    Abstract:

    Background The 2016 International Consensus Statement on Allergy and Rhinology: Rhinosinusitis (ICAR:RS) is a collaborative distillation of available research and consensus recommendations for the management for chronic rhinosinusitis (CRS). However, implementation of the ICAR:RS recommendations in the reality of clinical practice is not clearly defined. Methods An anonymous, web-based survey of the American Rhinologic Society membership was performed in October, 2018. Respondents were asked about the frequency that they recommended the various treatments reviewed in ICAR:RS in the context of medical management for CRS. A 7-point Likert-type scale assessed the frequency of treatment patterns. Results A total of 140 members completed the survey (response rate 11.9%). Seventy-two (51.4%) were in practice for 0 to 15 years, 61 (43.6%) completed a Rhinology fellowship, and 73 (52.1%) worked in private practice. Disparate treatment patterns were reported for each of the therapies assessed for CRS, including those that were "recommended" or "recommended against" in ICAR:RS. Members with 100 sinus surgeries per year were more likely to use topical antibiotics. Conclusion The range of reported treatment patterns identified in this study despite the availability of the ICAR:RS recommendations may suggest the need for improved standardization of CRS management.

    Background Rhinology has rapidly evolved as a subspecialty over the past decade. The professional activities of Rhinology faculty in otolaryngology residency programs is an important defining feature of this process but remains incompletely understood. Methods An examination of faculty profiles of otolaryngology residency programs in the United States was performed to examine the professional activities of rhinologists. An anonymous, web-based survey of Rhinology faculty was also performed to query professional activities and career satisfaction. Results Nine percent of chairmen and 12% of residency program directors were rhinologists. The number of full-time Rhinology faculty members varied significantly among departments (mean 1; range, 0-4). Rhinology faculty members were noted to have a high number of scientific publications over the past 5 years (mean 15 per faculty), a high level of membership to the American Rhinologic Society (90%) and modest levels of membership to other societies. As reported by the 45 respondents who successfully completed the survey, higher percentages of professional time was devoted to clinical medicine when compared with administrative and research activities. Inflammatory sinusitis represented the most common clinical condition treated, and there was variability with respect to other disorders and procedures. Career satisfaction scores were highest for medical and surgical care, teaching activities, financial and emotional well being, and overall career to date. Lower satisfaction scores were noted for research and administrative activities and for balance of personal life with work. Conclusion This study further defines the professional, clinical, and surgical activities of academic rhinologists. Continued analysis of the subspecialization of Rhinology is required.

    Background The past decade has witnessed the emergence of Rhinology as a distinct subspecialty within otolaryngology. A critical evaluation of this process is necessary to define the field and to determine the impact of subspecialization on residency education. Methods An Internet-based survey of otolaryngology residency chairmen was performed to query attitudes about the importance of various aspects of rhinologic education including didactics, presence of Rhinology faculty, exposure to research, office-based Rhinology, and various surgical procedures. A 7-point Likert scale was used to rate responses. Results A 30.2% response rate was noted for the chairmen survey. From the chairmen's perspective, overall high levels of importance were ascribed to Rhinology education and surgical training in otolaryngology residency. Lower scores for importance were noted for basic science research, exposure to office-based allergy care, and advanced surgical procedures. High scores were noted for the importance of fellowship training in the professional development of a resident interested in pursuing a career in Rhinology. Conclusion Overall high levels of importance were ascribed by the participating chairmen to the importance of Rhinology education in residency. As the subspecialty of Rhinology continues to develop, the role of the various educational, research and clinical activities will require continued evaluation.

    Objectives To define the educational goals and determine the success of formal fellowship programs in Rhinology. Design An anonymous, Internet-based survey of current Rhinology fellows and those from the past 5 years. Main Outcome Measure A 5-point ordinal Likert scale was used, with higher scores being more favorable. Results Complete responses were collected from 46 of 70 eligible participants (66%), representing 19 fellowship programs. High overall satisfaction with the fellowship experience was reported (mean score, 4.7). Pooled scores for comfort levels with the management of medical issues (mean, 4.8) and surgical procedures (mean, 4.5) were also positive. Following completion of training, lesser levels of comfort were associated with craniofacial procedures ( P P P  = .002) compared with all surveyed procedures. Respondents reported a greater interest in (mean score, 4.3 vs 2.4; P P Conclusions The Rhinology fellowship experience appears to be generally favorable in terms of meeting stated training goals and helping fellows achieve subjective comfort with medical and surgical management of rhinological disorders. Continued discussion of the goals of Rhinology fellowship training is necessary.

After the successful launch of EPOS2020 in Spring 2020, the editorial team of Rhinology is proud to present to you the latest and most exciting data in Rhinology research. Getting insight into the complexity and relevance of proteomics in CRS, epithelial-mesenchymal contribution to CRS, zinc levels in nasal and systemic compartment of CRS, nasal biomarkers of CRSwNP that predict recurrence of disease after sinus surgery, and the odor identification test for children, called "U-Sniff", and FID scores (Frequency, Intensity and Duration) scores for epistaxis are all in the 2020 October issue and highly relevant for Rhinology practice. These studies build further on the solid grounds of previous Rhinology research meeting the unmets needs in the field.

Physicians treating patients with rhinologic disorders might find it extremely interesting to find reports on predictors of success of medical or surgical interventions. Prediction of success of treatment is crucial to allow the patient become an active partner in the decision-making process of medical or surgical treatment. Defining the clinical phenotype of patients with chronic rhinosinusitis responding well to either revision surgery or long-term oral macrolide treatment represents a true challenge.

