Sørensen, Robbert J J Gobbens, Hans Lund, Mette AadahlAbstract:Abstract The Tilburg Frailty Indicator (TFI) is a self-administered questionnaire with a bio-psycho-social integrated approach that measures the degree of frailty in elderly persons. The TFI was developed in the Netherlands and tested in a population of elderly Dutch men and women. The aim of this study was to translate and culturally adapt the TFI to a Danish context, and to test face validity of the Danish version by Cognitive Interviewing. An internationally recognized procedure was applied as a basis for the translation process. The primary tasks were forward translation, reconciliation, back translation, harmonization and pretest. Pretest and review of the preliminary version by Cognitive Interviewing, were performed at a local community center and in an acute medical ward at the University Hospital in Aalborg, Denmark respectively. A large agreement regarding meaning of the items in the forward translation and reconciliation process was seen. Minor discrepancies were solved by consensus. Back translation revealed unclear wording in one matter. The harmonization committee agreed on a version for Cognitive Interviewing after revision of minor issues and thirty-four participants were interviewed. Two issues became evident and these were revised. The Cognitive interviews and final lay-out resulted in minor adjustments as text type size, specific font, and lining for optimizing readability. In conclusion, we consider the TFI to be translated in such rigorous manner that the instrument can be further tested in clinical practice. The overall objective of the questionnaire being to identify frailty and improve the interventions relating to frail elderly persons in Denmark.

Methods: The validity of responses to the questionnaire was evaluated by Cognitive Interviewing in 16 Danish men and women aged 21—70 years. Construct validity was validated in 342 men and women aged 35—66 years by assessing agreement between 24-h MET-scores obtained from average weekly physical activity measured by PAS 2 and a 24-h Physical Activity Scale (PAS 1), previously found to overestimate physical activity. Results: Cognitive Interviewing revealed few problems in the questions on physical activity in different domains. No problems regarding the structure of the questionnaire were identified. Cognitive Interviewing in survey design state of the science and future directions
2018Co-Authors: Gordon WillisAbstract:Survey pretesting, and Cognitive Interviewing in particular, are important elements of survey development, and contributions from psychology have made Cognitive Interviewing one of the most powerful and widely used pretesting tools in survey research. However, widely accepted best practices for Cognitive Interviewing have been elusive and much of what is done in practice tends to be idiosyncratic, and this provides an extremely important area for future research. This chapter provides an overview of Cognitive Interviewing and suggests an agenda for future research that will shape the practice of survey pretesting.

Despite the widespread adoption of Cognitive testing, there remain large variations in the manner in which specific procedures are implemented, and it is not clear from reports and publications that have utilized Cognitive Interviewing exactly what procedures have been used, as critical details are often missing. Especially for establishing the effectiveness of procedural variants, it is essential that Cognitive Interviewing reports contain a comprehensive description of the methods used. One approach to working toward more complete reporting would be to develop and adhere to a common framework for reporting these results. In this article we introduce the Cognitive Interviewing Reporting Framework (CIRF), which applies a checklist approach, and which is based on several existing checklists for reviewing and reporting qualitative research. We propose that researchers apply the CIRF in order to test its usability and to suggest potential adjustments. Over the longer term, the CIRF can be evaluated with respect to its utility in improving the quality of Cognitive Interviewing reports.

Authors generally found the CIRF checklist to be usable, and that it led to a more complete description of key steps involved. However, despite the explicit direction by the CIRF to include a full explanation of major steps and features (e.g., research objectives and research design), the three Cognitive testing reports tended to simply state what was done, without further justification. Authors varied in their judgments concerning whether the CIRF requires the appropriate level of detail. Overall, we believe that current Cognitive Interviewing practice will benefit from including, within Cognitive testing reports, the 10 categories of information specified by the CIRF.
