    Abstract:

    Background: Consumer Health information technology can improve patient engagement in their Health care and assist in navigating the complexities of Health care delivery. However, the Consumer Health information technology offerings of Health systems are often driven by provider rather than patient perspectives and inadequately address patient needs, thus limiting their adoption by patients. Consideration given to patients as stakeholders in the development of such technologies may improve adoption, efficacy, and Consumer Health information technology resource allocation. Objective: The aims of this paper were to measure patient interest in different Health system Consumer Health information technology apps and determine the influence of patient characteristics on Consumer Health information technology interest. Methods: Patients seen at the Cleveland Clinic Neurological Institute were electronically surveyed on their interest in using different Consumer Health information technology apps. A self-efficacy scale, Patient Health Questionnaire-9 depression screen, and EuroQol 5 dimensions Health-related quality of life scale were also completed by patients. Logistic regression was used to determine the influence of patient characteristics on interest in Consumer Health information technology in the categories of self-management, education, and communication. Results: The majority of 3852 patient respondents had an interest in all technology categories assessed in the survey. The highest interest was in apps that allow patients to ask questions of providers (3476/3852, 90.24%) and to schedule appointments (3211/3839, 83.64%). Patient interest in Consumer Health information technology was significantly associated with greater depression symptoms, worse quality of life, greater Health self-efficacy, and smartphone ownership (P<.001 for all listed). Conclusions: Patients should be viewed as active stakeholders in Consumer Health information technology development and their perspectives should consistently guide development efforts. Health systems should consider focusing on Consumer Health information technologies that assist patients in scheduling appointments and asking questions of providers. Patients with depression should also be considered for targeted Consumer Health information technology implementation. Health self-efficacy is a valid predictor of Consumer Health information technology interest and may play a role in the utilization of Consumer Health information technologies. Health systems, broadly, should put forth greater effort to understand the needs and interests of patients in the Consumer Health information technology development process. Consumer Health information technology design and implementation may be improved by understanding which technologies patients want. [J Med Internet Res 2018;20(4):e128]

    User-centered design (UCD) is well recognized as an effective human factor engineering strategy for designing ease of use in the total customer experience with products and information technology that has been applied specifically to Health care information technology systems. We conducted a literature review to analyze the current research regarding the use of UCD methods and principles to support the development or evaluation of diabetes-related Consumer Health informatics technology (CHIT) initiatives. Findings indicate that (1) UCD activities have been applied across the technology development life cycle stages, (2) there are benefits to incorporating UCD to better inform CHIT development in this area, and (3) the degree of adoption of the UCD process is quite uneven across diabetes CHIT studies. In addition, few to no studies report on methods used across all phases of the life cycle with process detail. To address that void, the Appendix provides an illustrative case study example of UCD techniques across development stages.

    Health Information Consumers are increasingly seeking access to timely, accurate and accessible information, as made possible through Information Technology (IT). These technological developments have direct implications for access, awareness and use of Health information by Consumers, as well as patient decision making, safety, and quality of Health care provision. Healthcare information systems are expected to reduce medical errors, improve quality of patient care and safety. These systems are increasingly supporting evidence-based medicine and patientcentric technologies, including monitoring of patient outcomes and adverse events, as well as better informing and empowering Consumers themselves to work for better outcomes. This mini-track embraces all aspects of Consumer Health informatics and Consumer-centric technologies or studies aimed at improving patient safety and quality of care, including: supporting Consumers taking an active role in understanding, deciding about and/or managing their Health; doctor-patient communication; clinical guideline and protocol support; monitoring and prevention of adverse events; and electronic Health records (especially, security and privacy, access control rights, and Consumer ability to make entries into the Health record, including home monitoring). The Consumer Health Informatics, Patient Safety and Quality of Practice Minitrack uses the international and interdisciplinary forum provided by HICSS for the expression of practical, theoretical, academic and industrial insight in this area.

    Abstract:

    Objectives: To present the breadth and depth of Consumer Health Informatics (CHI) initiatives and in particular, identify areas to further enhance such solutions with the incorporation of advances in artificial intelligence (AI). Methods: A literature review was conducted. Inclusion criteria focused on barriers and facilitators as well as key milestones for CHI over the last 10 years. In addition, significant international initiatives in regions and /or countries and opportunities to incorporate the advances of AI also made up the inclusion criteria. Exclusion criteria included factors that made reach/range of solutions to individuals or groups of individuals less than 50 people. Journals in Medline were accessed as well as the grey literature. Results: 200 papers were identified but this was narrowed to 25 due to repetitions and similarities of the types of solutions presented. From this, it was possible to identify key barriers and facilitators including: socio-political factors, financial factors, infrastructure, as well as levels of Health literacy. Conclusions: The relatively nascent domain of CHI has developed and is growing. Full benefits will only be realized when more precision solutions that incorporate AI advances will be designed. The example of RxPredict is provided as a case vignette to illustrate.

