For this reason, the therapeutic potential of contact lens may not be fully appreciated by eye care providers. To follow is an update on contact lens for the treatment of ocular surface Disease (OSD) and for the treatment of Corneal ectasia. Principles will be reviewed by general lens category, which typically refers to material, first for ocular surface Disease and then for Corneal ectasia and astigmatism. Therapeutic lenses can vary further in specific material and in design. Lens design is the collective parametric features of a particular brand and model of lens, other than optical power. Recent reports on the use of therapeutic lens will be reviewed, first for ocular surface Disease and then for Corneal ectasia and astigmatism.

To assess optical coherence tomography (OCT) for guiding design and fit of a prosthetic device for Corneal Disease. Methods. A prototype time domain OCT scanner was used to image the anterior segment of patients fitted with large diameter (18.5‐20mm) prosthetic devices for Corneal Disease. OCT images were processed and analyzed to characterize Corneal diameter, Corneal sagittal height, scleral sagittal height, scleral toricity, and alignment of device. Within-subject variance of OCT-measured parameters was evaluated. OCT-measured parameters were compared with device parameters for each eye fitted. OCT image correspondence with ocular alignment and clinical fit was assessed. Results. Six eyes in 5 patients were studied. OCT measurement of Corneal diameter (coefficient of variation, CV = 0.76%), cornea sagittal height (CV = 2.06%), and scleral sagittal height (CV = 3.39%) is highly repeatable within each subject. OCT image-derived measurements reveal strong correlation between Corneal sagittal height and device Corneal height (r = 0.975) and modest correlation between scleral and on-eye device toricity (r = 0.581). Qualitative assessment of a fitted device on OCT montages reveals correspondence with slit lamp images and clinical assessment of fit. Conclusions. OCT imaging of the anterior segment is suitable for custom design and fit of large diameter (18.5‐ 20mm) prosthetic devices used in the treatment of Corneal Disease.

Nerve growth factor therapy for Corneal Disease

NGF plays a key role in the modulation of immune reaction, trophic support, healing of ocular surface, Corneal sensitivity and tear film function. These properties of NGF make this neurotrophin a potential therapeutic agent for several Corneal Diseases. In this review, experimental evidence of the mechanisms of action of NGF on the ocular surface and clinical data on topical NGF use are described and discussed. This review includes the studies performed on Corneal Diseases such as neurotrophic keratitis, peripheral ulcerative keratopathy, dry eye and Corneal surgery. Moreover, experimental studies that extended the NGF action on herpes virus Corneal infection and ocular surface stem cell differentiation and proliferation are also reviewed. Since the first clinical use of topical NGF therapy in patients with neurotrophic keratitis, the ocular surface healing and immune-modulating actions of NGF have been extensively studied and demonstrated in the past two decades, opening new perspectives for its use in clinical practice in patients with infective and noninfective Diseases of the ocular surface.

