Sleep Debt the impact of weekday Sleep deprivation on cardiovascular health in older women

However, it is uncertain whether Sleep Debt, a measure of Sleep deficiency during the week compared to the weekend, confers increased cardiovascular risk. Because Sleep disturbances increase with age particularly in women, we examined the relationship between Sleep Debt and ideal cardiovascular health (ICH) in older women. METHODS Sleep Debt is defined as the difference between self-reported total weekday and weekend Sleep hours of at least 2 hours among women without apparent CVD and cancer participating in the Women's Health Stress Study follow-up cohort of female health professionals (N = 22 082). The ICH consisted of seven health factors and behaviors as defined by the American Heart Association Strategic 2020 goals including body mass index, smoking, physical activity, diet, blood pressure, total cholesterol, and glucose. RESULTS Mean age was 72.1 ± 6.0 years. Compared to women with no Sleep Debt, women with Sleep Debt were more likely to be obese and have hypertension (pall < .05). Linear regression models adjusted for age and race/ethnicity revealed that Sleep Debt was significantly associated with poorer ICH (B = -0.13 [95% CI = -0.18 to -0.08]). The relationship was attenuated but remained significant after adjustment for education, income, depression/anxiety, cumulative stress, and snoring. CONCLUSION Sleep Debt was associated with poorer ICH, despite taking into account socioeconomic status and psychosocial factors. These results suggest that weekly Sleep duration variation, possibly leading to circadian misalignment, may be associated with cardiovascular risk in older women.

There is therefore a need for biomarkers to monitor Sleep Debt status. None are currently available. We applied Elastic-net and Ridge regression to entire and pre-filtered transcriptome samples collected in healthy young adults during acute total Sleep deprivation and following 1 week of either chronic insufficient (< 6 h) or sufficient Sleep (~8.6 h) to identify panels of mRNA biomarkers of Sleep Debt status. The size of identified panels ranged from 9-74 biomarkers. Panel performance, assessed by leave-one-subject-out cross-validation and independent validation, varied between Sleep Debt conditions. Using between-subject assessments based on one blood sample, the accuracy of classifying ‘Acute Sleep loss’ was 92%, but only 57% for classifying ‘Chronic Sleep insufficiency’. A reasonable accuracy for classifying ‘chronic Sleep insufficiency’ could only be achieved by a within-subject comparison of blood samples. Biomarkers for Sleep Debt status showed little overlap with previously identified biomarkers for circadian phase. Biomarkers for acute and chronic Sleep loss also showed little overlap but were associated with common functions related to the cellular stress response, such as heat shock protein activity, the unfolded protein response, protein ubiquitination and endoplasmic reticulum associated protein degradation, and apoptosis. This characteristic response of whole blood to Sleep loss can further aid our understanding of how Sleep insufficiencies negatively affect health. Further development of these novel biomarkers for research and clinical practice requires validation in other protocols and age groups.

role of Sleep duration and quality in the risk and severity of type 2 diabetes mellitus

We examined whether short or poor Sleep is associated with glycemic control in African Americans with type 2 diabetes mellitus. Methods We conducted a cross-sectional study of volunteers with type 2 diabetes interviewed at the University of Chicago Hospitals, Chicago, Ill. The final analysis included 161 participants. Glycemic control was assessed by hemoglobin A 1c (HbA 1c ) level obtained from medical charts. Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI). Perceived Sleep Debt was calculated as the difference between preferred and actual weekday Sleep duration. Results The mean ± SD Sleep duration was 6.0 ± 1.6 hours, and 71% of the participants were classified as having poor quality Sleep (PSQI score >5). We excluded patients with Sleep frequently disrupted by pain (n = 39). In patients without diabetic complications, glycemic control was associated with perceived Sleep Debt but not PSQI score. The predicted increase in HbA 1c level for a perceived Sleep Debt of 3 hours per night was 1.1% above the median. In patients with at least 1 complication, HbA 1c level was associated with PSQI score but not perceived Sleep Debt. The predicted increase in HbA 1c level for a 5-point increase in PSQI was 1.9% above the median. Conclusions In our sample, Sleep duration and quality were significant predictors of HbA 1c , a key marker of glycemic control. Combined with existing evidence linking Sleep loss to increased diabetes risk, these data suggest that optimizing Sleep duration and quality should be tested as an intervention to improve glucose control in patients with type 2 diabetes.

