    Backround: Mortality in asthma is very low but the disease has effects on quality of life and work ability. Here, the probability of long-term work disability associated with asthma alone or asthma together with chronic comorbidity(ies) was studied. Methods: A total of 2 332 asthmatic employees in public sector in Finland were analysed for long-term (≥90 days) work disability. Employees without asthma were used as control subjects (N=66 354) and were adjusted for age, gender, socioeconomic status, type of employment contract and type of organization. Diagnosis of asthma was based on reimbursement for asthma medication granted by the Social Insurance Institution and data on all sickness absences were obtained from national registers. Six main disease categories were used in this study as comorbidity for asthma: depression, ischemic heart disease, diabetes, rheumatic disease, malignancy and hypertension. Diagnoses for comorbidities were based on the reimbursement for disease medication by the Social Insurance Institution. Results: Asthma increased the risk of all-cause work disability with HR 1.8 (95% CI 1.6-2.1) compared to controls (no asthma). Asthma and one other chronic co-morbidity increased the risk for work disability with HR 2.2 (95% CI 1.8-2.8). Asthma together with two or more other chronic conditions increased the risk with HR 4.5 (95% CI 3.0-6.8). Depression together with asthma increased the risk of long-term work disability by threefold compared to those without asthma. Conclusions: Asthma alone or together with another chronic comorbidity increased the risk of long-term work disability and the risk was especially high for those with asthma and depression.

    OBJECTIVE To study the allocation of rehabilitation measures provided by the Finnish Social Insurance Institution in relation to the characteristics and health status of rehabilitants. DESIGN A register linkage study. SUBJECTS A total of 67,106 full-time local government employees with a minimum of 10-month job contracts in 10 Finnish towns during the period 1994-2002. METHODS Data on the rehabilitation granted between 1994 and 2002, special medication reimbursements for chronic diseases, and disability retirement, were derived from the registers of the Social Insurance Institution as an indicator of chronic morbidity and linked to the employers' records on demographic characteristics and rates of sickness absence. RESULTS In comparison with non-rehabilitants, the rate of sickness absence (> 21 days) was 2.2-2.9-fold (95% confidence interval (CI) 2.0-3.0) higher, the odds ratios of special medication reimbursement 1.5-6.1-fold (95% CI 1.3-6.9) higher and disability retirement 3.1-7.5-fold (95% CI 2.7-9.3) higher among rehabilitants. Older women and employees in manual or lower-grade non-manual jobs predominated in the rehabilitation groups. The proportion of temporary employees receiving rehabilitation was low. CONCLUSION Permanently employed older women with an excess burden of health problems predominate in the receipt of rehabilitation provided by the Social Insurance Institution.

    BACKGROUND: Population-based studies on the association between work stress and mental disorders are scarce, and it is not known whether work stress predicts mental disorders requiring treatment. AIMS: To examine the associations of work stress with DSM-IV mental disorders and subsequent antidepressant medication. METHODS: 3366 participants from a representative sample of the Finnish working population responded to a survey (The Health 2000 Study). 12-month prevalence of depressive or anxiety disorders was examined with the Composite International Diagnostic Interview. Data on antidepressant prescriptions with a 3-year follow-up period were collected from a nationwide register of Social Insurance Institution. RESULTS: In men and women, high job demands, low job control and high job strain were associated with 12-month prevalence of depressive or anxiety disorders. After adjustment for lifetime and baseline mental disorders, men with high job demands and high job strain had increased risk of future antidepressant medication. CONCLUSIONS: Work stress is associated with mental disorders among both sexes and among men it is a risk factor for mental disorders treated with antidepressant medication.

    Objective We set out to compare the agreement between self-reported psychotropic medication use and information obtained from the administrative prescription database of the Social Insurance Institution of Finland. We compared the point prevalence of psychotropic medication use, and self-reported vs. register-based information on antipsychotic medication dosage. Methods The study population consisted of 905 participants from a population-based genetic study of schizophrenia, of whom 366 had schizophrenia spectrum disorder, 56 had bipolar spectrum disorder, and 483 were unaffected family members. Current medication use was obtained by interview and from the prescription reimbursement database. Agreement between data sources was compared using Cohen’s kappa statistic and correlation coefficients. Results The agreement between the two sources was generally good.
