    About 1 week after the induction of status epilepticus in male rats by a single systemic injection of lithium (3 mEq/kg) and pilocarpine (30 g/kg), rats were continuously administered one of three herbal treatments through the water supply for 30 days. A fourth group received colloidal minerals and diluted food grade hydrogen peroxide in tap water, while a fifth group of rats received only tap water (control). Herbal treatments were selected for their historical antiseizure activities and sedative actions on the nervous system. The numbers of spontaneous seizures per day during a 15 min observation interval were recorded for each rat during the treatment period and during an additional 30 days when only tap water was given. Rats that received a weak solution of the three herbal fluid extracts of Scutellaria lateri flora (Skullcap), Gelsemium sempervirens (Gelsemium) and Datura stramonium (Jimson Weed) displayed no seizures during treatment while all the other groups were not seizure-free. However, when this treatment was removed, the rats in this group displayed numbers of spontaneous seizures comparable to the controls. Although there is no proof that herbal remedies can control limbic or temporal lobe epilepsy, the results of this experiment strongly suggest that the appropriate combination of herbal compounds may be helpful as adjunctive interventions.

    Scutellaria lateriflora, a traditional herbal remedy for stress and anxiety, was tested on human volunteers for its effects on mood. In a placebo-controlled, double-blind, crossover study, 43 healthy participants were randomised to a sequence of three times daily S. lateriflora (350 mg) or placebo, each over two weeks. In this relatively non-anxious population (81% were mildly anxious or less, i.e. Beck Anxiety Inventory (BAI) scores ≤ 15), there was no significant difference between skullcap and placebo with BAI (p = 0.191). However, there was a significant group effect (p = 0.049), suggesting a carryover effect of skullcap. For Total Mood Disturbance measured by the Profile of Mood States, there was a highly significant (p = <0.001) decrease from pre-test scores with skullcap but not placebo (p = 0.072). The limitations of carryover effect, generally low anxiety scores and differences in anxiety levels between groups at baseline (p = 0.022), may have reduced the chances of statistical significance in this study. However, as S. lateriflora significantly enhanced global mood without a reduction in energy or cognition, further study assessing its putative anxiolytic effects in notably anxious subjects with co-morbid depression is warranted.

    Traditional use of American skullcap (Scutellaria lateriflora) for anxiety and related conditions is well documented. There is evidence of flavonoid instability in S. lateriflora and a high rate of substitution with other skullcap species or adulteration with potentially hepatotoxic germanders (Teucrium spp.). It is therefore essential for the identity, quality and safety of a commercial S. lateriflora product to be verified prior to clinical use. The objective was to review the literature relating to substitution and adulteration of S. lateriflora and to present a simple, optimised high performance liquid chromatography (HPLC) method to verify the absence of adulterants in a commercial sample of S. lateriflora, by comparing its chromatographic profile with that of authenticated S. lateriflora. S. lateriflora reference material and a freeze-dried commercial sample were extracted with methanol and water (80:20, v/v) and compared by HPLC analysis. The commercial sample showed reproducible retention times (RTs) of the flavonoid biomarkers baicalin (RT = 14.8 min; mean ± SD = 11.71 ± 1.16 mg/g); baicalein (RT = 20.4 min; 7.67 ± 0.89 mg/g); wogonin (RT = 23.7 min; 0.65 ± 0.06 mg/g). It appeared to be free from adulteration with germander (verbascoside was not detected; RT = 9.1 min) and its phytochemical profile was consistent with that of the S. lateriflora reference material. It is crucial that commercial products are adequately identified prior to use. The reported HPLC method has shown the potential to compare non-authenticated S. lateriflora samples with authenticated voucher specimens – essential when conducting any phytochemical analysis of the herb.

    Abstract American skullcap ( Scutellaria lateriflora ) is a popular herb in traditional medicine systems and western materia medica for the treatment of anxiety and related disorders. It is reported to be one of the most widely used medicinal herbs, with anecdotal evidence for minimal side-effects and with no known toxicity. This article summarises the results of a pilot survey conducted amongst herbal medicine practitioners on their use of S. lateriflora . An email survey was conducted amongst herbal medicine practitioners in the UK and Ireland. It aimed to gather information on the extent of, and indications for, current use of S. lateriflora , its perceived effectiveness and its safety. Herbal medicine practitioners were selected from the membership list of the National Institute of Medical Herbalists (NIMH). All members with identifiable email addresses were contacted (n = 377) and responses were received from 62 (a 16% response rate). The results of the survey suggested that S. lateriflora is highly regarded amongst herbal medicine practitioners as an effective intervention for reducing anxiety and stress and is commonly prescribed for these conditions and related co-morbidities. The results were not conclusive as the response rate was low and respondents were only those with email access.

