Pentavalent Technetium-99m-Dimercaptosuccinic Acid Scintigraphy in Renal Osteodystrophy

The bone scan showed diffusely increased tracer uptake in the whole skeleton, and no change of tracer distribution was noted before or after vitamin D 3 pulse therapy. However, 99m Tc(V)-DMSA scintigraphy revealed diffusely increased tracer uptake in the whole skeleton before therapy, and markedly decreased tracer uptake in the bones was seen at 5 mo after therapy. Increased uptake of 99m Tc(V)-DMSA was observed at 7 mo after therapy, which reflected the laboratory findings. Technetium-99m-(V)-DMSA scintigraphy appeared to be more sensitive than the conventional 99m Tc-HMDP bone scan in assessing the characteristics and therapeutic effect of bone disease in ROD.

Pentavalent Technetium-99m Dimercaptosuccinic Acid Uptake in Primary Amyloidosis: Comparison with Autopsy Findings

Definite uptake of the tracer was noted in the thyroid, liver, spleen, and kidney as well as in the heart. Three months later, she died of heart failure, and autopsy findings demonstrated diffuse deposits of AL-type amyloid in those organs that were consistent with the accumulation of 99mTc(V)-DMSA.

European Journal of Nuclear Medicine and Molecular Imaging, 1994Co-Authors: Tsuneo Hirano, Tomio Inoue, Katsumi Tomiyoshi, Ying Jian Zhang, Tsunehiro Ishida, Keigo EndoAbstract:We describe a simple method of pentavalent technetium-99m Dimercaptosuccinic Acid [99mTc(V)-DMSA] preparation for the imaging of medullary carcinoma of the thyroid using commercially available kits. 99mTc(V)-DMSA is available at high pH (approximately 7.5) by adding NaHCO3 solution in the presence of a small amout of reducing agent (SnCl2). On the other hand, trivalent 99mTc-DMSA [99m-Tc(III)-DMSA] can be obtained at low pH (below 3) in the presence of an excess amount of reducing agent. In the clinical evaluation of a patient with a medullary carcinoma of the thyroid, only 99mTc(V)-DMSA revealed an area of intense accumulation.

MATERIALS AND METHODS: Tc-99m (V) DMSA scintigraphy was performed in 76 patients with histologic proof of soft-tissue tumors. In 57 of these patients, gallium-67 citrate scintigraphy was performed within 2 weeks after Tc-99m (V) DMSA scintigraphy. RESULTS: Uptake of Tc-99m (V) DMSA was clearly detected in almost all sarcomas, metastatic carcinomas, highly recurrent benign tumors (ie, extra-abdominal desmoids and tenosynovial giant cell tumors), superficial and deep hemangiomas, and granulomatous soft-tissue lesions but was not found in other benign solid tumors of soft tissue. Uptake of Ga-67 citrate was clearly detected in all inflammatory lesions and metastatic carcinomas, eight of 14 sarcomas (57%), and two of 10 highly recurrent benign tumors (20%) but was not found in hemangiomas or benign soft-tissue tumors. We therefore performed studies in animals (mice) and humans to investigate the effect of such oxygen bubbling on the labelling efficiency of and on the renal uptake of99mTc. The method of labelling of99mTc (V) DMSA was that of Hirano. It was found that oxygen bubbling oxidized the contaminated99mTc (III) DMSA into99mTc (V) DMSA in vitro and decreased the uptake of radioactivity in the kidney in both animals and humans.

AzevedoAbstract:Magnetic nanoparticles can be used for numerous in vitro and in vivo applications. However, since uptake by the reticuloendothelial system represents an obstacle for the achievement of nanoparticle diagnostic and therapeutic goals, the aim of the present study was to evaluate the uptake of Dimercaptosuccinic Acid coated magnetic nanoparticles by reticuloendothelial system phagocytic cells present in lymph nodes, spleen, and liver tissue and how the presence of these particles could have an impact on the morphology of these organs in capuchin monkeys (Sapajus spp.). Animals were intravenously injected with Dimercaptosuccinic Acid coated magnetic nanoparticles and euthanized 12 hours and 90 days post-injection. Organs were processed by transmission electron microscopy and histological techniques. Samples of spleen and lymph nodes showed no morphological changes. Nevertheless, liver samples collected 90 days post-administration showed slight morphological alteration in space of Disse. Moreover, morphometrical analysis of hepatic mitochondria was performed, suggesting a clear positive correlation between mitochondrial area and Dimercaptosuccinic Acid coated magnetic nanoparticles administration time. The present results are directly relevant to current safety considerations in clinical diagnostic and therapeutic uses of magnetic nanoparticles.

