    Background: Meal-Q and its shorter version, MiniMeal-Q, are 2 new Web-based Food Frequency Questionnaires. Their meal-based and interactive format was designed to promote ease of use and to minimize answering time, desirable improvements in large epidemiological studies. Objective: We evaluated the validity of energy and macronutrient intake assessed with Meal-Q and MiniMeal-Q as well as the reproducibility of Meal-Q. Methods: Healthy volunteers aged 20-63 years recruited from Stockholm County filled out the 174-item Meal-Q. The questionnaire was compared to 7-day weighed Food records (WFR; n=163), for energy and macronutrient intake, and to doubly labeled water (DLW; n=39), for total energy expenditure. In addition, the 126-item MiniMeal-Q was evaluated in a simulated validation using truncated Meal-Q data. We also assessed the answering time and ease of use of both Questionnaires. Results: Bland-Altman plots showed a varying bias within the intake range for all validity comparisons. Cross-classification of quartiles placed 70%-86% in the same/adjacent quartile with WFR and 77% with DLW. Deattenuated and energy-adjusted Pearson correlation coefficients with the WFR ranged from r=0.33-0.74 for macronutrients and was r=0.18 for energy. Correlations with DLW were r=0.42 for Meal-Q and r=0.38 for MiniMeal-Q. Intraclass correlations for Meal-Q ranged from r=0.57-0.90. Median answering time was 17 minutes for Meal-Q and 7 minutes for MiniMeal-Q, and participants rated both Questionnaires as easy to use. Conclusions: Meal-Q and MiniMeal-Q are easy to use and have short answering times. The ranking agreement is good for most of the nutrients for both Questionnaires and Meal-Q shows fair reproducibility.

    The Food Frequency questionnaire (FFQ) is the most commonly used dietary assessment instrument in epidemiologic research.1-6 Data collected with an FFQ are used to calculate nutrient intake and to relate consumption of Foods or nutrients to specific disease outcomes. Most FFQs include between 50 and 200 Food items.7 Like any structured self-administered questionnaire with prespecified response options, the FFQ provides the opportunity for nonresponse. Missing values can affect the calculation of nutrient intake and the estimation of the diet-disease relation. Few studies have investigated the distribution of blanks on an FFQ. In the Kaiser Permanente Diet Survey, a 60-item FFQ was administered; 41% of participants completed all questions and 12% left more than 10 items blank.8 In the Iowa Women's Health Study, 11% of the women left 10 or more items blank on a 126-item FFQ. 8,9 On the FFQ used in the New York University Women's Health Study, 3% of participants were missing 10 or more items.10 In the Norwegian Women and Cancer study, a 136-item FFQ had 18% missing values,11 and in the GISSI-Prevenzione study in Italy nonresponse increased from 0.3% in a first application of a diet questionnaire to 10% 12 months later and 20% 36 months after the first application.12 Nonresponse may differentially affect certain Foods, and individuals may differ in their likelihood of omitting Food items depending on personal characteristics. Food items on an FFQ may be omitted because the Food was not consumed or because of difficulties remembering the Frequency and amount of intake, especially because the FFQ does not provide a “Don't remember” option. In dietary analyses it is typically assumed that a blank on the questionnaire really means that the Food was not consumed, hence intake is set to zero.7 If the assumptions are misspecified, intake will be misclassified. We used data from the Nurses’ Health Study II (NHS II) to examine patterns and predictors of partial nonresponse on an FFQ. We resurveyed NHS II participants who omitted Food items to verify the assumption of zero intake.

    We compared fatty acid amounts in adipose tissue with fatty acid intake calculated from 2 separate weeks of diet recording and two Food-Frequency Questionnaires for 140 participants in the Nurses' Health Study. Our results showed that the amounts of polyunsaturated and trans fatty acids in adipose tissue reflect dietary intake and confirm those of previous studies. The correlation between the polyunsaturated fatty acid content of adipose tissue and polyunsaturated fatty acid intake from the average of the two Food-Frequency Questionnaires was 0.40; this correlation for trans fatty acids was also 0.40. Linolenic acid amounts in adipose tissue were also moderately correlated with intake from the average of the Food-Frequency Questionnaires (r = 0.34). An estimate of trans fatty acid intake from vegetable sources correlated much more strongly with adipose trans fatty acids than did an estimate of trans fatty acids from animal sources. Adipose tissue aspirates can be used to indicate intake of exogenous fatty acids.