    The first Rhinology Future Debates was held in Brussels in December 2016, organized by EUFOREA (European Forum for Research and Education in Allergy and Airways diseases). The purpose of these debates is to bring novel developments in the field of Rhinology to the attention of the medical, paramedical and patient community, in a highly credible and balanced context. For the first time in Rhinology, a peer to peer scientific exchange with key experts in the field of Rhinology and key medical colleagues from leading industries let to a brainstorming and discussion event on a number of hot issues in Rhinology. Novel developments are presented by key experts from industry and/or key thought leaders in Rhinology, and then followed by a lively debate on the potential positioning of new developments in care pathways, the strengths and weaknesses of the novel development(s), and comparisons with existing and/or competing products, devices, and/or molecules. As all debates are recorded and distributed on-line with limited editing (www.Rhinology-future.com), EUFOREA aims at maximizing the education of the target groups on novel developments, allowing a critical appraisal of the future and a more rapid implementation of promising novel tools, techniques and/or molecules in clinical practise in Europe. The next Rhinology Future debate will be held in Brussels in December 2017.

This article aims to provide recommendations and relevant information for rhinologists, based on the limited amount of (anecdotal) data, to guarantee high-quality patient care and adequate levels of infection prevention in the Rhinology clinic.

    The first European Rhinology Research Forum organized by the European Forum for Research and Education in Allergy and Airway Diseases (EUFOREA) was held in the Royal Academy of Medicine in Brussels on 17th and 18th November 2016, in collaboration with the European Rhinologic Society (ERS) and the Global Allergy and Asthma European Network (GA2LEN). One hundred and thirty participants (medical doctors from different specialties, researchers, as well as patients and industry representatives) from 27 countries took part in the multiple perspective discussions including brainstorming sessions on care pathways and research needs in rhinitis and rhinosinusitis. The debates started with an overview of the current state of the art, including weaknesses and strengths of the current practices, followed by the identification of essential research needs, thoroughly integrated in the context of Precision Medicine (PM), with personalized care, prediction of success of treatment, participation of the patient and prevention of disease as key principles for improving current clinical practices. This report provides a concise summary of the outcomes of the brainstorming sessions of the European Rhinology Research Forum 2016.

    BACKGROUND Ear symptoms are common among patients presenting to a Rhinology clinic. Validated inventories are available for patient quality-of-life in sinonasal disease and Eustachian tube dysfunction (ETD). This study sought to determine the extent of ETD symptoms, using validated metrics, in a large population of patients presenting to a tertiary Rhinology clinic. METHODS Seven-item Eustachian Tube Dysfunction Questionnaires (ETDQ-7) and 22-item Sino-Nasal Outcome Tests (SNOT-22) were prospectively collected from 492 patients treated in a tertiary Rhinology clinic. Patient diagnoses were retrospectively determined from the medical record. Correlation between reported SNOT-22 and ETDQ-7 scores were analyzed, in addition to the number of patients meeting the criterion score for ETD. RESULTS A total of 213 patients reported mean ETDQ-7 scores >2.1, consistent with a diagnosis of ETD. Overall SNOT-22 scores were strongly correlated with ETDQ-7 scores (r = 0.679, p < 0.001). The SNOT-22 ear subdomain was very strongly correlated with the ETDQ-7 score (r = 0.847, p < 0.001), while there was a moderate correlation with the SNOT-22 rhinologic subdomain (r = 0.486, p < 0.001). Patients with allergic fungal sinusitis had decreased ETDQ-7 scores compared to patients with other rhinologic diagnoses (p = 0.010). CONCLUSION Symptoms of ETD are common in patients treated in a Rhinology clinic, with 43.3% of patients reporting symptom scores consistent with a formal diagnosis of ETD. Increased SNOT-22 scores are correlated with increased ETDQ-7 scores, even when only the SNOT-22 rhinologic subdomain is considered. Patients presenting for treatment of rhinologic symptoms likely have an increased prevalence of otologic symptoms and ETD.

    Background: The past 5 years have witnessed the introduction of numerous Rhinology fellowships that may have altered the education of individuals electing to pursue postgraduate training. The objective of this study was to evaluate the Rhinology fellowship experience and its possible impact on current practice patterns of U.S. fellowship-trained rhinologists. Methods: A 27-item web-based survey (www.surveymonkey.com) was conducted under the purview of the American Rhinologic Society (ARS) Fellowship Committee between November 2009 and January 2010. The target group included 112 rhinologists who completed fellowship training between 1990 and 2009. Results: A total of 62 fellows (55.4%) completed the survey. The respondents represented all regions of the country, with most common being southeast (25%) and south central (18%). The most common type of clinical practice was academic in 68%, followed by single-specialty group (19%), multispecialty (6.5%), and solo practice (6.5%). Rhinology entailed >75% and 50% to 75% of clinical practice for 61% and 24% of respondents, respectively. The primary reasons for selecting a Rhinology fellowship were career in academics (64%) and interest in rhinologic procedures (31%). Overall fellowship experience was rated as excellent and good by 84% and 16%, respectively. The mean number of rhinologic procedures during fellowship were >200 in 59% and 151 to 200 in 27% of respondents. Allergy training exposure was reported by 59% during training. Clinical and basic science research exposure was rated as excellent or good by 90% and 44%, respectively. Conclusion: The Rhinology fellowship experience to date appears to be well-regarded by majority of respondents. Future dialogue should focus on identification of deficiencies in the current Rhinology fellowship educational process. © 2011 ARS-AAOA, LLC.
