    Abstract:

    To identify the impact of MUC5B polymorphisms on Dental Caries. A case-control study was performed with patients recruited at Pontifícia Universidade Católica do Paraná. These individuals were aged 12 years old or more. Dental Caries was diagnosed using the International Caries Detection and Assessment System, and the effects of socioeconomic, dietary, and hygiene factors on Dental Caries were investigated. Furthermore, buccal cells were collected, and their DNA was extracted and amplified using PCR. Uni-, bi-, and multivariate analyses were performed. Two hundred patients were recruited, 100 were assigned to the case group and 100 to the control group. In the bivariate analysis, the following variables showed significant results: ethnicity (p = .008), biofilm (p < .001), and gingivitis (p < .001). The MUC5B gene affected Dental Caries with the markers rs2735733 (p < .001), rs2249073 (p < .001), and rs2857476 (p < .001). In the multivariate analysis, the biofilm variable remained significant (p = .026), as did the following markers from the MUC5B gene: rs2735733 (p = .019), rs2249073 (p < .025), and rs2857476 (p < .005).     PURPOSE The purposes of this investigation were to: (1) provide a descriptive account of untreated Dental Caries for Virginia schoolchildren; and (2) examine the relationship between a child's insurance status and the presence of untreated Dental Caries. METHODS In 1999, the Virginia Department of Health completed an Oral Health Assessment on first-, third-, and 10th-grade children. The oral assessment used a probability proportional to size sampling scheme to obtain a representative sample of public schoolchildren. Descriptive and multivariable regression analyses were completed to examine the relationship between insurance status and the presence of untreated Dental Caries. RESULTS In the primary dentition, schoolchildren with no insurance had the highest prevalence of untreated Dental Caries (65%) compared to: (1) schoolchildren with medical insurance only (42%); and (2) those with both medical and Dental insurance (25%). With the exception of children having only medical insurance, children enrolled in the free and reduced lunch program were more likely to have untreated Dental Caries. CONCLUSIONS In the primary dentition, children with no insurance were more likely to have untreated Dental Caries than those with both medical and Dental insurance. Insurance status does not appear to be associated with untreated Dental Caries in the permanent dentition.

    ABSTRACT Background The multiple functions of saliva play a significant role in the prevention of Dental Caries. Methods Chewing gum is known to stimulate salivary flow, and the results of studies of the role of stimulated saliva in the oral clearance of food particles, neutralization of Dental plaque acids and reduction of the incidence of Dental Caries have been reported. The author reviews the results of these clinical Caries trials. Results Seven clinical trials have evaluated the impact of chewing gum on Caries incidence. These studies have shown that chewing sugar-free gum after meals results in a significant decrease in the incidence of Dental Caries and that the benefit is due to stimulating salivary flow rather than any chewing gum ingredient. Conclusions Stimulating salivary flow through the chewing of sugar-free gum after meals has been shown to reduce the incidence of Dental Caries. Clinical Implications Practical measures for stimulating salivary flow after meals or snacks should be considered in Caries prevention programs.

    As with any disease process, the early initiation of therapy for Dental Caries is often the most effective means to ensure resolution. However, for any therapy to be effective, early diagnosis is paramount to success. Unfortunately, current tools used in Dental Caries detection are not sensitive enough to diagnose the disease process in its early stages and, frequently, once a diagnosis is made restoration is the only effective means of treatment. Quantitative light-induced fluorescence, electrical conductance measurements, direct digital radiography, and direct fiber optic transillumination are methods that may offer promise as effective tools in diagnosing early Dental Caries. This article describes some of the research that has been performed to make these methods a reality.

    Abstract:

    All 55-yr-old citizens (n = 1012) of Oulu (a middle-sized Finnish town) were invited to a clinical examination; 780 agreed to participate. The associations of lifestyle with decayed tooth surfaces and the number of teeth were studied in the 533 dentate subjects. Measures of lifestyle included dietary habits, a smoking habit, alcohol consumption and physical activity. When the associations of Dental Caries with lifestyle, sex, Dental health behavior, social class and number of teeth were studied by logistic regression analysis, lifestyle was shown to have an independent association with Dental Caries. Further analysis of the data showed that Dental Caries increased with a more negative lifestyle in every social class, but most significantly in the lowest. Number of teeth was not associated with lifestyle, but subjects with lower social status had fewer teeth than those with higher social status. The association of lifestyle with Dental Caries supports the idea that behavior in a broader sense should be taken into consideration in Caries prevention.
