    Abstract:

    We assessed the effects of the sacubitril/valsartan combination drug (LCZ696), in comparison to valsartan alone, on the progression of atherosclerotic plaque formation and inflammatory gene expression in apolipoprotein E- deficient mice (apoE−/− mice). Seventy-two apoE−/− mice were fed a western diet and a constrictive Silastic Tube was used to elicit carotid lesion formation. The animals were separated into a control group, a valsartan group or an LCZ696 group (n = 24 in each group). Plaques in the carotid artery were harvested 12 weeks later for histological examination. The levels of pro-inflammatory genes in the plasma and lesions were detected using real-time PCR and ELISA. Valsartan or LCZ696 treatment remarkably inhibited the expression of pro-inflammatory genes, including interleukin-6, matrix metalloproteinase-8 and monocyte chemotactic protein-1, in comparison with the control group. Meanwhile, both valsartan and LCZ696 suppressed the formation of atherosclerotic plaques by decreasing plaque lipid content and cross-sectional plaque area and increasing the content of plaque collagen and fibrous cap thickness. In particular, LCZ696 performed the best in suppressing atherosclerosis and inhibiting the level of pro-inflammatory genes. LCZ696 significantly ameliorated atherosclerosis and inflammation in apoE−/− mice compared with valsartan.

    STUDY DESIGN The pathophysiologic changes of the spinal nerve root caused by chronic compression were assessed in the lumbar spine of the adult dog. METHODS The seventh lumbar nerve root was banded with a Silastic Tube 5 mm long and a 3 mm internal diameter, which is slightly larger than the diameter of the nerve root. Histologic and electrophysiologic studies as well as assessment of the blood-nerve barrier function in the nerve root were conducted at 24 hours through 12 months. RESULTS The earliest findings were thickening of the dura mater and arachnoid membrane around the affected nerve root corresponding to the alteration of the blood-nerve barrier in the nerve root after 1 month. After 3 months, large myelinated fibers decreased in number and small newly formed fibers increased in the periphery of the fascicle. At 6 months, endoneurial fibrosis and Wallerian degeneration of nerve fibers became obvious. Compound action potentials and sensory nerve conduction velocity decreased by 3 and 12 months, respectively. Decrease in amplitude of the compressed action potential was identical to the period of decrease in large myelinated fibers, and nerve conduction velocity did not decrease as long as some large myelinated fiber remained until 12 months after tubing. CONCLUSION Intraradicular edema caused by alteration of the blood-nerve barrier is the most important factor in the nerve root dysfunction due to chronic compression.

    STUDY DESIGN: A case report and literature review of the treatment of "noncommunicating" syringomyelia. OBJECTIVE.: The aim of this report is to document the timing and the treatment of hydromyelia holocord after surgical treatment for both tethering and retethering of spinal lipoma. SUMMARY OF BACKGROUND DATA: Syringomyelia associated with spinal lipoma presents a different pathogenesis and treatment in comparison to the "communicating" hydromyelia in the myelomeningocele. After the primary retethering operation performed in symptomatic patients, recurrent retethering can occur with an increase of the syringomyelia signs and symptoms. METHODS: Syringomyelia treated with a thin Silastic Tube passed from the syrinx to the subarachnoidal space for drainage and decompression. Prior operations were: (1) initial untethering at birth, (2) second untethering at 5 years of age, (3) posterior fossa and cervical decompression. RESULTS: Magnetic resonance imaging 6 months post shunt operation demonstrated decompression of the hydromyelia holocord and syringobulbia with improvement of motor function of the legs and improvement in sensory symptoms. CONCLUSION: Usefulness of syrinx-subarachnoidal shunt is demonstrated in this case report after unsuccessful decompression and detethering. When the enlargement of the ependymal channel is greater than 50% of the spinal cord's diameter, neurologic, and urological symptoms are evident and the patient benefitted from cord untethering and syrinx drainage. (1) The terminal "noncommunicating" syringomyelia in lumbar sacral lipoma has been reported to be associated with retethering in spinal lipoma in the 25% of the cases. (2) The rise of distal syringomyelia isn't only linked to the kind of the spinal lipoma, but also to the difficulty to obtain the untethering and a smooth cerebrospinal fluid flow between the subarachnoidal space and the ependymal canal. (3) In patients with hydromyelia holocord greater than the 50% of the spinal cord's diameter a myelotomy and insert an ependymal channel/syrinx to the subarachnoidal space shunt can resolve of the syrinx. In this case, the enlargement of the ependymal channel in "noncommunicating" syringomyelia associated with lumbosacral lipoma is greater than 50% of the spinal cord's diameter; neurologic and urological symptoms occurred and the patient benefited from cord untethering and concurrent syrinx drainage.
