Plastic and Reconstructive Surgery, 2007Co-Authors: G J AlterAbstract:Background: Aesthetic surgery of female genitalia is an uncommon procedure, and of the techniques available, Labia Minora reduction can achieve excellent results. Recently, more conservative Labia Minora reduction techniques have been developed, because the simple isolated strategy of straight amputation does not ensure a favorable outcome. This study was designed to review a series of Labia Minora reductions using inferior wedge resection and superior pedicle flap reconstruction. Methods: Twenty-one patients underwent inferior wedge resection and superior pedicle flap reconstruction. The mean follow-up was 46 months. Aesthetic results and postoperative outcomes were collected retrospectively and evaluated. Results: Twenty patients (95.2 percent) underwent bilateral procedures, and 90.4 percent of patients had a congenital Labia Minora hypertrophy. Five complications occurred in 21 patients (23.8 percent). Wound-healing problems were observed more frequently. The cosmetic result was considered to be good or very good in 85.7 percent of patients, and 95.2 percent were very satisfied with the procedure. All complications except one were observed immediately after the procedure. Conclusions: The results of this study demonstrate that inferior wedge resection and superior pedicle flap reconstruction is a simple and consistent technique and deserves a place among the main procedures available. The complications observed were not unexpected and did not extend hospital stay or interfere with the normal postoperative period. The success of the procedure depends on patient selection, careful preoperative planning, and adequate intraoperative management.

Annals of Plastic Surgery, 1998Co-Authors: G J AlterAbstract:A new technique has been developed to reduce the Labia Minora yet maintain the normal Labial edge and color. Labia Minora enlargement can be congenital or acquired by chronic irritation, exogenous androgenic hormones, and stretching with weights. This can cause inflammation, poor hygiene, interference with sexual intercourse, or intermittent urinary self-catheterization. Aesthetically, asymmetrical or enlarged Labia Minora causes self-consciousness sexually and when the subject wears tight pants. Previously Labia Minora reduction was performed by amputation of the protuberant segment and oversewing the edge. Now, rather than amputation, a wedge of protuberant Labial tissue is excised and the Labial edges are reapproximated. Four patients have undergone this aesthetic procedure with excellent results. No complications occurred. The new technique is relatively simple and can greatly enhance the patient's confidence.

Between January 2011 and January 2016, 4 cases of traumatic urethrovaginal fistula in young girls were treated using a pedicle flap from the Labia Minora. The mean follow-up was 27 months. All patients voided well and achieved normal urinary control. One patient had recurrent urethrovaginal fistula. The remaining 3 patients required no additional treatment. A Labia Minora skin flap for urethral reconstruction in young girls with urethrovaginal fistula secondary to pelvic fractures is a reliable technique.

Pigmented vulvar lesions are present in 10% to 12% of white women and the Labia Minora are the most common site for vulvar melanomas and atypical melanocytic nevi. Therefore, to perform a correct reconstruction, the surgeon must absolutely have a good understanding of the anatomy of the Labia Minora, as well as of the surgical options available, one of which is the wedge resection in the vulvar region. A classical clinical case will be used here to highlight the usefulness of wedge resection, a simple surgical technique that is applicable to the Labia Minora of the vulva in addition to its standard anatomic sites (auricle, lip, and eyelid).

A variety of procedures with varying degrees of success have been described, but none has proved to be the ideal method. We have done 15 vaginal reconstructions using Labia Minora flaps. In the first eight patients, we used two Labia Minora flaps and sutured them together to create a neovagina. In the last eight patients this new technique of vaginoplasty using horse shoe Labia Minora flap was used. The neovagina is dilated gradually for a period of 3 months and at the end of 3 months the patient has an adequate sized vagina. All were done for vaginal agenesis. Horse shoe flap vaginoplasty in Mayer, Rokitansky-Kuster-Hauser syndrome is a new technique. The entire hairless skin within the Labia majora is used to create a neovagina. Both the Labia Minora along with the prepucial skin of the clitoris is elevated as a single flap thereby increasing the dimension of the Neovagina and also augmenting the blood supply of the flap, making it more reliable and finally the design simplifies the technique of suturing the neovaginal tube.

Labia Minora hypertrophy: causes, impact on women’s health, and treatment options

Bateni, Alessandro Porrello, Roberto PiergentiliAbstract:Introduction and hypothesis We provide a review of the literature about the onset and development of hypertrophy of the Labia Minora, together with some expert opinions on the appropriateness of Labiaplasty. Methods We searched PubMed and used popular search engines, with a greater emphasis on the physiology and hormone-mediated metabolism of these structures, and less emphasis on their surgical treatment. Results We describe major embryological, cytological, and biochemical features of this anatomical part and summarize the clinical aspects of its hypertrophy, evaluating types of discomfort reported by women and the medical treatments available. Also, based on what is known about the artificial elongation and spontaneous hypertrophy of the inner Labia, we illustrate and discuss the main biological factors that may trigger this medical condition. There are not enough data identifying a clear inheritance of inner Labia hypertrophy in the absence of other pathological conditions; instead, we found indirect evidence for an association with transient episodes of local inflammation either before birth or during puberty. We also analyze the role played by estrogen receptors and other factors with regard to the onset of this condition and highlight the importance of their timing in determining the size of women’s Labia Minora. Remarkably, most cases of enlarged Labia Minora should be considered as outliers that are within the physiological range of size variation described for these structures. Conclusions We generally advise against surgical treatment of Labia Minora, especially in young, pre-pubertal girls, unless specific medical conditions are also present and/or the psychological impact on the patient is deemed particularly negative.
