Reliability of histologic diagnosis of clinically normal intraoral tissue adjacent to clinically suspicious lesions in former upper aerodigestive tract cancer patients

SchwartzAbstract:Oral lesion biopsies often include adjacent clinically normal tissue to assess lesion spread, but the reliability of adjacent tissue histologic diagnoses has not been reported. Sixty-seven patients presenting with subsequent suspicious intraoral lesions underwent tissue staining with Tolonium Chloride. Biopsies of unstained tissue adjacent to suspicious lesions were reviewed by independent pathologists. The weighted kappa (kappa(w)) for histologic classification was 0.75 (95% confidence interval [CI] 0.64, 0.86) and was lower for: inflamed specimens [kappa(w)=-0.13 (95% CI -0.30, 0.05)] versus noninflamed specimens [kappa(w)=0.81 (95% CI 0.69, 0.92)], wedge biopsies [kappa(w)=0.58 (95% CI 0.37, 0.79)] versus punch biopsies [kappa(w)=0.90 (95% CI 0.83, 0.97)], and current smokers [w=0.59 (95% CI 0.32, 0.86)] versus past and never smokers [kappa(w)=0.82 (95% CI 0.72, 0.91)]. Histologic diagnosis of subsets of clinically normal oral specimens adjacent to suspicious tissue has low reliability. Molecular techniques may improve diagnoses in these subsets.

The utility of Tolonium Chloride rinse in the diagnosis of recurrent or second primary cancers in patients with prior upper aerodigestive tract cancer.

The clinical utility of Tolonium Chloride rinse was compared with unaided visual examination alone in the diagnosis of oral carcinoma in patients previously treated for carcinoma of the upper aerodigestive tract. Methods. A total of 668 patients were enrolled in this multicenter study. At each site, an oral clinical visual examination was completed by one investigator followed by Tolonium Chloride rinse and examination by a second investigator blinded to the examination findings of the first investigator. If a lesion was considered suspicious (urgent biopsy required at the first visit), the lesion was biopsied after Tolonium Chloride rinsing. Patients with lesions characterized at the first visit as not suspicious (biopsy not urgent) or that stained with Tolonium Chloride were asked to return for a second visit. At the second visit, any residual lesion or lesions that retained Tolonium Chloride were biopsied. Results. A total of 96 biopsies was performed in 81 of the 668 patients (12.1%), of which 30 (31.3%) were diagnosed histologically as carcinoma/carcinoma in situ (CIS) and the remainder as inflammation (31.3%), keratosis (26.6%), dysplasia (21.9%), ulcer (2.1%), other (3.1%), or no abnormality (1.0%). Of the 30 lesions with the diagnosis of carcinoma/CIS, 12 (sensitivity 40.0%) were considered to be clinically suspicious (CS+), whereas 29 (sensitivity 96.7%) retained Tolonium Chloride (p = .0002). The predictive values of a positive test for clinical examination and Tolonium Chloride staining were similar (36.4% vs 32.6%; p = .5871), indicating that the greater sensitivity of Tolonium Chloride was not associated with an excessive number of unnecessary biopsies (false positives). Conclusions. Tolonium Chloride rinse is more sensitive than clinical examination alone in detecting lesions that might be found on biopsy to be carcinoma or CIS. The increased sensitivity is largely attributed to lesions that stain but were not detected clinically on visual examination. © 2003 Wiley Periodicals, Inc. Head Neck 25: 000–000, 2003

The comparison of penetration depth of two different photosensitizers in root canals with and without smear layer: An in vitro study.

Materials & methods Forty extracted single-rooted human teeth with straight canals that extracted for periodontal reasons were collected and stored in the sterile saline until employment in the experiment. Teeth were decoronated to a standard 12 mm root segment using diamond disc. After instrumentation of specimens, the external root surface was sealed with two layers of nail polish to avoid environmental contamination. The apical foramen was subsequently closed with composite material. Teeth were divided randomly in two major groups consist of indocyanine green solution (ICG) and Tolonium Chloride solution (TCH) with and without EDTA in their subgroups. Specimens in all groups grooved longitudinally with a diamond disc and split in two halves with a stainless steel chisel. The measurements were done by the stereo microscope under 20× magnification in three zones of each specimen and the penetration depth of dye was measured. Results The results of this study showed that the mean of lateral penetration depth of ICG (224.04 μm) was significantly (P 0.05). Conclusion Further to the findings of this study, it could be assumed that ICG could penetrate in deeper regions of the root canal wall.

The progress of the reaction was monitored spectrophotometrically, and it follows pseudo first order kinetics. The effect of variation of different parameters, like concentration of Tolonium Chloride, pH, amount of semiconductor and light intensity on the rate of bleaching, was also studied. A tentative mechanism for the photocatalytic bleaching of Tolonium Chloride has been proposed.

Studies on Tolonium Chloride labeled with radioiodine

This study establishes the optimal preparation of radioiodinated Tolonium Chloride (RTC) and evaluate its radiochemical and biological characteristics. Instant thin layer chromatography (ITLC), ion exchange chromatography (IC), paper electrophoresis, and the effect of pH on labeling efficiency revealed the chemical characterization of RTC. Biodistribution, blood clearance, urinary excretion, toxic effect, and Lugol’s solution effect on the thyroid uptake of RTC revealed RTCs biological characteristics. The optimal labeling condition was pH = 2.96 after 15 h stirring, the labeling efficiency was 60%. After purification by IC, the radiochemical purity of RTC was 94%, and the shelf life of RTC was at least 90 days. In the biodistribution study, the liver was major target organs, approximated 6.11% of injected dose accumulate in per gram of tissue (6.11% ID/g) at 10 min after injection. In liver, the tissue-to-blood radioactivity ratio was 2.2 ± 0.51 at 10 min after injection, and increased to 22.4 ± 4.52 at 120 min after injection. The blood clearance study showed a significant decrease in blood radioactivity. The radioactivity in the blood was about 2.76% of the injected dose per milliliter blood at 10 min post-injection, but decreased to 0.12% at 120 min post-injection. Lugol’s solution does not affect the thyroid uptake of RTC. Almost all the administered RTC recovered at 60 h after injection, with 79% recovered in feces and 17% recovered in urine. This study shows that RTC is non-toxic, and may be suitable as a liver imaging agent.