    Diabetes-related Consumer Health information technology (CHIT) has been designed to facilitate self-management practices, and its use has improved Health outcomes for many Consumers. This analysis sought to identify tendencies in diabetes-related CHIT research from 2010-2015 to help researchers find novel research topics, periodicals, collaborators, and funding agencies and experts and lay Consumers to find scholarly information. Six search engines encompassing computer science, engineering, and medicine yielded potential diabetes-related CHIT publications. Abstracts and full texts were screened based on inclusion and exclusion criteria. Information on year, periodical, periodical domain, keywords, author location, author institutions, authors, and funding agencies were collected from included publications. The screening process yielded 1551 publications. Studies were published in a core of twenty periodicals, commonly comprising medicine or technology domains. "Telemedicine" was the most frequently used keyword. Harvard University, Dr. Eirik Årsand, and the National Institute of Diabetes and Digestive Kidney Diseases were the most frequent author institution, author, and funding agency, respectively, associated with publications. This analysis revealed potential for novel research on the sociology and economics of diabetes-related CHIT, among other topics. A lack of collaboration between top authors in the field indicates potential for new, impactful collaborations. Ongoing bibliometric research will be necessary to assess changes in this field. The opportunity exists to inform lay Consumers and researchers through bibliometric analyses of other Consumer Health informatics topics.

    ABSTRACTA basic premise of macroergonomic theory is that better physical and psychological outcomes and reduced unintended consequences are achieved when there is alignment between a technology and the user’s work system. The user’s work system is defined as the social subsystem, technical subsystem, and the external environment within which work is performed. Cultural context has been conceptualized as part of the external environment. Thus, from a theoretical viewpoint, creating technology that is aligned with users’ cultural contexts will result in better performance outcomes. The need to align with cultural context is particularly important for technologies such as Consumer Health information technology, which are intended for use within the naturalistic spaces of patients’ homes and communities.     Designing patient-centered Consumer Health informatics (CHI) applications requires understanding and creating alignment with patients’ and their family members’ Health-related activities, referred to here as ‘patient work’. A patient work approach to CHI draws on medical social science and human factors engineering models and simultaneously attends to patients, their family members, activities, and context. A patient work approach extends existing approaches to CHI design that are responsive to patients’ biomedical realities and personal skills and behaviors. It focuses on the embeddedness of patients’ Health management in larger processes and contexts and prioritizes patients’ perspectives on illness management. Future research is required to advance (1) theories of patient work, (2) methods for assessing patient work, and (3) techniques for translating knowledge of patient work into CHI application design. Advancing a patient work approach within CHI is integral to developing and deploying Consumer-facing technologies that are integrated with patients’ everyday lives.

    This paper presents an idealized conceptual framework for Consumer Health informatics research drawing from complementary disciplines: information science and Health campaign research. This synthesis is designed to provide researchers with a flexible model to evaluate current research and inform future studies. Following a description of the major components, we describe a recent evaluation of Consumer perceptions of a Health information system, Genetics Health Reference. This study illustrates how the framework may be applied to provide some direction and insights into ongoing Consumer Health informatics research. While this model represents a work in progress, we present it in support of efforts to understand the multidimensional impacts of the public's access to Health information. We also discuss challenges that remain to develop a better conceptual understanding of how Consumers converge on Health informatics services.

    Laypersons (''Consumers'') often have difficulty finding, understanding, and acting on Health in- formation due to gaps in their domain knowledge. Ideally, Consumer Health vocabularies (CHVs) would reflect the different ways Consumers express and think about Health topics, helping to bridge this vocabulary gap. However, despite the recent research on mismatches between Consumer and professional language (e.g., lexical, semantic, and explanatory), there have been few systematic efforts to develop and evaluate CHVs. This paper presents the point of view that CHV development is practical and necessary for extending research on informatics-based tools to facilitate Consumer Health information seeking, retrieval, and understanding. In support of the view, we briefly describe a dis- tributed, bottom-up approach for (1) exploring the relationship between common Consumer Health expressions and professional concepts and (2) developing an open-access, preliminary (draft) ''first-generation'' CHV.     Abstract:

    Consumer Health informatics (CHI) is a rapidly evolving sub-discipline of medical informatics. Such developing fields typically share common needs, such as harmonizing terms and building a common foundation of research methods and instruments. The authors describe a pilot study to conceptualize and develop a “CHI toolbox,” a repository of existing methods and instruments across relevant established fields. The challenges encountered in attempting to organize concepts in a nascent, interdisciplinary field are discussed. The authors’ experiences in creating a comprehensive CHI toolbox suggest that a larger, concerted effort to develop a similar product by members of the relevant research communities could accelerate the development of common terms, operational definitions, variables, and instruments within the CHI field.
