Safety of Intracytoplasmic Sperm Injection

Neri, Zev RosenwaksAbstract:Early follow-up studies of IVF children showed that the frequency of birth anomalies resembled those arising with natural conception. More detailed analyses confirmed these findings, reinforcing the concept of the preimplantation period as teratologically "safe." The use of Intracytoplasmic Sperm Injection (ICSI) to achieve fertilization introduced another variable.ICSI's safety has often been criticized because the fertilizing Spermatozoon neither binds to the zona pellucida nor fuses with oolemma. Bypassing these physiologic steps together with the arbitrary selection of the Spermatozoon has been reason for concern. Thus far, ICSI offspring undergoing adolescence and beyond has provided sufficient information to reassure these qualms. In fact, the health of the offspring generated through ICSI, once taken into consideration the gestational order, the age and the genetic makeup of the couples are generally reassuring.

We compared Intracytoplasmic Sperm Injection outcomes following the Injection of ejaculated or surgically retrieved Spermatozoa from these men.Materials and Methods: A study was performed of Intracytoplasmic Sperm Injection cycles with no Spermatozoa on initial semen analysis and 100 or fewer following centrifugation (cryptozooSpermia). Only 16 couples that underwent Intracytoplasmic Sperm Injection cycles with ejaculated Spermatozoa and cycles with testicular Spermatozoa were included.Results: Initial analysis was done to compare outcomes between the 2 semen origins. There was no difference in the rate of normal or abnormal fertilization between the 2 groups. Sperm characteristics and outcome of human assisted fertilization by subzonal insemination and Intracytoplasmic Sperm Injection

Design A retrospective analysis of 300 consecutive cycles of assisted fertilization concerning 202 infertile couples was performed. One hundred fifty-three couples underwent 362 unsuccessful IVF cycles, whereas on 49 couples IVF was not performed because of poor Sperm characteristics. Setting Procedures were performed in an institutional research environment. Patients, Participants Couples in which the male partner was the presumed cause of repeated failure to achieve conception by IVF or in which seminal parameters were unacceptable for IVF. Interventions Three hundred transvaginal oocyte retrievals were performed after superovulation by GnRH agonist and gonadotropins. Main Outcome Measures After SUZI and Intracytoplasmic Sperm Injection the following parameters were evaluated: fertilization, cleavage, pregnancy, and implantation rates in relation to the Sperm parameters and the proportion of acrosome-free Spermatozoa after different treatments. Results Normal fertilization occurred in 18% of the oocytes treated by SUZI and in 44% after Intracytoplasmic Sperm Injection. Only the treatment by electroporation showed a positive correlation with the fertilization rate. Fourteen pregnancies were obtained after SUZI, 8 pregnancies after Intracytoplasmic Sperm Injection, and 8 pregnancies after a combination of the two procedures. A score calculated from the Sperm parameters after selection correlated with the fertilization obtained after SUZI, whereas a score calculated from the parameters before Sperm selection correlated with the pregnancy rate. Sperm morphology influenced the implantation rate of the embryos obtained with these two procedures. Conclusions Intracytoplasmic Sperm Injection and SUZI can successfully treat couples who fail IVF or who cannot benefit from IVF. Different treatments can be applied to semen samples to increase the number of acrosome-reacted Spermatozoa. The few significant relations found between Sperm characteristics and the outcome of assisted fertilization cannot predict the outcome.

Use of frozen-thawed testicular Sperm for Intracytoplasmic Sperm Injection

Design: Prospective clinical study. Setting: A university hospital. Patient(s): One hundred seventy-five azooSpermic men participating in a routine Intracytoplasmic Sperm Injection program. Intervention(s): The men underwent testicular biopsy for cryopreservation of tissue to be used in consecutive Intracytoplasmic Sperm Injection treatment cycles. Their female partners underwent controlled ovarian hyperstimulation for conventional IVF treatment. Main Outcome Measure(s): Fertilization and pregnancy rates. Result(s): In 77% of the patients, Spermatozoa could be harvested from the testis by an open testicular biopsy technique and used for Intracytoplasmic Sperm Injection after freezing and thawing of testicular tissue. Histopathologic evaluation revealed a Sertoli cell–only pattern in 21%, maturation arrest in 60%, and hypoSpermatogenesis in 19% of the patients. In 2.9% of the patients, carcinoma in situ or a germ cell tumor was detected. In all patients, viable Spermatozoa could be visualized after the tissue samples were thawed. One hundred thirty-five Intracytoplasmic Sperm Injection treatment cycles were performed, with a fertilization rate of 45% and a clinical pregnancy rate of 30% per oocyte retrieved. Conclusion(s): The use of frozen-thawed testicular tissue allows ovarian stimulation of the female partner to be timed and avoids cancellation of ovum pick-up when Spermatozoa cannot be retrieved.

In addition, we define predictive parameters for successful testicular Sperm extraction in these patients.Materials and Methods: A total of 154 patients with nonobstructive azooSpermia underwent multiple testicular biopsies to obtain testicular Spermatozoa and for histopathological diagnosis. Results of testicular Sperm extraction were related to suspected etiology of azooSpermia, patient age, maximal testicular volume, serum follicle-stimulating hormone and histopathology. When testicular Sperm extraction was successful, Intracytoplasmic Sperm Injection was performed.Results: Spermatozoa were obtained from 63 patients (41%). No potential predictive parameters precluded successful testicular Sperm retrieval. Fertilization was achieved in 74 of 76 Intracytoplasmic Sperm Injection cycles. Intracytoplasmic Sperm Injection: A Treatment for Extreme OligoSpermia
The Journal of urology, 1996Co-Authors: Manuel Gil-salom, Yolanda Minguez, Carmen Rubio, Amparo Ruiz, José Remohí, Antonio PellicerAbstract:AbstractPurpose: We evaluated the efficacy of Intracytoplasmic Sperm Injection in patients with extreme oligoSpermia.Materials and Methods: A total of 67 Intracytoplasmic Sperm Injection cycles was attempted in 58 infertile couples in which the husbands had extreme oligoSpermia (less than 100,000 Spermatozoa per ml. ejaculate).Results: Fertilization was achieved in 65 of 67 cycles. Mean fertilization rate per cycle was 66.4 percent. A total of 18 clinical pregnancies was obtained, for a pregnancy rate of 26.8 percent per started cycle. There were 4 miscarriages and 8 live births from 5 deliveries. Nine pregnancies are ongoing.Conclusions: Intracytoplasmic Sperm Injection in patients with extreme oligoSpermia is associated with high fertilization rates and offers the chance of pregnancy to these otherwise intractably infertile couples.

We investigated the use of a method of percutaneous needle aspiration previously used for diagnostic purposes to obtain testicular Sperm for Intracytoplasmic Sperm Injection.Materials and Methods: A method of percutaneous aspiration of Sperm was developed to facilitate Intracytoplasmic Sperm Injection. A total of 69 testicular aspirations were performed for diagnostic purposes and 179 to obtain Sperm on the day of egg retrieval for couples undergoing in vitro fertilization with Intracytoplasmic Sperm Injection. The procedures were performed in an outpatient facility.
