    Abstract:

    The treatment of pelvic floor disorders using biofeedback, behavioral therapies, and other Applied psychophysiological treatments has been well documented as effective. Practitioners must take due care to ensure that they practice within the boundaries of what is common practice for their discipline and within the scope of practice allowed by their professional license as outlined by the appropriate state licensing law(s), the ethical principles and practice guidelines and standards for their discipline, and those of the Association of Applied Psychophysiology and Biofeedback if using a biofeedback assessment or treatment. Being competent to provide a particular treatment does not necessarily make it legal and/or ethical. This paper provides a set of recommended practice guidelines for use in the assessment and treatment of pelvic floor disorders. Please note that they have not at this time been endorsed as an official position of the Association of Applied Psychophysiology and Biofeedback or any other professional organization.

    This paper raises issues in further refining the working definition of “ Applied Psychophysiology” proposed by Schwartz (1999). Issues to be addressed include defining what the word “psycho” means within the definition of Applied Psychophysiology and the implications of this word in practice. Other issues discussed include the meaning of the word “Applied,” and how incorporating the acquisition of self-regulatory skills or behaviors as one aspect of the definition of Applied Psychophysiology would help differentiate Applied Psychophysiology from other specialty areas. Finally, a revised definition is proposed to stimulate further discussion.

  • Creating the future of Applied Psychophysiology and biofeedback: from fantasy to reality.

    Abstract:

    AAPB and its membership are faced with a number of giant challenges, including but not limited to: (1) the cost savings efforts of third-party payors and managed care organizations; (2) the lack of public awareness of biofeedback and its usefulness; and (3) the lack of sufficient research data on both the effectiveness and efficacy of biofeedback. In spite of these challenges, there are windows of opportunity that have been or which could be created to move biofeedback further into the realm of conventional treatment. We must focus our efforts on working together to: (1) create strategic plans for creating the future of Applied Psychophysiology and biofeedback; (2) educate all decision makers, including the general public; (3) establish better relationships with other professionals with common interests; (4) conduct more efficacy and effectiveness research; and (5) create a demand for our services so that the public will be more willing to pay for our services “out of their own pocket.” In order for this to happen, we must stop fighting with each other and direct our energies to productive activities that can change fantasies into realities.

    Abstract:

    Ethical codes and principles and laws govern the behavior of health-care professionals. Yet, the impact that ethical codes and laws have on the actual moral behaviors of health-care professionals is relatively unknown. A survey on the ethical beliefs and practices of health-care professionals was sent to the United States membership of the Association for Applied Psychophysiology and Biofeedback. AAPB's heterogeneous membership offers a unique opportunity to compare the ethical beliefs and behaviors of professionals across various health-care professions, educational levels, licensure and certification statuses, age, years in practice, and gender. The survey examined the respondents' ethical beliefs and practices in the domains of confidentiality, dual relationships, and professional practice. Five hundred thirty-six surveys were returned completed. The results of the survey indicate that all respondents have substantially the same reported ethical beliefs and practices across the three domains. There were no statistically significant differences between the reported ethical beliefs or practices when compared across disciplines, educational levels, licensure or certification statuses, age, or years in practice. Statistically significant gender differences were found.

    Abstract:

    Recognition of the growing interest in Quantitative EEG led me to extend an invitation to the Neurofeedback Division of the Association for Applied Psychophysiology and Biofeedback to assemble a series of papers focusing on this topic. Jonathan Walker accepted this invitation and solicited the contributions included in this issue, which range from a controlled trial to collections of clinical cases, as well as methodological commentary. It is my hope that this brief series will stimulate further interest in this topic, leading to more rigorous tests of the preliminary notions advanced herein. Around the same time the above-mentioned papers were being assembled, the journal began to receive a number of papers addressing Autism Spectrum Disorders, also known as Pervasive Developmental Disorders. As the later name implies, these conditions can cause severe and pervasive impairments in cognition, affect, language, and ability to relate to one another. To date, no single treatment or treatment package is universally accepted. Growing recognition of the importance of research in this area led the National Institutes of Health recently to award more than 50 grants on this topic, totaling more than $65 million dollars, supported by the American Recovery and Reinvestment Act (Recovery Act) fund. Three papers addressing Autism Spectrum Disorders are included in this issue—2 comprehensive reviews and 1 large-scale clinical replication series. All include a major focus on EEG assessment and treatment. Although the review papers address similar topics, they were found different enough to warrant each being published. It is my further hope that these papers, and others soon to follow, will encourage others to become involved in this important, but clearly understudied, area of research.

