    A phase 2 evaluation of live attenuated parainfluenza type 3 (PIV3) -cold passage mutant 45 (cp45) vaccine was conducted in 380 children 6 - 18 months old; 226 children (59%) were seronegative for PIV3. Of the 226 seronegative children, 114 received PIV3-cp45 vaccine, and 112 received placebo. No significant difference in the occurrence of adverse events (i.e., runny nose, cough, or temperature greater than or equal to38 degreesC) was noted during the 14 days after vaccination. There was no difference between groups in the occurrence of acute Otitis Media or Serous Otitis Media. Paired serum samples were available for 109 of the seronegative vaccine recipients and for 110 of the seronegative placebo recipients; 84% of seronegative vaccine recipients developed a greater than or equal to4-fold increase in antibody titers. The geometric mean antibody titer after vaccination was 1: 25 in the vaccine group and <1: 4 in the placebo group. PIV3-cp45 vaccine was safe and immunogenic in seronegative children and should be evaluated for efficacy in a phase 3 field trial.

    Abstract:

    The relationship between passive smoking and middle ear disease was reviewed. The hypothesis that acute Otitis Media, Serous Otitis Media, and related diseases in children are caused by inhalation of second-hand smoke has been accepted by many. We reviewed the literature on this topic. There are many studies that do not support the hypothesis. There is no indication that the smokers themselves have a higher incidence of middle ear disease. Considering the difficulty of publishing negative studies, the need for academics to publish significant findings, and the poor foundation for some authors' conclusions in the literature, we find that the literature does not offer sufficient support for the hypothesis that second-hand smoke causes middle ear disease to accept the hypothesis.

    Abstract:

    ÖZETSeröz Otitis Mediada medikal tedaviye cevap vermeyen durumlarda ventilasyon tüpü uygulanmaktadır. Böylece orta kulak ventilasyonu sağlanmaktadır. Orta kulak ventilasyonun 2-3 hafta yeterli olduğu düşünüldüğünde ventilasyon tüpü uygulamasının getirdiği riskler ve komplikasyonlar nedeniyle alternatif yöntemler araştırılmaktadır. Bu alternatif yöntemler arasında termal miringotomiler, lokal Mitomisin C ve 5 Floururasil uygulamaları sayılabilir. Bu çalışmada, daha önceki çalışmlarda perforasyon süresini arttırdığı tespit edilmiş olan Mitomisin C ile bu konuda hakkında çok az çalışma yapılmış olan Kolşisin karşılaştırılmış ve Kolşisin’in Mitomisin C’ nin alternatifi olup olmadığı incelenmiştir. Çalışmamızda 30 adet Wistar cinsi sıçan kullanıldı. Hayvanlar 10’ar adet 3 gruba ayrıldı. Bir grubun sağ ve sol kulaklarına Mitomisin C, diğer gruba Kolşisin lokal olarak perforasyona uygulandı. 3. grup ise kontrol grubu olarak ayrıldı ve serum fizyolojik sürüldü. Hayvanlar haftalık olarak mikroskop altında kontrol edildi ve perforasyonların kapanma süreleri Kaplan-Meier Survival istatistik tekniği kullanılarak karşılaştırıldı. Perforasyonların kapanma süreleri Mitomisin C grubunda 4,9 hafta, Kolşisin grubunda 3,1 hafta, kontrol grubunda 1,1 hafta olarak tespit edildi. Mitomisin C ve Kolşisin grupları kontrol grubuyla karşılaştırıldığında istatistiksel olarak perforasyon kapanmasını anlamlı derecede uzattığı tespit edildi. Bu iki grup arasında Mitomisin C’nin Kolşisin’e oranla istatistiksel olarak anlamlı deredece daha çok uzattığı tespit edildi. Mitomisin C grubuna ait kulaklardan birinde perforasyon kapanmadı. Mitomisin’in perforasyon kapanma sürelerine olan etkisi Kolşisin’e oranla daha çok etkili olmasına rağmen Kolşisin’in de etkili ve yeterli olduğu saptandı. So middle ear ventilation provided. 2 or 3 weeks of continuous aeration of middle ear is thought to be suffcient in treating Serous Otitis Media. However, high incidence of complications of ventilation tube insertion is the reason fort he searching of alternative surgical procedures. Recent studies have showed that application of topical Mitomycin C or 5- Flourouracil to myringotomies can lengthen the closure time of perforations and termal myringotomies. The aim of the present study was to compare the effects of topically applied Mitomycin C and Cholchisine on potency time of perforations and investigate whether this technique can be an alternative precedure to ventilation tube insertion by the information gathered from this study. This study was performed on 30 healty Wistar rats. Myringotomies were performed under general anesthesia. Mitomycin C pledgets were applied to the right and left ears of 10 rats and Cholchicine were applied to the right and left ears of 10 rats. 10 rats were selected as a control group. Animal were monitonied using otomicroscopy weekly and Kaplan – Meier survival tecnique were used to compare patency time between groups. The mean patency time was 4,9 weeks in Mitomycin C, 3,1 weeks in Cholchicine group and 1,1 weeks in control group. The test showed the study groups to have significantly longer patency then the control group. Allthrough the patency rate of Mitomycin C group was longer than Cholchicine group, the difference between these groups was statistically significant. One permanent perforation occured in mitomycin group. Mitomycin C and Cholchicine application to myringotomies prolongs myringotomy patency time and be proposed as an alternative to the ventilation tube insetion technique especially for indications that need short ventilation time .     Abstract:

