primary seminal vesicle adenocarcinoma presenting as isolated metastasis to penis responding to chemotherapy and hormonal therapy

Gupta, Sumeet GujralAbstract:We report a rare case of isolated Penile metastasis from seminal vesicle adenocarcinoma associated with ipsilateral renal agenesis in a 62-year-old man. Imaging studies confirmed seminal vesicle origin, and biopsy revealed a papillary mucin-secreting adenocarcinoma, with cytokeratin 20 positivity and prostate-specific antigen negativity. The sigmoidoscopy and metastatic workup findings were normal. The patient received six cycles of 5-fluorouracil, leucovorin, and oxaliplatin chemotherapy and underwent bilateral orchiectomy. The patient was symptomatically better, and the Penile Swelling and seminal vesicle mass had regressed considerably. He later developed multiple lung metastases and died of the disease.

traumatic avulsion of the dorsal Penile artery mimicking a Penile fracture
The Journal of Urology, 2001Co-Authors: Noel A Armenakas, David A Hochberg, John A FracchiaAbstract:A 41-year-old black man presented with painful Penile Swelling. Two hours before presentation, during coitus, he heard a cracking sound and experienced immediate pain and Penile Swelling. He later admitted to continuing with intercourse for several minutes after the acute traumatic episode. Physical examination revealed marked edema and ecchymosis of the penis, confined to the shaft (part A of figure). He had no difficulty voiding but the presence of microhematuria prompted a retrograde urethrogram, which was normal. Presumptive diagnosis was Penile fracture and the penis was explored through a circumferential subcoronal incision. An extensive hematoma was identified below Buck’s fascia with its epicenter at the dorsal mid shaft. Careful dissection in this region identified an avulsed left dorsal Penile artery with no active bleeding (part B of figure). No other injuries were noted, and the remaining dorsal neurovascular bundle and the entire tunica albuginea were intact. The hematoma was evacuated and both severed arterial ends were ligated. The Penile incision was reapproximated using absorbable suture material. The patient was discharged from the hospital the following day. He did not have any untoward effects after ligation of the dorsal Penile artery and has normal erections.

Isolated corpus spongiosum and urethral injury during sexual intercourse—a rare case -

Penile fracture involved the rupture of corpus cavernosum and may be associated with rupture of corpus spongiosum or urethra. We report a case of a 22-year old man presenting with Penile Swelling and pain, with retention of urine after getting a trauma during sexual intercourse. After exploration in emergency, we found that there was isolated rupture of corpus spongiosum with urethra, both corpus cavernosum were intact. We did repair of the ruptured urethra and corpus spongiosum. In follow-up, the patient did not present any complications such as distortion of penis and erectile dysfunction.