We examined the impact of semichronic partial Sleep loss, a highly prevalent condition, on the 24-h growth hormone profile. Eleven young men were studied after six nights of restricted bedtimes (0100-0500) and after 7 nights of extended bedtimes (2100-0900). Slow-wave Sleep (SWS) was estimated as the duration of stages III and IV. Slow-wave activity (SWA) was calculated as electroencephalogram power density in the 0.5- to 3-Hz frequency range. During the state of Sleep Debt, the GH secretory pattern was biphasic, with both a preSleep onset "circadian" pulse and a postSleep onset pulse. PostSleep onset GH secretion was negatively related to preSleep onset secretion and tended to be positively correlated with the amount of concomitant SWA. When Sleep was restricted, both SWS and SWA were increased during early Sleep. Unexpectedly, the increase in SWA affected the second, rather than the first, SWA cycle, suggesting that preSleep onset GH secretion may have limited SWA in the first cycle, possibly via an inhibition of central GH-releasing hormone activity. Thus neither the GH profile nor the distribution of SWA conformed with predictions from acute Sleep deprivation studies, indicating that adaptation mechanisms are operative during chronic partial Sleep loss.

Impact of Sleep Debt on metabolic and endocrine function

Sleep Debt is currently believed to have no adverse effect on health. We investigated the effect of Sleep Debt on metabolic and endocrine functions. Methods We assessed carbohydrate metabolism, thyrotropic function, activity of the hypothalamo-pituitary-adrenal axis, and sympathovagal balance in 11 young men after time in bed had been restricted to 4 h per night for 6 nights. We compared the Sleep-Debt condition with measurements taken at the end of a Sleep-recovery period when participants were allowed 12 h in bed per night for 6 nights. Findings Glucose tolerance was lower in the Sleep-Debt condition than in the fully rested condition (p Interpretation Sleep Debt has a harmful impact on carbohydrate metabolism and endocrine function. The effects are similar to those seen in normal ageing and, therefore, Sleep Debt may increase the severity of age-related chronic disorders.

Here, we describe in detail Sleep-wake behavior in a fast-forward rotating 12-h shift schedule, which involves at least 24 hours off after each shift and thus allows examining the role of immediate recovery after shift-specific Sleep Debt. Thirty-five participants at two chemical plants in Germany were chronotyped using the Munich ChronoType Questionnaire for Shift-Workers (MCTQ(Shift)) and wore actimeters throughout the two-week study period. From these actimetry recordings, we computed Sleep and nap duration, social jetlag (a measure of circadian misalignment), and the daily timing of activity and Sleep (center of gravity and mid-Sleep, respectively). We observed that the long off-work periods between each shift create a fast alternation between shortened (mean ± standard deviation, 5h 17min ± 56min) and extended (8h 25min ± 72min) Sleep episodes resulting in immanent reductions of Sleep Debt. Additionally, extensive napping of early chronotypes (up to 3 hours before the night shift) statistically compensated short Sleep durations after the night shift. Partial rank correlations showed chronotype-dependent patterns of Sleep and activity that were similar to those previously described in 8-h schedules; however, Sleep before the day shift did not differ between chronotypes. Our findings indicate that schedules preventing a build-up of chronic Sleep Debt may reduce detrimental effects of shift work irrespective of shift duration. Prospective studies are needed to further elucidate the relationship between Sleep, the circadian system, and health and safety hazards.

More recently, the Munich ChronoType Questionnaire (MCTQ) was developed: it asks for Sleep-wake behavior on work and free days and uses the midpoint of Sleep on free days (MSF), corrected for Sleep Debt accumulated during the work week as an indicator of chronotype (MSFsc). In this study, we developed a Japanese version of the MCTQ by using a translation/back-translation approach including an examination of its semantic validity. In a subsequent questionnaire survey, 450 adult men and women completed the Japanese versions of the MCTQ and MEQ. Results showed that MEQ scores were significantly negatively correlated with mid-Sleep parameters assessed by the MCTQ, on both, work and free days, as well as with the chronotype measure MSFsc (r = −0.580 to −0.652, all p < 0.001). Therefore, investigations into how shift-work affects Sleep, health, and cognition may provide more reliable insights if they consider individual circadian time (chronotype). The Munich ChronoType Questionnaire (MCTQ) is a useful tool for determining chronotype. It assesses chronotype based on Sleep behavior, specifically on the local time of mid-Sleep on free days corrected for Sleep Debt accumulated over the workweek (MSFsc). Because the original MCTQ addresses people working standard hours, we developed an extended version that accommodates shift-work (MCTQShift). We first present the validation of this new version with daily Sleep logs (n = 52) and actimetry (n = 27). Next, we evaluated 371 MCTQShift entries of shift-workers (rotating through 8-h shifts starting at 0600 h, 1400 h, and 2200 h).
