At lower fluences (2.0 to 4.0 J/cm2), this laser has been purported to increase the amount of collagen in the extracellular matrix. OBJECTIVE To determine whether the 585-nm pulsed dye laser is effective in both Striae rubra and Striae alba after two treatments and, in addition, to determine whether there is a net effect on collagen density in treated samples. METHODS Twenty patients (skin types II to VI) with either Striae rubra (9) or Striae alba (11) were treated at baseline and at week 6. The total length of the study was 12 weeks. Untreated Striae on the same patient were used as control subjects. Striae clinical parameters such as color and area were measured before the first and after the last treatment by an independent examiner using a visual analog scale. A hydroxyproline assay was used to measure collagen changes in two of the Striae quantitatively. RESULTS The 585 nm had a moderate beneficial effect in reducing the degree of erythema in Striae rubra. There was no apparent clinical change on Striae alba. Total collagen per gram of dry weight of sampled tissue increased in Striae treated with pulsed dye laser versus control subjects. CONCLUSION We recommend the use of the 585-nm pulsed dye laser for Striae rubra in patients skin types II to IV. Extreme caution or avoidance should be observed in pulsed dye laser treatments for patients with phototypes V to VI even with the use of low fluences. Tissue collagen changes measured may be an early change, which precedes significant clinical improvement.

The Thermage (Therma Cool TC; Thermage Inc, Hayward, CA) is a radiofrequency device for the lifting of face and neck, and there is no report of using it for Striae Distensae. OBJECTIVE The purpose was to evaluate the effectiveness of the Thermage in combination with pulsed dye laser. MATERIALS AND METHODS Thirty-seven patients with abdominal Striae Distensae were treated with the Thermage and 585-nm pulsed dye laser in the first session at baseline. An additional two sessions of pulsed dye laser were performed at Weeks 4 and 8. Thermage was used at fluences of 53 to 97 J/cm2 and pulsed dye laser at fluences of 3.0 J/cm2 with 10-mm spot. Skin biopsies were taken of nine patients. RESULTS In the subjective assessment, 89.2% of the patients showed “good and very good” to overall improvement, and 59.4% were graded as “good and very good” in elasticity. All of the nine specimens showed an increase in the amount of collagen fibers, and increased elastic fibers were found in six specimens. CONCLUSION The Thermage and pulsed dye laser appear to be an effective treatment for Striae distansae.

The aim of this research was to obtain a topical product containing herbal active ingredients with documented antioxidant and anti-inflammatory activity (Punica granatum seed oil and Croton lechleri resin extract) and demonstrate its positive effect on prevention and treatment of Striae Distensae. First, the cream base formulation was optimized through experimental design. Secondly, the cream containing the two active ingredients was investigated in an interventional nonrandomized clinical trial. The clinical outcome was assessed through biophysical parameters and ultrasonographic evaluation. The state of the skin was evaluated by biophysical measurements and ultrasonography at the beginning of the study and after 3 and 6 weeks. The experimental design was successfully used to set the best ranges for the technological and formulation factors to obtain a cosmetic formulation with optimal characteristics. The study of clinical efficacy on the optimal formulation revealed an increase in the dermis thickness, hydration and elasticity values in both groups after 6 weeks of cream application. The new oil-in-water cream containing P. granatum seed oil and C. lechleri resin extract can be helpful in the prevention or improving of skin changes associated with Striae.

High resolution skin colorimetry, strain mapping and mechanobiology.

They are governed by ethnicities and phototypes, and further influenced by a variety of factors including photoexposures and sustained mechanical stress. Indeed, mechanobiology is a feature affecting the epidermal melanization. High-resolution epiluminescence colorimetry helps in deciphering the effects of forces generated by Langer's lines or relaxed skin tension lines on the melanocyte activity. The same procedure shows a prominent laddering pattern of melanization in Striae Distensae contrasting with the regular honeycomb pattern in the surrounding skin.Peer reviewe

Streamlining of cellulite concepts.

