Infectious agents disseminating through the blood flow can damage the testis by causing orchitis. On the other hand, infections ascending via spermatic pathways typically lead to epididymitis [ 1 ].

Testicular abnormalities can also associate with chronic anemia, obesity, and neoplasia. Although many of the disorders that affect the abovementioned glands and tissues are congenital, acquired lesions may result in hypogonadism in children and adolescents.

This requires the combined action of the inhibitory anti-mullerian hormone (AMH, secreted by Sertoli cells) to block the development of the uterus and fallopian tubes from the mullerian duct, together with the trophic stimulus of testosterone (a Leydig cell product), which leads to virilization of the wolffian ducts. Additionally, the development of external genitalia depends on the conversion of testosterone to dihydrotestosterone by the enzyme 5-α-reductase. Failure of any of these mechanisms leads to deficient virilization or the so-called “male pseudohermaphroditism” syndromes.

Pediatric Pathology fellowship recruitment report of a survey conducted by the fellowship committee of the society for Pediatric Pathology

Bush, Raja RabahAbstract:Pediatric Pathology (PP) is a subspecialty of Pathology encompassing disease states during human development from the fetus to the young adult. Despite the existence of ACGME-accredited fellowship programs and opportunity for Pediatric Pathology subspecialty board certification, many Pediatric Pathology fellowship positions remain unfilled in North America. We sought to understand the difficulties in recruitment to the PP training programs by conducting a survey. A 3-pronged survey targeting Pathology residents (PR), PP fellows and recent fellowship graduates (FG and administered online via SurveyMonkey. There were 175 responses to PR survey, 29 to FG however, many are not exposed to Pediatric Pathology early on in training. The survey results suggest that existing PP fellowship positions likely will continue to exceed demand for subspecialty training. The results of the study could aid in developing strategies to boost recruitment to PP.

Fellowship Training in Pediatric Pathology: A Guide for Program Directors

TimmonsAbstract:ABSTRACT The Accreditation Council for Graduate Medical Education (ACGME) has provided guidance for specialty and subspecialty fellowship training programs by defining 6 core competencies that must be met. Furthermore, the ACGME has defined several program requirements for Pathology training, including those applicable to several Pathology subspecialties. However, the requirements are broad and lack specific details, particularly as they pertain to the unique nature of Pediatric Pathology. The Fellowship Committee of the Society for Pediatric Pathology examined the ACGME requirements and interpreted the guidelines with respect to their application to training in Pediatric Pathology. The Committee worked within the ACGME guidelines to provide an expanded and more comprehensive set of guidelines for use by Pediatric Pathology fellowship directors and trainees. The resultant document lists the educational goals, core competencies, and program requirements with specific application to Pediatric Pathology. In addition, methods for assessing and documenting the progress of the individual trainees as they progress through each requirement are provided. It is to be emphasized that many of the guidelines set forthwith are flexible, and allowances should be made for individual differences of each training program.