We evaluated the effects of nerve growth factor in patients with this disorder. Methods Twelve patients (14 eyes) with severe Corneal neurotrophic ulcers associated with Corneal anesthesia were treated with topical nerve growth factor 10 times daily for two days and then 6 times daily until the ulcers healed. Treatment continued for 2 weeks after the ulcers healed, and the patients were then followed for up to 15 months. The evolution of the Corneal Disease during treatment and follow-up was evaluated by slit-lamp examination, photography, fluorescein-dye testing, and tests of Corneal sensitivity and best corrected visual acuity. Journal of Virology, 2018Co-Authors: David J Davido, Eleain M Tu, Hong Wang, Maria Korom, Andreu Gazquez Casals, Jahnu P Reddy, Heba H MostafaAbstract:We previously isolated a herpes simplex virus 1 (HSV-1) mutant, KOS-NA, that carries two nonsynonymous mutations in UL39, resulting in L393P and R950H amino acid substitutions in infected cell protein 6 (ICP6). Our published data studying KOS-NA pathogenesis strongly suggest that one of these ICP6 substitutions expressed from KOS-NA, R950H, severely impaired acute viral replication in the eyes and trigeminal ganglia of mice after inoculation onto the cornea and consequently impaired establishment and reactivation from latency. Because of its significant neuroattenuation, we tested KOS-NA as a potential prophylactic vaccine against HSV-1 in a mouse model of Corneal infection. KOS-NA stimulated stronger antibody and T cell responses than a replication-competent ICP0-null mutant and a replication-incompetent ICP8-null mutant optimized for immunogenicity. Immunizations with the ICP0−, ICP8−, and KOS-NA viruses all reduced replication of wild-type HSV-1 challenge virus in the Corneal epithelium to similar extents. Low immunizing doses of KOS-NA and the ICP8− virus, but not the ICP0− virus, protected mice against eyelid Disease (blepharitis). Notably, only KOS-NA protected almost completely against Corneal Disease (keratitis) and greatly reduced latent infection by challenge virus. Thus, vaccination of mice with KOS-NA prior to Corneal challenge provides significant protection against HSV-1-mediated Disease of the eye, even at a very low immunizing dose. These results suggest that KOS-NA may be the foundation of an effective prophylactic vaccine to prevent or limit HSV-1 ocular Diseases. IMPORTANCE HSV-1 is a ubiquitous human pathogen that infects the majority of the world's population. Although most infections are asymptomatic, HSV-1 establishes lifelong latency in infected sensory neurons, from which it can reactivate to cause deadly encephalitis or potentially blinding eye Disease. No clinically effective vaccine is available. In this study, we tested the protective potential of a neuroattenuated HSV-1 mutant (KOS-NA) as a vaccine in mice. We compared the effects of immunization with KOS-NA to those of two other attenuated viruses, a replication-competent (ICP0−) virus and a replication-incompetent (ICP8−) virus. Our data show that KOS-NA proved superior to the ICP0- and ICP8-null mutants in protecting mice from Corneal Disease and latent infection. With its significant neuroattenuation, severe impairment in establishing latency, and excellent protective effect, KOS-NA represents a significant discovery in the field of HSV-1 vaccine development.

Attitudes and Perception Towards Eye Donation in Patients with Corneal Disease: A Case-controlled Population-based Study

Design Cross-sectional, population-based study. Methods The Corneal Opacity Rural Epidemiological (CORE) study included 12 899 participants from 25 randomly selected clusters of rural Gurgaon, Haryana, India, with the primary objective of determining the prevalence of Corneal Disease in the general population during July 2011 to January 2013. VR-QoL was assessed through Indian Vision Function questionnaire (IND-VFQ-33) in adult participants (aged ≥18 years) detected with Corneal opacity and equal number of healthy controls (no ocular pathology with visual acuity of 6/6 binocularly) selected from the same clusters. Scores of the three subscales of IND-VFQ-33 (vision-specific mobility, psychosocial impact and visual symptoms) were computed, analysed and compared separately across various groups. Results Overall, 12 113 participants of all ages underwent detailed ophthalmic examination and VR-QoL was assessed in 435 cases with Corneal Disease and 435 controls without any ophthalmic Disease. The Diseased population had significantly higher scores and hence poorer VR-QoL across all three domains of vision function (scores of 28 vs 22; 6 vs 5 and 14 vs 9, respectively; p Conclusions VR-QoL is impaired in patients with Corneal Disease, more so in patients with Corneal blindness. This is the first population-based study to document VR-QoL through IND-VFQ-33 in the Indian population with Corneal Disease.

Prevalence of Corneal Diseases in the rural Indian population: the Corneal Opacity Rural Epidemiological (CORE) study

Design Population-based study in India with 12 899 participants of all ages. Methods Participants were recruited from 25 village clusters of district Gurgaon, Haryana, India using random cluster sampling strategy. All individuals were examined in detail with a portable slit lamp for evidence of any Corneal Disease during the door-to-door examination. Comprehensive ocular examination including logMar visual acuity, slit lamp biomicroscopy, non-contact tonometry and dilated retinal evaluation was performed at a central clinic site in the respective villages. Results Overall, 12 113 of 12 899 people (93.9% response rate) were examined during the household visits. Prevalence of Corneal Disease was 3.7% (95% CI 3.4% to 4.1%) and that of Corneal blindness was 0.12% (95% CI 0.05% to 0.17%). Multivariable analysis demonstrated that Corneal Disease was significantly higher in the elderly (p Conclusions The study findings demonstrate that currently ocular trauma, infectious keratitis, post-surgical bullous keratopathy, and Corneal degenerations are responsible for the major burden of Corneal blindness and morbidity in the Indian population. The prevalence of Corneal morbidity due to vitamin A deficiency and trachoma was low in this rural population.
