Ikinci olgumuz ise sagaltima ragmen duzelmeyen dermatolojik bulgular ile klinigimize basvurdu. Klinik muayenede alopesi, yaygin kabuklanma ve kil ortusunde matlasma goruldu. Deriden alinan punc biyopsi sonuclari, artan serum total tiroksin ve serbest T4 ve iliskili miliyar Dermatitis ile hipertiroidizm tanisi konuldu. Bu olgumuzda metimazol ve hiperimmun sut faktoru baslanildi.     Abstract:

    Allergies are often suspected in cats and they are mainly hypersensitivity reactions against insect bites, food- or environmental allergens. Cats, with non flea induced atopic Dermatitis, normally present with one oft he following reaction patterns: Miliary Dermatitis, eosinophilic Dermatitis, selfinduced alopecia or head and neck excoriations. None of these reaction patterns is nevertheless pathognomonic for allergic Dermatitis, therefore the diagnosis is based on the one hand on the exclusion of similar diseases on the other hand on the successful response on a certain therapy. Recently a study on the clinical presentation of cats with non flea induced atopic Dermatitis was published. In this study certain criteria for diagnosing atopy in cats were proposed. For therapy of allergic cats cyclosporin, glucocorticoids, antihistamines, hypoallergenic diets and allergen specific immunotherapy are used. This article should provide a recent overview on the clinical symptoms, diagnosis and therapy of feline allergic Dermatitis.

    Practical relevance:Hypersensitivity Dermatitis (HD) is often suspected in cats and is mostly caused by insect bites, food or environmental allergens. Cats with non-flea induced HD are reported to present frequently with one or more of the following cutaneous reaction patterns: Miliary Dermatitis, eosinophilic Dermatitis, self-induced symmetrical alopecia or head and neck excoriations/pruritus.Clinical challenges:None of the above patterns are, however, pathognomonic for non-flea induced HD and the diagnosis of this condition is based on exclusion of diseases presenting similarly and an adequate response to treatment. Therapeutic approaches to affected cats include use of immunomodulatory drugs (ciclosporin, glucocorticoids, antihistamines), hypoallergenic diets and allergen-specific immunotherapy.Evidence base:This review provides an update on the clinical signs, diagnosis and treatment of feline non-flea induced HD.   • Establishment of diagnostic criteria for feline nonflea‐induced hypersensitivity Dermatitis

    Abstract:

    Hypersensitivity dermatitides (HD) are commonly seen in cats, and they are usually caused by environmental, food and/or flea allergens. Affected cats normally present with one of the following clinical reaction patterns: head and neck excoriations, usually symmetrical self-induced alopecia, eosinophilic skin lesions or Miliary Dermatitis. Importantly, none of these clinical presentations is considered to be pathognomonic for HD skin diseases, and the diagnosis of HD is usually based on the exclusion of other pruritic diseases and on a positive response to therapy. The objectives of this study were to propose sets of criteria for the diagnosis of nonflea-induced HD (NFHD). We recruited 501 cats with pruritus and skin lesions and compared clinical parameters between cats with NFHD (encompassing those with nonflea, nonfood HD and those with food HD), flea HD and other pruritic conditions. Using simulated annealing techniques, we established two sets of proposed criteria for the following two different clinical situations: (i) the diagnosis of NFHD in a population of pruritic cats; and (ii) the diagnosis of NFHD after exclusion of cats with flea HD. These criteria sets were associated with good sensitivity and specificity and may be useful for homogeneity of enrolment in clinical trials and to evaluate the probability of diagnosis of NFHD in clinical practice. Finally, these criteria were not useful to differentiate cats with NFHD from those with food HD.

    Objectives: To evaluate the efficacy of ciclosporin in cats with allergic skin disease. Methods: Ten cats with signs of allergic skin disease were administered ciclosporin daily at a dose of 3·6 to 8·3 mg/kg for one month. None of these cats had previously responded to a hypoallergenic diet trial, and all animals had previously been treated with endectoparasiticidal drugs, with no improvement two weeks before entering the trial. On days 0 and 30, owners assessed pruritus with a visual analogue scale, and veterinarians evaluated cutaneous lesions. Results: All the cats had pruritus and erythema, five had alopecia, two had an eosinophilic plaque, one had Miliary Dermatitis and two had both alopecia and an eosinophilic plaque. Good or excellent improvement was observed in 40 per cent of cats for pruritus, 57 per cent of cats for alopecia and 60 per cent of cats for erythema.     The efficacy of cyclosporin in canine atopic Dermatitis has been well documented, but reports on its use in feline allergic dermatoses are scarce. The aim of this study was to evaluate the efficacy of cyclosporin in cats affected by pruritus and eosinophilic dermatoses. Ten otherwise healthy cats were selected. All cats had no response to a hypoallergenic diet trial, had not been treated with short-acting corticosteroids or antihistamines for at least 2 weeks, had not been treated with long-acting steroids for 2 months, and had been treated with endo- and ectoparasiticidal drugs with no improvement 2 weeks before entering the study. All cats were pruritic and had erythema, seven had alopecia, five had an eosinophilic plaque, three had facial pruritus, one had Miliary Dermatitis, and one had an eosinophilic granuloma. Cyclosporin was administered daily at the dosage of 5 mg/kg for 1 month. Owners assessed pruritus by means of a visual analogue scale, and veterinarians evaluated cutaneous lesions on days 0 and 30 by a newly developed index called Feline Eosinophilic Granuloma, Eosinophilic Plaque, Extension and Severity Index (FEGEPESI), similar to the Canine Atopic Dermatitis Extension and Severity Index (CADESI). Fifty percent of the cats had a reduction of pruritus ≥ 50%, and in 50% of the animals, the lesions improved or disappeared completely. However, overall pruritus and FEGEPESI scores did not improve significantly. In conclusion, cyclosporin was helpful in symptomatically treating about half of the cases of feline pruritus and eosinophilic dermatitides. Funding: Novartis.