    La hipoacusia tiene repercusión negativa en el aprendizaje y comportamiento social de los niños, por lo que se hace necesaria la aplicación de técnicas quirúrgicas cada día más eficientes que reduzcan esta problemática. Con el objetivo de demostrar la eficacia de una nueva modalidad quirúrgica consistente en realizar adenoidectomía y doble miringotomía con aspiración del contenido seromucoso y de proponer una estrategia para solución quirúrgica definitiva en el nivel secundario y preventiva en el nivel primario, se realizó una investigación de innovación _ tecnológica, descriptiva, longitudinal prospectiva en niños con hipoacusia conductiva por Otitis Media serosa secundaria a hipertrofia adenoidea, que asistieron a las consultas de audiología del Hospital Pediátrico Provincial Docente" Pepe Portilla" y del Policlínico Universitario Dr." Ernesto Guevara" de Sandino. El universo estuvo constituido por todos los niños con hipoacusia conductiva registrados en consulta de audiología. La muestra resultó de 109 pacientes con hipoacusia por Otitis Media Serosa secundaria a Hipertrofia Adenoidea, provenientes de 10 municipios de nuestra provincia, entre los 4 y 12 años de edad; Mediante muestreo intencional. A todos los pacientes se les realizó examen otorrinolaringológico y audiometría tonal. Se utilizaron métodos de encuesta, análisis documental y de la Estadística Descriptiva utilizando medidas de frecuencias absolutas y relativas porcentuales. Se calcularon intervalos de confianza para algunas de las frecuencias relativas buscadas y previo consentimiento informado se trataron quirúrgicamente, reevaluados en consulta, donde se comprobó que 108 pacientes evolucionaron satisfactoriamente, concluyendo que es una técnica eficaz.Hypoakusia has a negative repercussion in learning and social behaviour in children so it is mandatory the implementation of surgical techniques in order to minimize this condition.. A new descriptive longitudinal and prospective technique was performed in children suffering from conductive hypoakusia due to Serous Otitis Media secondary to adenoid hypertrophia attending to the hearing care office at "Pepe Portilla"Pediatric Hospital and "Dr.Ernesto Guevara "Policlinic in Sandino in order to show the efficacy of a new definitive surgical technique (adenotomy and double miringotomy with aspiration of the seromucus content ) and to propose an strategy for a definitive surgical solution in the secondary level being preventive at primary level. Universe was comprised of all the children suffering from conductive hypoakusia who were recorded in the auditory care office, the sample had 109 patients coming from 10 municipalities of our province and suffering from hipoakusia due to Serous Otitis Media secondary to adenoid hypertrophia, they were 4 and 12 year old. All the patients were given otolaringologic examinations and tonal audiometry. Survey methods, documental analisis and descriptive statistic method were applied using relative and absolute percentage frequency measurements.Confidence intervals for some relative frequencies and previous informed consent were estimated; patients were surgically treated and reevaluated at the office showing that 108 patients had a satisfactory natural history and concluding that it is an efficient technique
