient Preferences should be further explored with stakeholders to optimize their inclusion in decision-making.
