Aim To describe unusual variations in the root morphology and root canal systems of Mandibular first and Second Premolar teeth extracted for orthodontic reasons. Summary Normally Mandibular first and Second Premolar teeth have single roots with single canals. A 15-year-old patient presented for orthodontic treatment and two Mandibular Premolar teeth were examined post-extraction. The Mandibular first Premolar exhibited three distinct, separate roots and the Mandibular Second Premolar exhibited a C-shaped root canal system. The coronal morphology of each of the MandibularPremolarsrevealeddimensionsandanatomywithinnormallimits.Theincidence of a three-rooted Mandibular first Premolar is approximately 0.2%. Key learning points • Thorough clinical and radiographic interpretation is important in recognizing anomalous root and root canal systems. • The most common forms of root and canal systems and its aberrations must be understood to realize variations from normal do occur. • Successful root canal treatment requires an accurate diagnosis of the root canal system using all available aids. • Value of microcomputed tomography in the study of anatomy ex vivo and cone-beam tomography in clinical endodontics of complex Premolar cases is increasing.

DongAbstract:The objective was to review thoroughly the literature of the root and root canal morphology of the human Mandibular Second Premolar and compare the results with the Mandibular first Premolar. Published studies cite the anatomy and morphology of the Mandibular Second Premolar tooth for more than 7700 teeth. These studies were divided into anatomic studies reporting the number of roots, number of canals, and apical anatomy. Differences caused by gender and ethnicity have also been reported. Individual case reports of anomalies were included to demonstrate the extreme range of variation. Almost all of the teeth in the anatomic studies were single-rooted (99.6%). The incidence of 2 roots (0.3%) and 3 roots (0.1%) was extremely rare. Anatomic studies of the internal canal morphology found that a single canal was present in 91.0% of the teeth. A single apical foramen was found in 91.8% of the teeth. The incidence of more than 1 root (0.4%), more than 1 canal system (9.9%), and more than 1 foramen (8.2%) is lower than that of the Mandibular first Premolar tooth (2.0%, 24.2%, and 21.1%, respectively). However, the root and root canal morphology of the Mandibular Second Premolar can be extremely complex and requires careful assessment.

Rare Root Canal Configuration of Mandibular Second Premolar Using Cone-beam Computed Tomographic Scanning.

RingAbstract:Abstract The aim of this case report was to demonstrate how the use of cone-beam computed tomographic (CBCT) imagery is beneficial when treating a tooth with an unusual anatomic variation. A 43-year-old female patient presented to our private practice after a referral from her general dentist. Endodontic therapy was previously initiated and unable to be completed because of suspicious anatomic anatomy. CBCT imagery confirmed the presence of a C-shaped root configuration and 3 canals within the tooth. Root canal treatment was completed successfully without complications. The use of CBCT imaging was valuable in determining the root morphology and rare canal anatomy to enhance treatment and success.

Root canal morphology of Premolar teeth, either maxillary or Mandibular, and the discrepancies between first and Second Premolars, have been investigated and reported. A case report is presented of the root canal treatment of a Mandibular Second Premolar with four root canals. As far as the reviewed literature revealed, up to three root canals in Mandibular Premolars have been reported. No previous report of a similar case of four root canals in a Mandibular Premolar was found.

A thorough understanding of the basic root canal anatomy and its diversities are necessary for successful endodontic treatment. Mandibular Second Premolars are usually single-rooted and have one root canal. Incidence of three separate root canals in this tooth is quite rare and taurodontism with three separate canals has never been reported in literature so far. The use of cone-beam computed tomography scan in this rare case greatly contributed to making a confirmatory diagnosis and successful nonsurgical endodontic management thereafter.

The anatomy of the pulp chamber floor in these Premolars usually reveals one lingual and two buccal orifices at the same level. This case report describes a Second Premolar with three canals and an unusual pulpal floor anatomy with one distobuccal and one distolingual orifice at the same level and an orifice on the mesiolingual wall. Very careful examination of the pulpal space, preferably with an optical device, is recommended to locate any unusual orifices. (J. Oral Sci. 50, 363-366, 2008)

When three or more canals are present separation of the root canals has only been described at mid-root level. The present case describes root canal treatment of a root canal aberration at the level of the apical third (apical root canal separation starting at a length of 20 mm) where a combined use of the fiberoptic endoscope and the operating microscope was necessary to locate the canal orifices and to enter these orifices. The present report emphasizes the need of color contrast to enhance visualization.