We describe synthetic routes used to produce iron oxide-based magnetic nanoparticles, subsequently coated with DMSA as well as functionalization strategies for specific purposes with polymers, antibodies, and cytokines. Finally, we have collected data on biological interactions of DMSA-coated nanoparticles in vitro and in vivo, in particular cell interaction process, pharmacokinetics, and biodistribution in different animal models and their promising applications in drug delivery, NMR imaging, hyperthermia, nanothermometry, magnetic separation, and bioremediation.

Expression patterns of cell adhesion molecules in mice's lung after administration of meso-2,3-Dimercaptosuccinic Acid-coated maghemite nanoparticles.

AzevedoAbstract:We studied the expression pattern of cell adhesion molecules associated to transendothelial migration of leukocytes in different lung's vascular compartments after administration of a magnetic fluid sample containing maghemite nanoparticles surface-coated with meso-2,3-Dimercaptosuccinic Acid. The analyses were conducted in mice 4 and 12 h after endovenous administration of the magnetic fluid in control mice. Firstly, the migratory activity of leukocytes after magnetic fluid surface-coated with meso-2,3-Dimercaptosuccinic Acid administration was confirmed using broncho-alveolar lavage and light microscopy. Then, the expression of cell adhesion molecules in the lung's vascular compartments was investigated by immunofluorescence microscopy of frozen sections, using antibodies against L-selectin, P-selectin, E-selectin, macrophage antigen-1, and leukocyte function associated antigen-1. L- and P-selectin showed similar pattern of expression in the pulmonary vasculature in animals treated with magnetic fluid and in the control group. In contrast, macrophage antigen-1 and leukocyte function associated antigen-1 were found in capillary only in animals treated with magnetic fluid surface-coated with meso-2,3-Dimercaptosuccinic Acid administration. In addition, after magnetic fluid administration E-selectin was found in post-capillary sites. Our findings demonstrated that magnetic fluid surface-coated with meso-2,3-Dimercaptosuccinic Acid administration exhibits modulation effects on expression patterns of E-selectin, macrophage antigen-1, and leukocyte function associated antigen-1 in the lung's vascular compartments. These findings are very important in a strategy to reduce the potential toxicity of magnetic fluid surface-coated with meso-2,3-Dimercaptosuccinic Acid administration for medical applications.

Intravenous sodium calcium edetate has been the preferred agent, but there is increasing evidence that Dimercaptosuccinic Acid (DMSA) is also a potent chelator of lead. Methods: Oral DMSA 30 mg/kg/day was administered to adults with blood lead concentrations > 50mg/dl. The impact of DMSA on urine lead excretion, on blood lead concentrations and on symptoms was observed. The incidence and severity of adverse effects was also recorded. Results: Thirty-five courses were given to 17 patients. DMSA significantly (P < 0.0001) increased urine lead excretion and significantly (P < 0.0001) reduced blood lead concentrations. Methods. Medline, Toxline, and Embase were searched and 912 papers were identified and considered. Pharmacokinetics and pharmacodynamics. DMSA is absorbed rapidly but incompletely after oral administration, probably through an active transporter. There is evidence that enterohepatic circulation occurs. Most DMSA in plasma is protein (mainly albumin)-bound through a disulfide bond with cysteine; only a very small amount is present as free drug, which is filtered at the glomerulus then extensively reabsorbed into proximal tubule cells. Nonfiltered protein-bound DMSA in peritubular capillaries is also available for uptake into proximal tubule cells by active anion transport at the basolateral membrane. DMSA therefore accumulate...