  • Plant-Based Medicines for Anxiety Disorders, Part 2: A Review of Clinical Studies with Supporting Preclinical Evidence

    Research in the area of herbal psychopharmacology has revealed a variety of promising medicines that may provide benefit in the treatment of general anxiety and specific anxiety disorders. However, a comprehensive review of plant-based anxiolytics has been absent to date. Thus, our aim was to provide a comprehensive narrative review of plant-based medicines that have clinical and/or preclinical evidence of anxiolytic activity. We present the article in two parts. In part one, we reviewed herbal medicines for which only preclinical investigations for anxiolytic activity have been performed. In this current article (part two), we review herbal medicines for which there have been both preclinical and clinical investigations of anxiolytic activity. A search of MEDLINE (PubMed), CINAHL, Scopus and the Cochrane Library databases was conducted (up to 28 October 2012) for English language papers using the search terms ‘anxiety’ OR ‘anxiety disorder’ OR ‘generalized anxiety disorder’ OR ‘social phobia’ OR ‘post-traumatic stress disorder’ OR ‘panic disorder’ OR ‘agoraphobia’ OR ‘obsessive compulsive disorder’ in combination with the search terms ‘Herb*’ OR ‘Medicinal Plants’ OR ‘Botanical Medicine’ OR ‘Chinese herb*’, in addition to individual herbal medicines. This search of the literature revealed 1,525 papers, of which 53 plants were included in the review (having at least one study using the whole plant extract). Of these plants, 21 had human clinical trial evidence (reviewed here in part two), with the other 32 having solely preclinical evidence (reviewed in part one). Support for efficacy was found for chronic use (i.e. greater than one day) of the following herbs in treating a range of anxiety disorders in human clinical trials: Piper methysticum, Matricaria recutita, Ginkgo biloba, Scutellaria lateriflora , Silybum marianum, Passiflora incarnata, Withania somniferum, Galphimia glauca, Centella asiatica, Rhodiola rosea, Echinacea spp., Melissa officinalis and Echium amoenum . For several of the plants studied, conclusions need to be tempered due to methodological issues such as small sample sizes, brief intervention durations and non-replication. Current evidence does not support Hypericum perforatum or Valeriana spp. for any anxiety disorder. Acute anxiolytic activity was found for Centella asiatica, Salvia spp., Melissa officinalis, Passiflora incarnata and Citrus aurantium. Bacopa monnieri has shown anxiolytic effects in people with cognitive decline. The therapeutic application of psychotropic plant-based treatments for anxiety disorders is also discussed, specifically Psychotria viridis and Banisteriopsis caarti (ayahuasca), Psilocybe spp. and cannabidiol-enriched (low tetrahydrocannabinol (Δ^9-THC)) Cannabis spp.

    Research in the area of herbal psychopharmacology has increased markedly over the past decades. To date however, a comprehensive review of herbal antidepressant, anxiolytic and hypnotic psychopharmacology and applications in depression, anxiety and insomnia has been absent. A search of MEDLINE (PubMed), CINAHL, PsycINFO, and the Cochrane Library databases was conducted (up to February 21st 2011) on commonly used psychotropic herbal medicines. A review of the literature was conducted to ascertain mechanisms of action of these botanicals, in addition to a systematic review of controlled clinical trials for treatment of mood, anxiety and sleep disorders, which are common comorbid psychiatric disorders. Specific emphasis was given to emerging phytomedicines. Analysis of evidence levels was conducted, as were effect sizes (Cohen's d) where data were available. Results provided evidence of a range of neurochemical, endocrinological, and epigenetic effects for 21 individual phytomedicines, which are detailed in this paper. Sixty six controlled studies were located involving eleven phytomedicines. Several of these provide a high level of evidence, such as Hypericum perforatum for major depression, and Piper methysticum for anxiety disorders. Several human clinical trials provide preliminary positive evidence of antidepressant effects (Echium amoenum, Crocus sativus, and Rhodiola rosea) and anxiolytic activity (Matricaria recutita, Ginkgo biloba, Passiflora incanata, E. amoenum, and Scutellaria lateriflora). Caution should however be taken when interpreting the results as many studies have not been replicated. Several herbal medicines with in vitro and in vivo evidence are currently unexplored in human studies, and along with use of emerging genetic technologies "herbomics", are areas of potential future research.

    This paper reports a critical review of 27 herbal medicines and formulas in treating a broad range of psychiatric disorders (in addition to anxiety and depression), including obsessive-compulsive, seasonal affective, bipolar depressive, psychotic, phobic and somatoform disorders. Ovid Medline, Pubmed and the Cochrane Library were searched for pharmacological and clinical evidence of herbal medicines with psychotropic activity. A forward search of later citations was also conducted. Whilst substantial high-quality evidence exists for the use of kava and St John's wort in the treatment of anxiety and depression respectively, currently there is insufficient robust clinical evidence for the use of many other herbal medicines in psychiatric disorders. Phytotherapies which potentially have significant use in psychiatry, and urgently require more research are Rhodiola rosea (roseroot) and Crocus sativus (saffron) for depression; Passiflora incarnata (passionflower), Scutellaria lateriflora (scullcap) and Zizyphus jujuba (sour date) for anxiety disorders; and Piper methysticum (kava) for phobic, panic and obsessive-compulsive disorders. While depression and anxiety are commonly researched, the efficacy of herbal medicines in other mental disorders requires attention. The review addresses current issues in herbal psychotherapy: herbal safety, future areas of application, the relationship of herbal medicine with pharmaceuticals and the potential prescriptive integration of phytomedicines with synthetic psychotropic medicines.
