a systematic review of Simulated Patient methods used in community pharmacy to assess the provision of non prescription medicines

Methods The databases IPA (International Pharmaceutical Abstracts), EMBASE and MEDLINE were searched for articles published between 1990 and 2010 outlining studies using Simulated-Patient methods. Key findings Thirty studies from 31 articles were reviewed. The majority used Simulated-Patient methods to purely assess counselling behaviour of pharmacy staff, rather than as an opportunity to provide educational feedback to improve counselling behaviour. Conclusions Few Simulated-Patient studies have incorporated performance feedback to encourage behavioural change and improve counselling skills. Studies that incorporated feedback did not provide sufficient detail, and few studies have explored participant perceptions. Additionally, very few studies have employed scenarios involving children's medicines. Future studies should test the feasibility of using the Simulated-Patient method, with appropriate performance feedback and describe participant perceptions of the value and acceptability of this training method.

Developing communication skills in pharmacy: a systematic review of the use of Simulated Patient methods.

Methodology We searched Embase, Lilacs, Medline, Scielo, and Scopus databases between 1980 and 2008, using “communication skills”, “Patient counseling” and “pharmacist” as keywords. This search was then further refined by using “Simulated Patients”, “pseudo-customer”, “standardized Patients”, and “mystery shoppers” as additional keywords. Results The initial search identified 241 published studies. Once further refined, 15 studies met inclusion criteria. Conclusion The majority of studies had an assessment focus aimed at documenting counseling behavior of practicing pharmacists, rather than an educational focus aimed at equipping pharmacists with effective communication skills. In instances where Simulated Patient methods were used for educational purposes, little regard was given to the role of performance and corrective feedback in shaping communication behavior of pharmacists. The majority of studies failed to describe the competencies and skills being investigated in relation to communication in the practice of pharmacy. Practice implications Simulated Patient methods provide pharmacy educators with a tool for implementing communication skills in the practice of pharmacy and will serve as a basis for implementing communication skills development programs at the College of Pharmacy of the Federal University of Sergipe in Brazil.

Teaching smoking-cessation counseling to medical students using Simulated Patients

ZweiflerAbstract:Objective Our objective was to evaluate the effectiveness of using Simulated Patient instructors and the Ockene method to instruct third-year medical students in smoking-cessation counseling techniques. Design We used a clinical exercise with self-study preparation and Simulated Patient instructors. Methods One hundred fifty-nine students participated in a smoking-cessation counseling session in which cognitive and behavioral endpoints were assessed by Simulated Patient instructors and the students themselves. Results Student performance in the cognitive and behavioral components of model smoking-cessation counseling was accept-Specific areas of weakness, such as the tendency of students to underemphasize the personal and social benefits of smoking cessation, and to overestimate their competence on a number of skill items, were identified. Student evaluation of the exercise was positive. Conclusions Smoking-cessation counseling can be taught effectively to third-year medical students by Simulated Patient instructors during a clinical clerkship.

Our objective was to review papers reporting on the use of SP methodology in pharmacy practice research 2006-2016 in community pharmacies worldwide. Methods We searched EMBASE and MEDLINE for papers reporting on the use of mystery shopping in pharmacy settings, using a wide range of terms for SPs, based on previous review. We removed irrelevant papers, duplicates, papers not written in English, and review papers and reviewed remaining papers. Two reviewers carried out data abstraction, using the same tool as the previous review and inserting data into Excel, focusing on how the SP methodology is used. Key findings A total of 148 papers from 52 countries from all regions of the world were included in the review. A wide range of terms described the method, and Simulated Patient was the most common (49 papers). Most studies were cross-sectional (124), and most investigated only community pharmacies (115). The most common aim was to evaluate some aspect of pharmacists' or other staff's advice and counselling (94). Number of visits is 2-7785. Many papers did not cover details, such as number of visits planned, and carried out, scenario used, training and background of SPs, and ethical approval for the study. Conclusions The use of SP methodology has increased substantially in the field of pharmacy over the past decade. This is a useful method in a wide range of countries and settings. Greater detail is required in reporting.

A systematic review of the use of Simulated Patients and pharmacy practice research

A Simulated Patient is an individual who is trained to visit a pharmacy to enact a scenario testing specific behaviour of the pharmacist or pharmacy staff. The aim of this study was to provide a definitive review of the use of Simulated Patients as a methodological tool for pharmacy practice research. Method A systematic review was undertaken to identify all pharmacy practice studies that had used Simulated Patient methodology. The electronic databases searched to identify relevant studies were MEDLINE, EMBASE and CINAHL. Articles fulfilling all the following criteria were considered for inclusion in the review: primary reports of trials conducted in community pharmacy and drug store settings which used Simulated Patients to derive outcome measures. The review was not restricted by language or by country. The review was restricted to publications from 1976 to May 2005. Key findings In total, 56 full publications were retrieved for further examination, of which 46 studies were included in the review, including: nine randomised controlled trials, three controlled trials, 30 cross-sectional, two time-series and two ‘other’ studies. Ten publications were excluded: seven reviews, one laboratory-based study, one telephone survey and one study presented only as an abstract. Conclusions There has been steady growth in the use of Simulated Patient methodology over the past 30 years. Although Simulated Patients have received negative attention in the pharmaceutical media, they can be a rigorous and robust method of measuring practice if used appropriately. This review demonstrates the range of activities for which this method can be used, including the assessment of counselling and advice provision, the treatment of minor and major illness, and the assessment of the public health activities of pharmacy and drug store staff. Simulated Patient methodology has been used in developing countries to a similar, if not greater extent, than the developed world, demonstrating its versatility and applicability to pharmacy practice research globally.
