    Abstract:

    Gedetailleerde kennis van de architectuur van het corpus Spongiosum is nodig voor tissue engineering, maar ontbreekt. Twee penissen van overleden mannen die hun lichaam ter beschikking van de wetenschap hadden gesteld werden ingevroren; een penis in slappe toestand, de ander artificieel in erectie gebracht. Transversale en frontale (lengterichting) weefselcoupes werden genomen en gekleurd. Na digitalisering werden oppervlakten, vorm en weefseldichtheid gemeten en vergeleken. Ook werden een digitale en een stereolithografische 3D-reconstructie gemaakt. Oppervlaktepercentages van vasculaire holten in beide richtingen lieten een oppervlaktetoename zien in erectie van 127 % transversaal en 140 % frontaal. De transversale coupes hadden een vergelijkbare structuur op de verschillende locaties, maar een grotere weefseldichtheid in de glans penis. De vasculaire holten waren groter in de frontale coupes met meer incomplete septae. Dit werd bevestigd in de 3D-modellen. Deze studie geeft informatie over de architectuur van het corpus Spongiosum, die belangrijk is bij tissue engineering.

    Purpose: Urethral reconstruction is performed for urethral stricture or hypospadias correction. Research on urethral tissue engineering is increasing. Because the corpus Spongiosum is important to support the urethra, urethral tissue engineering should ideally be combined with reconstruction of a corpus Spongiosum. We describe a method to visualize and measure the architecture of the corpus Spongiosum, which is needed for scaffold design.Materials and Methods: The penis was dissected from 2 unembalmed male cadavers. One penis was flaccid and the other was erect, as induced by saline infusion. Both were frozen in ice. At 6 sites sections were obtained in the transverse and frontal directions. After digitalizing the stained sections the images were edited, area measurements were taken and a 3-dimensional reconstruction was made.Results: In transverse sections the mean area of the vascular lumen was 60% and 77% in the flaccid and the erect corpus Spongiosum, and in frontal sections it was 53% and 74%, respec...

    Abstract:

    We aim to design a new glanuloplasty procedure combined with Spongiosum to reduce the incidence of glans dehiscence and coronal fistula after proximal hypospadias repair. Patients who underwent urethroplasty by dorsal preputial island flap for proximal hypospadias between January 2014 and December 2016 were reviewed in this retrospective cohort study. Those who underwent Spongiosum-combined glanuloplasty comprised the new-maneuver group, whereas those who underwent conventional glanuloplasty comprised the control group. The incidence of complications was then compared. In the new-maneuver group, dysplastic corpus Spongiosum alongside lateral Buck's fascia (0.3-0.4 cm wide) on both sides of the urethral plate was separated from the proximal normal spongy tissue, joining into the glans wings to increase tissue volume and covering the neourethra in the glans penis. In the control group, the neourethra was covered with superficial fascia under the coronal sulcus. As a result, the new-maneuver and control groups comprised 47 and 28 patients, respectively. In the new-maneuver group, no glans dehiscence was detected; however, two (4.3%) patients had coronal fistula, two (4.3%) had urethral stricture, and four (8.5%) had diverticulum. In the control group, two (7.1%) patients had glans dehiscence, eight (28.6%) had coronal fistula, four (14.3%) had urethral stricture, one (3.6%) had diverticulum, and one (3.6%) had penile curvature recurrence. The new-maneuver group had less incidences of coronal fistula (P < 0.001), glans dehiscence (P = 0.033), and urethral stricture (P = 0.008) but had a higher incidence of diverticulum than the control group (P = 0.040). It clearly demonstrates that Spongiosum-combined glanuloplasty can significantly reduce the incidences of coronal fistula and glans dehisce.

    Abstract Objective To investigate the histomorphological changes of the divergent corpus Spongiosum surrounding the urethral plate in hypospadias. Methods Seventy boys with hypospadias were divided into the distal (n = 44) and proximal (n = 26) groups by the location of the ectopic meatus. The morphology of the divergent corpus Spongiosum was evaluated and the histopathological studies were carried out by HE staining, Masson staining, and immunohistochemical analyses of α-SMA, CD34, TGF-β1 and androgen receptor. Western blotting was performed to measure the expression of TGF-β1 and androgen receptor. Results The divergent corpus Spongiosum was significantly thicker in the distal group than the proximal group (distal 3.14 ± 1.12 mm, proximal 1.74 ± 0.87 mm, P Conclusions Histomorphological analysis showed that the divergent corpus Spongiosum in hypospadias is abnormal. The structural variation is positively correlated with the severity of hypospadias.

Histology and immunohistochemistry of blood vessels indicated an abnormal structure of cavernous sinus in hypospadias group.It was characterized by a wide vascular lumen [(control (7.20±1.12)μm, hypospadias (13.75±8.08), P<0.05], thickening of vascular wall [control (5.40±1.28) μm, hypospadias (14.11±7.59) μm, P<0.05], lowered cavernous sinus density (hypospadias 15.66±1.17, control 7.24±4.19, P<0.05), decreased cavernous trabecula density (hypospadias 9.80±1.92, control 3.68±2.87, P<0.05) and degree of structural abnormity was more obvious in proximal group.Immunohistochemistry of TGF-β1 showed a lower expression of TGF-β1 in hypospadias (P<0.05). Western bolt and qRT-PCR also confirmed the lowered expression of TGF-β1 in proximal hypospadias as compared with control group(all P<0.05). Conclusions The histomorphology of corpus Spongiosum is abnormal around urethral plate in hypospadias.And degree of variation and severity of hypospadias are positively correlated.     Abstract:

    Objective To explore the clinical efficacy of modified Cantwell-Ransley repair via a ventral penile approach for preserving the integrity of corpus Spongiosum for male epispadias. Methods Retrospective analysis was performed for 18 cases of isolated male epispadias since December 2015. The relevant clinical data included patient profiles, operative approaches and such outcomes as voiding condition, penile cosmesis and complications. Results Primary repair was performed in 15 cases (glanular, n=7; penile shaft, n=4; penopubic, n=4). Secondary salvage repair was performed (n=3). The median age was 21 (12-168) months. Kelly's plus Young-Dees-Leadbetter's procedure was performed for reconstructing bladder neck for incontinence. The average follow-up period was 13.1 (6-21) months. There were no such severe complications such as glanular dehiscence, loss of glans or corpora, urethral stricture or permanent urethrocutaneous fistula. All dorsal curvatures were corrected, yet penile length seemed shorter than normal boys. And 88.9% families were satisfied with penile cosmesis. Four incontinent cases achieved grade I-II urinary continence. Conclusions Modified Cantwell-Ransley repair may recover anatomical penile structure and avoid various complications by preserving the integrity of corpus Spongiosum for male epispadias.
