Moral dilemmas, moral reasoning, and Genital Herpes

The participants' Kohlbergian level of moral reasoning was assessed using the Kohlberg moral stage model. The moral orientation of the participants was assessed using Gilligan's moral orientation model, which distinguishes between justice and care reasoning. Methods We conducted two double-blind, randomized trials of a Herpes simplex virus type 2 (HSV-2) glycoprotein-D–subunit vaccine with alum and 3-O-deacylated-monophosphoryl lipid A in subjects whose regular sexual partners had a history of Genital Herpes. In Study 1, subjects were seronegative for Herpes simplex virus type 1 (HSV-1) and HSV-2; in Study 2, subjects were of any HSV serologic status. At months 0, 1, and 6, subjects received either vaccine or a control injection and were evaluated for 19 months. The primary end point was the occurrence of Genital Herpes disease in all subjects in Study 1 and in HSV-2–seronegative female subjects in Study 2. Genital Herpes Simplex Virus Infection in the Adolescent

In developed countries, during the 1990s, adolescent HSV type 2 seroprevalence rates ranged from 4 to 30% depending on the population studied. The clinical diagnosis of Genital Herpes may be unreliable and laboratory testing is recommended. Aciclovir, valaciclovir, and famciclovir are three antiviral drugs that have proven efficacy in the treatment of Genital Herpes. These drugs can be used in the treatment of the first episode or for recurrent infections, or can be used long term to suppress recurrent infections. Once or twice daily administration regimens are preferable for adolescents for reasons of adherence, and because it avoids the need to take medication at school. Unproven remedies are used commonly and can be harmful. In addition to antiviral therapy, proper management of the adolescent with Genital Herpes should also include developmentally appropriate explanations of the diagnosis, treatment and potential complications, recommendations for symptomatic relief, screening for other sexually transmitted infections, and discussion of safer sexual practices. All adolescents should have follow-up visits to complete education and counseling and to assess compliance with antiviral therapy.

College students' attitudes regarding vaccination to prevent Genital Herpes.

StanberryAbstract:Background and objectives Vaccines represent an important strategy for the control of infectious diseases but are only effective if accepted by the public. Unique attitudinal issues may affect usage of vaccines to control sexually transmitted diseases (STDs). The object of this study was to examine among college students factors that may influence acceptance of a vaccine against Genital Herpes. Goals To examine attitudes regarding acceptance of vaccination for Genital Herpes by college students using aspects of health behavior theories. Study design Subjects were recruited from an undergraduate psychology class to complete a questionnaire regarding attitudes and beliefs about receiving a potential Genital Herpes vaccine. Results Of the 518 subjects, 100 would not get the vaccine, 207 would, and 211 were unsure. In the final models, factors that influenced acceptance were low cost and an opinion that the vaccine should be recommended for everyone. Those who were unsure were more likely to be influenced by an opinion that the vaccine would be unsuccessful. While not remaining in the final models, those who believed they were at low risk of acquiring Genital Herpes were unlikely to accept a Genital Herpes vaccine. Conclusions Our results indicate that most college students would not reject vaccination, a priori, against Genital Herpes. Vaccine acceptance may be maximized if vaccine cost is low, the public is educated regarding risk of acquiring Genital Herpes, and public health policy encourages universal vaccination.

However, unlike partners with Genital Herpes in prospectively monitored couples, most persons who transmit Genital Herpes are not aware of having the infection. Methods Because HSV has a short incubation period and most persons who acquire Genital Herpes can identify the transmitting partner, a time-to-event design was used to assess risks of HSV acquisition among patients with newly acquired Genital Herpes. Results Among 199 persons with laboratory-documented newly acquired Genital Herpes, the median duration of the sexual relationship with the transmitting partner was 3.5 months, and the median number of sex acts before transmission was 40. The median time to HSV-2 acquisition was greater among participants whose partners disclosed that they had Genital Herpes, compared with participants whose partners did not disclose their status (270 vs. 60 days; P = .03). In multivariate models, having a partner who disclosed that he or she had Genital Herpes remained a strong protective factor against Genital HSV-2 acquisition (hazard ratio, 0.48 [95% confidence interval, 0.25-0.91]). Conclusion These findings suggest that testing persons with HSV type-specific serologic assays and encouraging disclosure may result in a decreased risk of HSV-2 transmission to sex partners.

Brown, Carolyn Gardella, Anna Wald, Rhoda Ashley Morrow, Lawrence CoreyAbstract:Approximately 22% of pregnant women are infected with Herpes simplex virus (HSV)-2, and 2% of women will acquire HSV during pregnancy. Remarkably, up to 90% of these women are undiagnosed because they are asymptomatic or have subtle symptoms attributed to other vulvovaginal disorders. Diagnosis of Genital Herpes relies on laboratory confirmation with culture or polymerase chain reaction assay of Genital lesions and type-specific glycoprotein G-based serologic testing. Neonatal Herpes is the most severe complication of Genital HSV infection and is caused by contact with infected Genital secretions at the time of labor. Maternal acquisition of HSV in the third trimester of pregnancy carries the highest risk of neonatal transmission. Despite advances in the diagnosis and treatment of neonatal Herpes, little change in the incidence or serious sequelae from this infection has occurred. As such, prevention of the initial neonatal infection is critically important. Obstetricians are in a unique position to prevent vertical HSV transmission by identifying women with Genital lesions at the time of labor for cesarean delivery, prescribing antiviral suppressive therapy as appropriate, and avoiding unnecessary invasive intrapartum procedures in women with Genital Herpes. Enhanced prevention strategies include identification of women at risk for HSV acquisition during pregnancy by testing women and possibly their partners for HSV antibodies and providing counseling to prevent transmission to women in late pregnancy.

BrownAbstract:As with many other sexually transmitted diseases (STDs), Genital Herpes infections affect women disproportionately in several ways. First, the infection is more prevalent among women, regardless of the level of sexual activity. Second, the disease tends to be more clinically severe among women, who are more likely to develop systemic illness and neurologic complications during the initial infection. Finally, due to the threat of potential complications during pregnancy and infection of the neonate, women are more affected by the consequences of Genital Herpes. For pregnant women with Genital Herpes infections, the fear of transmitting the infection to their newborn is a major concern. Infection of the newborn with HSV (Herpes simplex virus) is a rare but devastating disease. The incidence of neonatal Herpes varies geographically, with the Pacific Northwest and Scandinavian countries appearing to have the highest rates in the world. Universal immunization with an effective vaccine against HSV-2 infection, delivered in infancy, is likely to be the most effective strategy for control of Genital Herpes. Such strategy has proven effective against the chronic hepatitis B virus infection. Unfortunately, an effective vaccine appears unlikely in the next few years. A vaccine for Genital Herpes has been of interest for many decades, and several candidate vaccines have been tested. Promising results have been obtained with the recombinant glycoproteins D and B preparation.