  • Biofeedback Via Telehealth: A New Frontier for Applied Psychophysiology

    Abstract:

    Psychophysiological diagnostic and therapeutic methods, to include biofeedback, have been found to be empirically effective in the treatment of a variety of physical disorders. In many areas of the country, however, certified biofeedback practitioners are not well distributed, limiting patient access to this treatment modality. Psychologists at Tripler Army Medical Center have pioneered efforts to develop and provide these needed services via a telehealth venue. Such capability significantly improves access to care, particularly for patient's located at considerable distance from the provider. As the telecommunications infrastructure in rural and remote areas is often quite basic, such a system must be capable of operating within these limited parameters. The system developed by the authors provides real-time video and audio interactivity and allows the therapist to monitor and control biofeedback equipment located at the remote site. The authors discuss the clinical applications, the technology, and relevant practical and ethical issues.

  • Aging and self-regulation: An introduction and overview

    Abstract:

    Changes in population trends and successes at treating infectious and chronic health conditions have resulted in older individuals representing an increasing percentage of the population. Unfortunately, this steady growth in the elderly population has not been met by a parallel increased focus on research. (It is interesting that a similar situation exists at the other end of the age spectrum -- children.) At the 1989 annual conference of the Association for Applied Psychophysiology and Biofeedback, largely at the instigation of then President Susan Middaugh, several invited presentations were arranged in an effort to highlight certain aspects of aging, point out some of the newer ongoing Applied psychophysiological and biofeedback research in this area, and encourage others to become more actively involved in study of aging. Several presentations from this meeting have been expanded and included in the fop lowing brief special series. In the first article, presented as a keynote address at the meeting, T. Franklin Williams, Director, National Institute on Aging, lays the groundwork for the series. Williams identifies and then convincingly debunks the many myths that have characterized views toward aging (such as the erroneous conclusions of inevitable physical, mental, and sexual failings) and which likely have accounted in large measure for the lack of significant research attention previously given this age group. Williams makes an important distinction between intrinsic and extrinsic, or lifestyle, factors, the latter of which are clearly open to prevention and modification and are relatively unexamined. The potential for new investigative approaches and clinical applications is clearly pointed out. In fact, Williams' paper can be viewed as challenging us to become more active in prevention and treatment of the major problems causing mortality and morbidity in the older segments of our population.

    This article presents a case study of a 39-year-old European American married woman with a history of child and adolescent incest,marital rape, and physical abuse from her husband for more than 10 years. She was referred to a pain clinic for treatment of headaches and Tourette's syndrome. The client was evaluated with the Ackerman-Banks Neuropsychological Rehabilitation Battery to identify neuropsychological strengths and weaknesses. The Vulnerability to Stress Audit was used to identify life events that were positively and negatively influencing her life. The client was treated for mild traumatic brain injury, post-traumatic stress disorder,cognitive difficulties, impulsivity, confabulation, low frustration tolerance, and inability to evaluate and make decisions about socially appropriate behaviors. Treatment involved traditional psychotherapy, hypnosis, cognitive rehabilitation, biofeedback training, electromyography, finger temperature, and blood pressure.

    A remarkable feature of the human brain is its sexual dimorphism. While it is well documented that the sexual dimorphism in brain structure and function exists, its clinical implications in healthy individuals as well as in those who suffer from various neuropsychiatric disorders remain to be further explored. The present paper aims to provide an overview of the remarkable features and the fundamental characteristics of the sexual dimorphism in brain performance along with clinical implications based on the review of the relevant meta-analyses published up-to-date. The primary aim is to highlight and discuss the synthesized results to advance human knowledge in the area of Applied Psychophysiology and support direct future interdisciplinary research efforts towards improvements in health and quality of life with regard to sexual dimorphism in brain structure–function interrelationships. The review also seeks to advance clinical management approaches to sexually dimorphic neurocognitive conditions. Better understanding of the areas implicated in sex-biased neuropsychiatric disorders can help to improve sex-specific referrals, diagnosis and proactive care management of the patients and achieving treat-to-target goals.
