    Abstract:

    Aim To assess whether Dual Rinse HEDP, an etidronate that can be combined with NaOCl to create an endodontic irrigating Solution containing both Hypochlorite and a chelator in the form of 1‐hydroxyethane 1,1‐diphosphonic acid (HEDP), alters the clinical efficacy of NaOCl or adds any untoward clinical effects. Methodology In this randomized controlled double‐blind single‐centre trial, a pure NaOCl Solution was compared to a HEDP‐containing counterpart regarding antimicrobial efficacy, postoperative pain, and the host response by means of changes in MMP‐9 levels in periapical fluid. Sixty patients presenting with asymptomatic apical periodontitis (one tooth each) were randomly divided into two groups (N = 30) based on irrigation regime. Pre‐ and post‐treatment microbial aerobic and anaerobic cultures and MMP‐9/total protein (TP) periapical fluid samples were collected. Postoperative pain levels were assessed 24 h after treatment. Categorical data were compared between groups using the Fisher's exact test, continuous data using the Wilcoxon signed‐rank test, α = 0.05. Results Irrigation with pure NaOCl rendered 40% canals free of culturable microorganisms, compared to 50% with the NaOCl/HEDP mixture (P = 0.60). As assessed by matrix‐assisted laser desorption/ionization‐time‐of‐flight analysis (MALDI‐TOF), no apparent selection of aerobic or anaerobic taxa occurred in either group. One patient in the NaOCl group experienced moderate pain, whilst two patients in the NaOCl/HEDP group experienced mild postoperative pain. MMP‐9/TP levels in periapical fluid declined significantly (P 0.05). Conclusions This trial found no influence of HEDP on clinical NaOCl effects.

    Abstract:

    AIM To test whether the incorporation of a chelation powder, etidronate, marketed for root canal irrigation (Dual Rinse HEDP) into a sodium Hypochlorite (NaOCl) Solution induced additional cytotoxic and genotoxic effects not observed with NaOCl alone. METHODOLOGY Fresh and 24-h-old mixtures of 0.9 g of etidronate in 10 mL of 2.5% NaOCl were assessed for their basic chemical features including pH and the ability to chelate Ca$^{2+}$ from hydroxylapatite. Pure NaOCl and phosphate-buffered saline (PBS) with/without etidronate served as control Solutions. Cytotoxic and genotoxic effects of diluted Solutions (1:10, 1:100, and 1:1000) were assessed on Chinese hamster lung fibroblast (V79) using the MTT, clonogenic and micronucleus assays, respectively. One-way ANOVA and Tukey's HSD test were applied with an alpha-type error of 5% (P < 0.05). RESULTS In mixtures of NaOCl and etidronate, the free available chlorine was lost completely after 24 h, and the pH dropped by more than 3 units. However, the ability of the etidronate to chelate Ca$^{2+}$ was maintained. The fresh mixtures of NaOCl and etidronate were not more toxic than NaOCl alone (P > 0.05), whilst the 24-h-old mixtures were less toxic (P < 0.05) and statistically similar to pure etidronate. Etidronate per se showed little cytotoxicity and no genotoxicity at the tested dilutions. CONCLUSIONS The ability of the used etidronate, Dual Rinse HEDP, to chelate calcium is not affected by NaOCl. Cytotoxicity and genotoxicity of mixed Solutions is dictated by the presence of free available chlorine therein.

    Introduction: Many clinicians use household bleach to irrigate root canals. Sodium Hypochlorite Solutions are alsoavailable from dentalsuppliers. We compared physicochemical features of these products and investigated their impact on pulp tissue disSolution. Methods: Six different brands of household bleach were bought from drugstores. These were compared with ChlorXTRA and technical NaOCl Solutions of controlled concentration and alkalinity regarding their chlorine content (wt% NaOCl), pH, alkaline capacity, osmolarity, surface tension (Wilhelmy plate method), and price. Bovine pulp tissue (n = 10 specimens per group) disSolution at 37C by test and control Solutions adjusted to 1.0% NaOCl was assessed. Reduction in tissue weight was compared between groups by one-way analysis of variance, followed by Bonferroni correction (P .05) among all Solutions. Price was about 100-fold higher per liter of Chlor-XTRA compared with household bleach. Conclusions: Other than its price, the Chlor-XTRA Solution had no unique features. In contrast to an earlier report, reduced surface tension did not result in greater soft tissue disSolution by NaOCl. (J Endod 2012;-:1–4)

    Abstract:

