  • Exploring new horizons in health care: A mechanistic review on the potential of Unani Medicines in combating epidemics of infectious diseases.

    Abstract:

    The 20th and 21st centuries have witnessed epidemics and pandemics of various infectious agents. The development of effective antimicrobials in the 20th century has been complemented with the emergence of resistant and mutant strains. In this context, we present a comprehensive overview of the preventive measures described in Unani Medicine during epidemics. Unani Medicine is a traditional Medicine system included in the Indian Systems of Medicine. Unani Medicine has an extensive description of epidemic infections and preventive and therapeutic measures for the same. Certain factors like environment, season, and geographical location of a place are known to determine the extent of infections, and their escalation to epidemics. Maintenance of general health, immune-stimulation, and disinfecting of the environment are advised as protective measures, for which many drugs are prescribed. In the case of illness, specific antimicrobial drugs of natural origin are prescribed. Herein we discuss these measures in detail, along with the scientific evidences of anti-microbial, immunomodulatory, and health-protective actions of these drugs.

    Abstract Background: Unani system of Medicine originated in ancient Greece, with the teachings of Buqrat (Hippocrates, 460-377 BC); and was introduced in India by the Mughal emperors. In Unani Medicine, diseases and drugs are classified on the basis of mizaj (temperament) which is one of the fundamental concepts of Unani Medicine. Oral health is given special emphasis in Unani Medicine due to the understanding that diseases of the mouth affect eating and nutrition which may affect all systems of the body. Oro-dental diseases are described extensively in Unani literature in a systematic manner and many drugs such as Aqar Qarha (Anacyclus pyrethrum DC.), Kaat safed (Acacia catechu Willd.) etc are described which have shown promising results in recent scientific studies. Methodology: First, a review of Unani classical books was completed and the drugs to be further researched were selected. Then, a search was made on major scientific search engines regarding scientific evidence of their efficacy and safety. Conclusion: The Unani drugs mentioned in classical literature have shown promising results as anti-inflammatory, antipyretic, antiseptic, anti-bacterial, anti-fungal and anti-ulcerative agents in many oral disorders. Also, most of the drugs have a multi-dimensional action which is both curative and preventive.

    Abstract:

    Since December 2019, a respiratory pandemic named as coronavirus disease 2019 (Covid-19) caused by a new coronavirus named as SARS-CoV-2, has taken the world by storm. The symptoms are fever, malaise, and cough which resolve in a few days in most cases; but may progress to respiratory distress and organ failure. Transmission is through droplet infection or fomites, but other modes such as airborne transmission and oro-fecal transmission are also speculated. Research is underway to develop effective vaccines and Medicines for the disease. In such a scenario, we present the measures described in Unani system of Medicine for health protection during epidemics. Unani is a traditional system of Medicine developed during the middle ages, which employs natural drugs of herbal, animal and mineral origin for treatment. In Unani Medicine, during an epidemic, apart from isolation and quarantine, three measures are of utmost importance, (i) purification of surroundings using certain herbal drugs as fumigants or sprays, (ii) health promotion and immune-modulation, and (iii) use of health-protecting drugs and symptom-specific drugs. Drugs such as loban (Styrax benzoides W. G. Craib), sandroos (Hymenaea verrucosa Gaertn.) za'fran (Crocus sativus L.), vinegar etc. are prescribed in various forms. Scientific researches on these drugs reveal the presence of a number of pharmacologically active substances, which may provide a new insight into the management of infections and epidemics.

  • Historical account of endocrinal disorders in Unani Medicine.

    Abstract:

    Hippocratic doctrine of four humors and qualities is implicated to be a pioneer of modern endocrinology because of the concept of dyscrasia. Imbalance in humors causes disease. Unani scholars were aware of endocrinological disorders like endocrinologic syndrome (i.e., association of amenorrhea and galactorrhoea in a non-pregnant woman), castration, contraceptives techniques, infertility, obesity, diabetes etc., and also their mode of remedy, albeit with a phenomenological approach. Their understanding of the symptoms and signs related to endocrinologic syndromes, which were explained in detail in the recent account of the endocrine system, is presented here with historical chronology. The survey was carried out from the literature of the Unani system of Medicine, and the same was analyzed from the observations reported in various indexed journals and reputed books. The paper details the account of endocrinologic syndrome from the Greek era to the end of the medieval ages. - Description of the endocrinal disorders is mentioned in Unani Medicine for centuries, albeit with a different name for the disease category.- According to Unani Medicine, this disease arises due to abnormal quality and quantity of humour.- Renowned Unani scholars like Hippocrates (370-460 BC), Aristotle (384-322 BC), Aretaeos of Kappadokia(81-96 AD), Galen (130-200 AD), Zakaria Razi (865-925 AD), Majusi (930-994 AD), Ibn-e-Sina (980-1037 AD), Albucasis (1013-1106 AD) and Ismail Jorjani (1042-1137 AD) also treated such ailments through Unani Medicine (plant, mineral and animal origin drugs).-Majusi performed surgery of Goiter and mentioned in his treatise and recommended the method of surgery.-Albucasis performed the first successful thyroidectomy under opium (Papaver somniferum L.) sedation and simple ligatures and hot cautery in the name of 'elephantiasis' of the throat (goiter).- The principles of treatment in Unani Medicine may provide a guide for better management of endocrinal disorders (e.g., hypothyroidism, hyperthyroidism, diabetes etc.).- The treatment in Unani Medicine for various diseases/syndromes/signs, which are now likely associated with hormone dysfunction, can open up the new field of Unani therapeutics for these diseases for the exploration of better treatment options.