It is not a result of increased body mass, but its aspect may be influenced by the waist-to-hip ratio. There are glaring discrepancies in the microanatomical descriptions regarding this condition. A lumpy aspect of the dermo-hypodermal interface is often mentioned, but this aspect rather represents a gender-linked characteristic of the thighs and buttocks of women without being a specific sign of cellulite. Incipient cellulite recognized by a discrete padded look or “orange peel” aspect appears correlated with the presence of a network of focally enlarged fibrosclerotic strands partitioning the hypodermis and serving as a physiologic buttress limiting the outpouching of fat lobules on pinching the skin. Such connective tissue structures have been ascribed to be the result of a hormonal-dependent reactive process to sustained mechanical tensions caused by the adipocyte lobules. Full blown cellulite is recognized by a lumpy-bumpy and dimpled skin surface. It likely represents subjugation of the hypertrophic response of the hypodermal connective tissue strands when their resistance is overcome by progressive fat accumulation. In these cases, histological aspects reminiscent of Striae Distensae are identified within the hypodermal connective tissue strands. The mechanical properties of skin involved in the cellulite process are altered. They influence the mechanobiology of connective tissue cells, in particular the Factor XIIIa-positive dermal dendrocytes

Laddering melanotic pattern of Langer's lines in skin of colour.

The clinical impact on melanocytes can be perceived by dermoscopy. Our aim was to assess the orientation of Langer's lines using the combination of ultrasound shear wave propagation and dermoscopy in 70 adults of darker skin complexion. On the back, 44/70 patients showed a honeycomb melanotic pattern without any main orientation. By contrast, a streaky parallel pattern of melanotic lines oriented in the direction of Langer's lines was found in 26/70 patients. Indeed, the maximum speed of ultrasound propagation was found parallel to the main orientation of the laddering melanotic pattern. The parallel melanotic pattern probably reflects the main orientation of the epidermal rete ridges aligned in the direction of Langer's lines. This aspect could be ascribed to the deepening of these structures and/or to mechanobiology affecting melanocytes. The aspect is reminiscent of that previously described in Striae Distensae and atrophic scars.Peer reviewe

High-resolution epiluminescence colorimetry of Striae Distensae.

A close look using dermoscopy discloses distinct patterns of melanized networks at these sites. The aim of the study was to design a method of high-\resolution analytical analysis of the skin colours using the combination of photographic dermoscopy and small field reflectance colorimetry. METHODS: Clinical photographs were taken from Striae Distensae and their surrounding skin using a Dermaphot (Heine Optotechnik, Hersching, Germany). A final magnification of 125x was obtained on paper photographs. The reflectance colorimeter Visi-Chroma VC-100 (Biophotonics, Lessines, Belgium) was used to measure colours of the pigmentary networks in the L*a*b* system. Differential colour parameters (deltaE*ab, deltaL*, deltaa*, deltab*) were calculated for each case between the lesional and the surrounding normal skin, and between the melanized reticulated pattern and the enclosed lighter areas. RESULTS: Objective colorimetric assessments distinguished four distinct types, namely Striae albae, Striae rubrae, Striae caeruleae and Striae nigrae. The latter peculiar hyperpigmented type of Striae Distensae was specifically identified by epiluminescence examination in dark-skinned subjects. The fine-melanized honeycomb network present on the adjacent intact skin was reshaped inside Striae in a streaky pattern perpendicular to the Striae axis. Strong linear correlations were found between all combinations of deltaL* and deltab* evaluating colours of the reticulated and the honeycomb alveolar patterns both inside and outside the Striae Distensae. By contrast, no correlations were found between deltaa* and the other colorimetric parameters. CONCLUSION: The direct and/or indirect influences of melanocyte mechanobiology appear to have a prominent effect on the various colours of Striae Distensae.Peer reviewe