    The accuracy and precision of four different Food intake assessment methods were evaluated in young and older women by comparing reported energy intakes with doubly labeled water measurements total energy expenditure (TEE). A study lasting 8 d was conducted in 10 young women aged 25.2 ± 1.1 y (x ± SEM) and in 10 older women aged 74.0 ± 1.4 y. Free-living TEE was measured over 7 d and Food consumption was determined from weighed Food intake data (7 d), a 24-h Food recall (in duplicate), and two different Food-Frequency Questionnaires [Fred Hutchinson Cancer Research Center (FHCRC)/Block and Willett, both in duplicate]. In addition, body composition was determined by using hydrodensitometry, and strenuous physical activity and the extent of dietary restraint were determined by questionnaire. In young women, 24-h recall gave mean energy intakes that were closest to measures of TEE (-0.34 ± 3.71 MJ/d compared with TEE, P = 0.178), and energy intakes by Food-Frequency Questionnaires were the only intake data that correlated significantly with individual values for TEE (P < 0.05). In older women, Food-Frequency Questionnaires gave mean energy intakes that were closest to measured TEE (+0.53 ± 2.95 MJ/d with the Willett questionnaire and -1.19 ± 3.02 MJ/d with the FHCRC/Block questionnaire). No energy intake data from this group correlated significantly with values for TEE. The 7-d weighed dietary intakes were significantly lower than measured TEE in both young and older women (-2.0 MJ/d in young and older women combined, P < 0.001), and did not correlate significantly with values for TEE, although they did most closely mirror the mean difference in TEE between the young and older women (2.30 MJ/d for TEE and 2.11 MJ/d for 7-d weighed intake). These data suggest that none of the methods studied gave accurate estimates of the usual energy requirements of individual subjects. In addition, the results suggest that for some types of studies, simple methods for assessing group mean dietary intake may actually give more accurate information than weighed dietary intakes.

    Abstract:

    The clear documentation during the last decade that dietary intake can be measured by standardized Food-Frequency Questionnaires has been fundamentally important for understanding the relation between dietary composition and health. Energy-adjusted correlation coefficients comparing nutrients assessed in this manner with those measured by dietary records or recalls have generally been in the range of 0.4-0.7 for a wide variety of populations and with use of Questionnaires that have varied greatly in length and detail. The latter finding, however, suggests that major increments in validity are not likely to be obtained by further refinements, although even modest gains are worthy of continued efforts. Additional effort must be given to developing and evaluating Questionnaires that extend the range of ages, populations, settings, and dietary factors that are studied. Comparisons with biochemical indicators of intake should be used when possible. Even well-established methods of assessment must be continually modified because of the rapidly changing patterns of Food consumption and dietary composition within contemporary populations.

    Most validation studies of Food Frequency Questionnaires have been done in populations capable of reporting their diet with unusual accuracy. In this study of ranchers and subjects selected at random, we compared nutrient intakes as assessed with multiple diet records with those assessed with a self-administered Food Frequency questionnaire (the Harvard-Willett instrument with 116 Food items). One hundred thirty-eight subjects from South Dakota and Wyoming, 64 males and 74 females, completed at least one Food Frequency questionnaire and multiple 1-day diet records (mean = 5 days) during a 6-month to 1-year period. Of invited subjects, 88% participated; 59% of participants had a high school education or less. For 22 dietary nutrients (excluding supplements) and alcohol, the median crude correlation between intakes based on diet records and the Food Frequency questionnaire completed before the diet records was 0.42; after adjustment for energy, age, and sex, and after correction for attenuation in the correlation coefficients due to error from a limited number of diet records, the median correlation was 0.52. Correlations for men and women were similar. The validity of the Food Frequency questionnaire in this population was similar to that reported previously and supports the use of self-administered Food Frequency Questionnaires in studies of general populations.

    A review of the literature concerning the design, utilisation and validation of Food-Frequency Questionnaires (FFQ) has been carried out using a semi-systematic approach to obtaining, reviewing and extracting data from articles. Databases were searched from 1980 to 1999. The present review identified 227 validation (from 1980 to September 1999) and 164 utilisation (for 1998 only) studies. A number of design issues have been evaluated through the present review. These include: the need to consider how portion sizes have been described, self-defined giving higher mean correlations; how an FFQ was administered, interviewer-administered giving higher mean correlations for some nutrients; how many items to include on an FFQ, those with the largest number of items having higher correlations. Validation techniques were described. Most validation studies involved comparing an FFQ against another dietary assessment method; only 19 % compared an FFQ to a biomarker. Measurement differences were most commonly assessed by correlation coefficients as opposed to other more appropriate methods. Mean correlation coefficients were highest for Ca and fat, and lowest for vitamin A and vegetables. The utilisation studies showed that FFQ were most commonly used in cross-sectional surveys, with ninety-three of the FFQ being designed to be disease-specific. The present review results were presented to a group of experts and a consensus arrived at concerning the development, validation and use of FFQ. Recommendations derived from the consensus arising from the literature review are presented as an appendix to the present paper.

