    RESUMEN La infeccion por Blastocystis spp., conocida como blastocistosis, es en la actualidad la parasitosis intestinal mas prevalente en Venezuela y quiza en el mundo. Los ninos resultan mas afectados y, aunque se ha senalado un amplio espectro de manifestaciones clinicas, la mayoria son asintomaticos. Se realizo un estudio de tipo transversal en ninos de la comunidad indigena Itopoicon, del estado Bolivar, Venezuela, para determinar la prevalencia de infeccion por Blastocystis spp. y conocer las principales manifestaciones clinicas. Una muestra fecal de cada individuo fue analizada mediante las tecnicas de examen directo, Kato, Willis y sedimentacion espontanea. A cada participante se le lleno una ficha clinico-epidemiologica y fue sometido a examen clinico y una anamnesis orientada. Se evaluaron 118 ninos, 69 hembras y 49 varones desde 0,1 hasta 14 anos (media 6,31 ± 3,58 anos); determinandose una prevalencia de parasitos intestinales de 84,7%. Blastocystis spp. fue el parasito mas comun con 67,8%. Solo 14 de los 80 casos tenian infeccion unica por Blastocystis spp., estando 4 (28,6%) asintomaticos. Entre los sintomaticos lo mas frecuente fue diarrea, bruxismo, vomitos, nauseas, perdida de peso e hiporexia. Las manifestaciones clinicas encontradas en los otros 66 ninos y adolescentes no pueden ser atribuidas unicamente a este parasito, debido al elevado porcentaje de poliparasitismo. No hubo relacion entre la carga parasitaria y las manifestaciones clinicas. En conclusion, se determino una elevada prevalencia de infeccion por Blastocystis spp. (67,8%) la cual, unida a otras parasitosis intestinales, se explican por las deficientes condiciones sociosanitarias observadas en la comunidad evaluada. ABSTRACT Blastocystis spp. infection, known as Blastocystosis, is currently the most prevalent intestinal parasitosis in Venezuela and perhaps in the world. Children are most affected and although there is a broad spectrum of clinical manifestations, most are asymptomatic. A cross-sectional study was conducted in children from Itopoicon, an indigenous community in Bolivar State, Venezuela, to determine the prevalence of infection by Blastocystsis spp. and to know the main clinical manifestations. Each parent or guardian signed the informed consent. A stool sample from each individual was analyzed by direct examination, Kato, Willis and spontaneous sedimentation techniques. A clinical epidemiological record was filled for each participant and underwent clinical examination and oriented anamnesis. A group of 118 children, 69 females and 49 males of 0.1 years to 14 years old (6.31 ± 3.58 years) was evaluated. A prevalence of intestinal parasites of 84.7% was detected. Blastocystis spp. was the most common parasite with a prevalence of 67.8%. Only 14 of the 80 cases had single infection by Blastocystis spp. Of these, four were asymptomatic (28.6%). Among symptomatic children, the most frequent manifestations were diarrhea, bruxism, vomiting, nausea, weight loss and hyporexia. Clinical features found in the other 66 children cannot be attributed solely to this parasite, due to the high percentage of polyparasitism. There was no relationship between parasite load and clinical manifestations. In conclusion, a high prevalence of infection by Blastocystis spp. was determined which, together with other intestinal parasites, could be explained by the poor sanitary conditions observed in the community evaluated.     Abstract:

    To evaluate the prevalence of Blastocystis hominis and its clinical relevance, 169 preschool children from the "Los Coquitos" nursery school living in Bolivar City, Venezuela, were studied. Stool samples were obtained and examined by direct microscopic examination, and the Faust and Willis concentration techniques. Some 72 of the children had intestinal parasites, of whom 32 (29.09%) had B. hominis. Prevalence for the latter was 18.93% ±5.93%. No differences were observed by sex or age (c2 = 1.84 DF = 3; p>0.05). In the majority (53.13%) of the children, B. hominis was the only parasite. Giardia lamblia was the parasite most frequently identified with B. hominis (39.13%). In 1994, in 12% of the cases more than five microorganisms per microscopic field were observed. Clinical manifestations were observed in 70.58% of the preschool children. Presence of parasites was not correlated with symptomatology, but only with severity. Proper clinical and parasitological response to treatment was observed in 80% and 90% of patients, respectively. The conclusion was that B. hominis is a relatively frequent intestinal parasite among the preschool children evaluated.

To determine the prevalence of infection by Blastocystis spp. and other intestinal parasites in children from a periurban school. Design. Observational and transversal study. Location. A school located in the district of Yura in Arequipa, Peru. Participants. 83 children between 4 and 8 years old. Interventions. Stool samples obtained from the children were analyzed with the modified Telemann method; a descriptive analysis was done by calculating frequency. Main outcome measures. Presence of the intestinal parasite. Results. The prevalence of Blastocystis spp. was 81.9% (68/83), the overall prevalence of parasites was 96.4% (80/83), and 80.7% (67/83) had more than ne type of parasite. Conclusions. There was a high prevalence of Blastocystis spp. and other intestinal parasites in the sample studied. There was also a high polyparasitism in the school population studied. It is associated with frequent and unquestionably very important controversy and lack of definition, above all due to its implications for general assistance and medical care. In that connection, there is considerable disagreement on the subject of the pathogenicity of this protozoan, which should be categorically defined. Other aspects besides the above, require clarification through results from well conducted studies aiming at attributing Blastocystis hominis a proper role within the context of public health. Another matter worthy of attention is the diagnostic value of the parasitological stool examination, with the proviso that it is adequate, as are fecal smears suspended in saline solution or permanent mounts stained with iron hematoxylin or thionine. The use of inadequate techniques tends to produce false negative results, thereby impeding investigation into the real importance of this microorganism.

Está relacionada com várias controvérsias e indefinições, sem dúvida muito importantes sobretudo pelas implicações médico-assistenciais que suscitam. A propósito, pendência expressiva diz respeito à patogenicidade do referido protozoário, que precisa ser categoricamente definida. Ao lado dessa particularidade outros assuntos exigem elucidações através de pesquisas bem conduzidas, para que a blastocistose fique devidamente situada no contexto da saúde pública. Aspecto também digno de atenção é o diagnóstico pelo exame parasitológico de fezes, necessariamente executável por meio de métodos adequados, como o direto e os permanentes, exemplificados pelos que usam a a hematoxilina férrica ou a tionina. A propósito, pendência expressiva diz respeito à patogenicidade do referido protozoário, que precisa ser categoricamente definida. Ao lado dessa particularidade outros assuntos exigem elucidações através de pesquisas bem conduzidas, para que a blastocistose fique devidamente situada no contexto da saúde pública. Aspecto também digno de atenção é o diagnóstico pelo exame parasitológico de fezes, necessariamente executável por meio de métodos adequados, como o direto e os permanentes, exemplificados pelos que usam a a hematoxilina férrica ou a tionina.