    Unani system of Medicine is based on the humoral theory postulated by Hippocrates, according to him the state of body health and disease are regulated by qualitative and quantitative equilibrium of four humours. Amraz-e-Waba is an umbrella term which is used in Unani Medicine for all types of epidemics (smallpox, measles, plague, Hameer Saifi, influenza, Nipaha, Ebola, Zika, and 2019 novel coronavirus, etc.) mostly fatal in nature. The coronavirus disease 2019 (COVID-19) is a severe acute respiratory infection, and the pathogenesis and clinical features resemble with those of Nazla-e-Wabaiya (influenza) and Zatul Riya (pneumonia) which were well described many years ago in Unani text such as high-grade fever, headache, nausea and vomiting, running nose, dry cough, respiratory distress, alternate and small pulse, asthenia, foul smell from breath, insomnia, frothy stool, syncope, coldness in both upper and lower extremities, etc. The World Health Organization declared COVID-19 as a global emergency pandemic. Unani scholars like Hippocrates (370-460 BC), Galen (130-200 AD), Rhazes (865-925 AD), and Avicenna (980-1037 AD) had described four etiological factors for Amraz-e-Waba viz., change in quality of air, water, Earth, and celestial bodies, accordingly mentioned various preventive measures to be adopted during epidemics such as restriction of movement, isolation or "quarantena", and fumigation with loban (Styrax benzoin W. G. Craib ex Hartwich.), sandalwood (Santalum album L.), Zafran (Crocus sativus L.), myrtle (Myrtus communis L.), and roses (Rosa damascena Mill.) and use of vinegar (sirka) and antidotes (Tiryaq) as prophylaxis, and avoiding consumption of milk, oil, sweet, meat, and alcohol. This review focuses and elaborates on the concept, prevention, and probable management of COVID-19 in the light of Amraz-e-Waba.

    Mankind has faced many hardships like natural disaster, drought and epidemics. Study focuses on epidemics caused by microbes.Unani Medicine has a long experience in treating epidemic diseases because its history is as old as the history of human being itself. More or less entire of the civilisations throughout the history became the basis for evolution of Unani Medicine. Hippocrates (460-380BC) regarded it asbothart and science, discussed the epidemics and wrote a book on Epidemics. Body is assumed healthy when the humours are balanced. So, Unani scholars have rightly said Fa’il (Active agent) is not able to produce any change (Actions & Reactions) in the body without the prior presence of Munfa’il(Pertinent) having the capacity to accept it like in Waba(epidemic). The aim is to explore the fundamental concept of Waba from the Unani literature and understand COVID-19 in reference to existing literature of Unani Medicine. The literature of Unani Medicine was surveyed for concept of Waba(Epidemic) & related concepts. Internet was used to access indexed papers using search engines like Medline, PubMed, Science Direct, etc. Logical preventive strategies like quarantine, and useof fumigants, prophylactic drugs are mentioned in Unani literature that have been used in epidemics with flue like symptoms. This knowledge and experience may be used for achieving methods for prophylaxis, cure or add on therapeutic measures for COVID-19 epidemic.

    Halela (Terminalia chebula Retz, Family: Combretaceae) is one of the drugs worthwhile to be used in Unani Medicine due to its extraordinary therapeutic efficacy. Globally the species is distributed in the Indo-Malesia region and Sri Lanka. Within India, it is distributed in the sub-Himalayan tracts to West Bengal and Assam. Halela is one of the drugs of choice to be used in Saudawi/ chronic diseases (diseases with the dominance of black bile) in Unani Medicine. Halela proposed to promotes digestion, absorption, elimination, rejuvenation, and elimination of black bile from the body (khilt sauda, a humour responsible to produce aging and a culprit for chronic diseases). Various scientific studies have also evidenced its beneficial effects. In this paper, a review of three different varieties of halela which have been used in Unani Medicine for centuries has been specified with special reference of Unani as well as other pieces of literatures.

    Abstract Hypothyroidism (Qillat-e-Ifraz-e-Darqiyya) is a condition where the thyroid gland is underactive and unable to produce enough thyroid hormone. The description of hypothyroidism as a disease is not directly found in Unani texts. However, the signs and symptom of hypothyroidism resemble the clinical manifestation associated with Su-e-Mizaj Barid Maddi (derangement in cold temperament), such as plethora (Imtila), excessive salivation (Kasrat-e-Luabe-e-Dahan), tiredness (Aa’yan), loss of appetite (Zoaf-e-Ishteha), excessive sleeping (Kasrat-e-Naum) and cold skin (Baroodat-e-Jildia). These signs and symptoms are the result of an excess in abnormal phlegm (Ghair Tabayi Balgham) in the body. This review article identifies the observations from Unani literature that describe derangement in cold temperament and relate them to the clinical presentation of primary hypothyroidism in conventional Medicine. We also discuss management of these symptoms in Unani Medicine.

    Coronavirus disease (COVID-19) is an infectious disease caused by a virus that has been detected recently. Most people infected with COVID-19 suffer from mild to moderate respiratory illness and recover without requiring special treatment. Older age group people and persons with underlying medical conditions such as cardiovascular disease, diabetes, chronic respiratory disease and cancer are more likely to develop serious illness. First case was observed in December 2019 in Wuhan city of China and later on spread all over the world and affected 213 countries. WHO declared it as a pandemic on 11th March 2020. The most common COVID-19 symptoms are fever, fatigue and dry cough. Some patients may have pain, congestion in their nose, runny nose, sore throat or diarrhoea. According to Unani Medicine, an epidemic is referred as Waba, it spreads only in the place where the natural state of air changes. Aside from isolation and quarantine, preventive measures must be taken during an epidemic. Some drugs are mentioned as preventive measures in various forms must be used during these days such as Gil-e-armani, Sirka, Halteet, Sibr, Loban etc.
