    Abstract:

    Objectives:Study Auditory Tube dysfunction that occurs after jaw surgery.Methods:Two hundred sixty-eight patients were enrolled (60 males and 208 females; mean age, 24.3 years old) who were diagnosed with jaw anamorphosis symptoms and underwent orthognathic surgery of the lower jaw, and who were tested for Auditory Tube function before and after surgery at our hospitals between December 2000 and February 2012. Auditory Tube function test pre- and post-operation were conducted, and post-operatively we searched for Auditory Tube dysfunction. Auditory Tube dysfunction in a high rate was seen after lower jaw orthognathic surgery.Results:Post-operative Auditory Tube dysfunction was noted subjectively in 49 patients (18.3%) and determined by TM in 162 (60.4%). There is also a relationship between pre-operative SON and TTAG and post-operative TM aggravation.Conclusions:Post-operative Auditory Tube dysfunction correlated with pre-operative TTAG values, indicating that TTAG was a useful test for the prediction of ...

    Abstract:

    Objective: Some patients with jaw anamorphosis symptom who underwent plastic surgery for lower jaw complain of earache, feeling of obstruction, and hearing disorders.By conducting Auditory Tube function tests before and after plastic surgery for the lower jaw, we studied pathophysiology, incidence, causes and treatment of Auditory Tube dysfunction.Method: This study was carried out between 2000 and 2010. The 290 patients were divided into 4 groups(A to D) and compared for the rate of aggravation of Auditory Tube function and duration for the function to recover.Results: Approximately 30% of patients with TM aggravation complained of ear symptoms. Among subjective ear symptoms, the most prevalent was feeling of ear obstruction, followed, in the order, by discuses. The number of patients who complained of ear symptoms was 23.9% in group A, 17.1% in group B, 6.7% in group C, and 9.1% in group D.     Intended for the 268 cases which did an orthognathic surgery for lower jaw with diacrisis of jaw anamorphosis symptom object. Executed Auditory Tube function test in preand post-operation, and searched Auditory Tube dysfunction of postoperative. Recognized Auditory Tube dysfunction in a high rate after lower jaw orthognathic surgery. The case that was conscious of a symptom by Auditory Tube functional disorder after art was 49 examples (18.3%), and recognized Auditory Tube dysfunction in Auditory Tube examination was 162 cases (60.4%).     To clarify the functions of the equine guttural pouch, the distribution of various immunoglobulin isotypes and subisotypes in the guttural pouch mucosa were examined in healthy horses. IgGa was present in the mucosa of guttural pouch, mucosal lymph nodules and submucosal lymph nodules. IgM was scattered in the mucosal lymph nodules and in the germinal centers of the submucosal lymph nodules. IgGc was recognized only in the submucosal lymph nodules. These immunoglobulin isotypes and subisotypes were found in lymphocytes and plasma cells. On the other hand, IgA was detected in glandular epithelial cells and the surface layer of the mucosal epithelium, as well as in free cells. This finding suggests that IgA is secreted through the glandular epithelium. Based on the above findings, we conclude that the guttural pouch has phylactic ability.

    The present study attempted to clarify the characteristics of the guttural pouch mucosa in equines and to evaluate its foreign substance clearance ability. The specimens were collected from nine regions (eight in the guttural pouch mucosa, and one in the nasopharynx mucosa). We first examined the pouch mucosa by light and electron microscopy. We then measured the frequency of goblet cells per 200 epithelial cells, the length of the cilia, the thickness of the epithelial cell layer and lamina propria and statistically analyzed the data. The guttural pouch mucosa consisted of stratified columnar epithelia with brush-like cilia, and there were almost no histological differences between adults and foals. The morphometrical study revealed significant differences in goblet cell frequency (p < 0.001) and the thickness of lamina propria (p < 0.05). By contrast, no statistically significant difference was detected in the length of the cilia or the thickness of the epithelial cell layer. These findings suggest that the guttural pouch mucosa provides foreign substance clearance ability, but that its ability varies among different regions of the epithelium.

    The characteristics of the equine Auditory Tube diverticulum (guttural pouches) were studied in detail by the naked eye and silicon mold method. First, we examined the anatomical relationship between the guttural pouches and their associated bones and muscles. Secondly, a silicon mold was fabricated to clarify the three-dimensional aspect of the guttural pouches, paying a special attention to the distribution of major arteries and nerves surrounding it. Thirdly, the volume of the silicon mold was measured by immersing it in water. The guttural pouches are a pair of pouches located dorso-posteriorly to the posterior pharynx. The pouches had a close contact rostrally with the Os sphenoidale, ventrally with the pharynx and the esophagus, and caudally with the Articulatio atlantoaxialis. The left and right guttural pouches had almost the same capacity in each horse. The A. carotis interna, cranial ganglia of cervical nerves and cervical Truncus sympathicus, as well as the N. vagus, N. glossopharyngeus, N. hypoglossus, and Radices spinales of the N. accessorius, were present in the mucosal crease extending from the roof of the guttural pouches towards the middle of the caudal wall. The N. facialis appeared from the Foramen stylomastoideum ran dorsocaudally along the lateral recess. The N. mandibularis appeared from the Foramen lacerum went over the muscular process of an ear bone, ran along the roof of the lateral recess and finally continued with the rostral side of the guttural pouches. Lymph nodes, scattered on the mucosal surface, appeared as tiny indentations with approximately millet seed size on the silicon surface. The capacity of guttural pouches in adult horses (472 +/- 12.4 cm3) was three-fold larger than that in foals (145 +/- 9.4 cm3).

    The domestic horse (Equus caballus) have the large symmetrical guttural pouches (the Auditory Tube diverticulum) formed by saccate bulge of the Auditory Tube. In this study, CT examination was carried out in the head of Przewalskirs horse (Equus przewalskii), the only true wild horse living at present. As results of the examination, Przewalskirs horse possessed the large symmetrical guttural pouches divided into medial and lateral compartments by the stylohyoid bone. Moreover, the right and left guttural pouches meet each other at the median part to form a thin septum. As CT sections get close to the part of the occipital condyle, the lateral compartment disappeared, and the medial compartment gradually became small toward the base of the skull. These results indicate that the nuchal-basal part of the medial compartment is not well-developed as compared with the domestic horse.

    Case Description—2 horses and 1 pony were evaluated for right-sided (1 horse and the pony) and left-sided (1 horse) acute epistaxis of 1 day's to 1 month's duration. Clinical Findings—Endoscopic examination of the 3 equids revealed that the hemorrhage originated from the right maxillary artery in 2 equids and from the left internal carotid artery in the third. Mycosis of the Auditory Tube diverticulum (guttural pouch) was detected in all 3 equids. Treatment and Outcome—All 3 equids underwent surgery, and transarterial nitinol intravascular plugs were placed to occlude affected blood vessels. All equids survived for a long period (ie, a minimum of 1 to 2 years) and returned to their previous use. All had complete regression of clinical signs of guttural pouch mycosis without additional medical treatment.     Two horses with chronic empyema of the Auditory Tube diverticulum (guttural pouch) were refractory to medical treatment; empyema was bilateral in 1 horse and unilateral in the other. Both horses were treated by fistulation of the cartilage of the pharyngeal orifice by use of a neodymium:yttrium-aluminum-garnet laser in a noncontact manner. To maintain patency of the fistulae, indwelling catheters were placed into the openings created by the laser. For both horses, long-term follow-up did not reveal complications, and both owners were satisfied with results of the procedure. The authors recommend this procedure for horses that are unresponsive to medical treatment for chronic guttural pouch empyema.