    Objective: The purpose of this review is to provide guidance on the development, validation and use of Food-Frequency Questionnaires (FFQs) for different study designs. It does not include any recommendations about the most appropriate method for dietary assessment (e.g. Food-Frequency questionnaire versus weighed record). Methods: A comprehensive search of electronic databases was carried out for publications from 1980 to 1999. Findings from the review were then commented upon and added to by a group of international experts. Results: Recommendations have been developed to aid in the design, validation and use of FFQs. Specific details of each of these areas are discussed in the text. Conclusions: FFQs are being used in a variety of ways and different study designs. There is no gold standard for directly assessing the validity of FFQs. Nevertheless, the outcome of this review should help those wishing to develop or adapt an FFQ to validate it for its intended use.

    OBJECTIVES To develop the methods for assessment of Food and nutrient intake using standardized Food Frequency Questionnaires (FFQ) in three African origin populations from Cameroon, Jamaica and Caribbean migrants to the United Kingdom. DESIGN Cross-sectional assessment of diet from a representative sample in each site, using either a 2-day Food diary or a 24-h recall method to determine Foods for inclusion on the Food Frequency questionnaire. SETTING A rural and urban site in Cameroon, Evodoula and Cite Verte in Yaounde, respectively; a district in Kingston Jamaica; African-Caribbeans living in central Manchester, UK. SUBJECTS Aged 25-79 years, 61 from the Cameroonian urban site, 62 from the village site; 102 subjects from Jamaica (additional analysis on a subsample of 20): 29 subjects from Manchester, UK. MAIN OUTCOME MEASURES Foods contributing to nutrients in each site to allow the development of a FFQ. RESULTS A high response rate was obtained in each site. Comparison of macronutrient intakes between the sites showed that carbohydrate was the most important contributor to energy intake in Jamaica (55%) and the least in rural Cameroon. In rural Cameroon, fat (mainly palm oil) was the most important contributor to energy intake (44%). Manchester had the highest contribution of protein to energy (17%). Foods contributing to total energy, protein, fat and carbohydrate were determined. In rural Cameroon, the top 10 Food items contributed 66% of the total energy intake compared to 37% for the top 10 Foods in Manchester. Foods contributing to energy were similar in Jamaica and Manchester. Cassava contributed 44% of the carbohydrate intake in rural Cameroon and only 6% in urban Cameroon. One FFQ has been developed for use in both sites in Cameroon containing 76 Food items. The FFQ for Jamaica contains 69 Foods and for Manchester 108 Food items. CONCLUSION Considerable variations exist within sites (Cameroon) and between sites in Foods which are important contributors to nutrient intakes. With careful exploration of eating habits it has been possible to develop standardized, but locally appropriate FFQs for use in African populations in different countries.

    Objectives. This study reports on Eat for Life, a multicomponent intervention to increase fruit and vegetable consumption among African Americans that was delivered through Black churches. Methods. Fourteen churches were randomly assigned to 3 treatment conditions: (1) comparison, (2) self-help intervention with 1 telephone cue call, and (3) self-help with 1 cue call and 3 counseling calls. The telephone counseling in group 3 was based on motivational interviewing. The primary outcome, assessed at baseline and 1-year follow-up, was fruit and vegetable intake as assessed by 3 Food Frequency Questionnaires. Results. Change in fruit and vegetable intake was significantly greater in the motvational interviewing group than in the comparison and self-help groups. The net difference between the motivational interviewing and comparison groups was 1.38, 1.03, and 1.21 servings of fruits and vegetables per day for the 2-item, 7-item, and 36-item Food Frequency Questionnaires, respectively.     The validity of self-reported fruit and vegetable intake in minority populations has not been adequately established. In this study, the authors examined the association of three Food Frequency Questionnaires (FFQs) and 24-hour dietary recalls with serum carotenoid levels. Approximately 1,000 African-American adults recruited from 15 churches in Atlanta, Georgia (1997–1998) completed three fruit and vegetable FFQs: a seven-item instrument assessing intake during the past month; a two-item measure assessing usual intake; and a 36-item measure adapted from the Health Habits and History Questionnaire. A total of 414 participants received a 24-hour recall by telephone, and 105 of them received two additional recalls. Serum levels of lycopene, lutein, cryptoxanthin, -carotene, and s-carotene were assessed in 813 participants and used as the validity criterion. The correlations of fruit and vegetable servings with specific and total serum carotenoid levels were generally higher for the 36-item FFQ than for the two-item and seven-item instruments. The strongest correlation of fruit and vegetable servings with total carotenoid levels was observed for the three recalls (r = 0.42), with the 36-item FFQ and the single 24-hour recall yielding comparable correlations (r = 0.35 and r = 0.37, respectively). The validity of the 36-item fruit and vegetable FFQ was generally as strong as the validity of both 1 and 3 days of recalls.