    The aim of this study is to examine morphological changes of dentin surfaces following air polishing or amino acid buffered Hypochlorite Solution application and to assess their influence on periodontal ligament (PDL) cell survival, attachment, and spreading to dentin discs in vitro. Bovine dentin discs were treated with either (i) Classic, (ii) Plus, or (iii) Perio powder (EMS). Furthermore, Perisolv® a Hypochlorite Solution buffered with various amino acids was investigated. Untreated dentin discs served as controls. Morphological changes to dentin discs were assessed using scanning electron microscopy (SEM). Human PDL cells were seeded onto the respectively treated discs, and samples were then investigated for PDL cell survival, attachment, and spreading using a live/dead assay, adhesion assay, and SEM imaging, respectively. Both control and Perisolv®-rinsed dentin discs demonstrated smooth surfaces at low and high magnifications. The Classic powders demonstrated the thickest coating followed by the Powder Plus. The Perio powder demonstrated marked alterations of dentin discs by revealing the potential to open dentinal tubules even before rinsing. Seeding of PDL cells demonstrated an almost 100 % survival rate on all samples demonstrating very high biocompatibility for all materials. Significantly higher PDL cell numbers were observed on samples treated with the Perio powder and the Perisolv® Solution (approximately 40 % more cells; p < 0.05). SEM imaging revealed the potential for PDL cells to attach and spread on all surfaces. The results from the present study demonstrate that cell survival and spreading of PDL cells on root surfaces is possible following either air polishing or application with Perisolv®. Future in vitro and animal testing is necessary to further characterize the beneficial effects of either system in a clinical setting. The use of air polishing or application with Perisolv amino acid buffered Hypochlorite Solution was effective in treating root surfaces and allowed for near 100 % PDL cell survival, attachment, and spreading onto all root surfaces.

    The aim of this study was to determine the effect of 10% NaOCl gel and 10% NaOCl Solution on dentin bond strengths of four adhesive systems. One hundred eighty bovine incisors were ground to achieve a flat polished surface, then divided into 12 groups: Gluma One Bond [G1-control; G2-NaOCl Solution; G3-NaOCl gel]; Prime & Bond 2.1 [G4-control; G5-NaOCl Solution; G6-NaOCl gel]; Single Bond [G7-control; G8-NaOCl Solution; G9-NaOCl gel]; Prime & Bond NT [G10-control; G11-NaOCl Solution; G12-NaOCl gel]. Dentin was etched, rinsed, and blot dried. For the experimental groups, after acid etching, 10% NaOCl Solution or 10% NaOCl gel was applied for 60 s, rinsed, and blot dried. Composite resin was inserted and light cured. Shear bond strengths were tested with a crosshead speed of 0.5 mm/min. The mean values MPa (SD) were analyzed with two-way ANOVA and Tukey's tests (alpha < 0.01). Ten percent NaOCl Solution significantly increased Gluma One Bond strength. No effect was observed for the other adhesives. The 10% NaOCl gel did not affect bond strengths. Ten percent NaOCl gel was less effective on collagen removal as compared to 10% NaOCl Solution. The influence of collagen removal on bond strength is dependent on adhesive system, where both the solvent and the monomer can influence the results.

    Abstract A chlorine sanitizer that gives high disinfection efficacy with minimal available chlorine has a potential to be an environmentally-friendly method for disinfection of vegetables. In the present study, disinfection efficacy of slightly acidic electrolyzed water (SlAEW: pH 6.1, 20 mg/L available chlorine) produced by electrolysis for fresh cut cabbage was compared to that of sodium Hypochlorite Solution (NaOCl Solution: pH 9.6, about 150 mg/L available chlorine). SlAEW reduced about by 1.5 log CFU/g for total aerobic bacteria and 1.3 log CFU/g for moulds and yeasts, compared to fresh cut cabbage before dipping. Statistical analysis of the results showed that the disinfectant efficacy of SlAEW was equivalent to or higher than that of NaOCl Solution. Results also indicated that SlAEW under shaded and sealed conditions could keep its available chlorine during storage.

    Summary Actinomyces israelii has been repeatedly implicated as a cause of failure of endodontic therapy. This study investigated the antimicrobial effect of antibiotics as well as intracanal medicaments, sodium Hypochlorite Solution and calcium hydroxide, on this important pathogen. Growth of A. israelii was inhibited by low concentrations of antibiotics, yet high concentrations were not bactericidal for A. israelii over 1 week. When A. israelii was exposed for 2-6 weeks at concentrations equivalent to clinical serum levels, the antibiotics were lethal. The results reveal a species - specific antibiotic tolerance for A. israelii. Both sodium Hypochlorite Solution and calcium hydroxide were found to be highly eifective in killing A. israelii.
