Validation of a Spanish version of the Revised Fibromyalgia Impact Questionnaire (FIQR).

CalandreAbstract:Background: The Revised version of the Fibromyalgia Impact Questionnaire (FIQR) was published in 2009. The aim of this study was to prepare a Spanish version, and to assess its psychometric properties in a sample of patients with Fibromyalgia. Methods: The FIQR was translated into Spanish and administered, along with the FIQ, the Hospital Anxiety Depression Scale (HADS), the 36-Item Short-Form Health Survey (SF-36), and the Brief Pain Inventory (BPI), to 113 Spanish Fibromyalgia patients. The administration of the Spanish FIQR was repeated a week later. Results: The Spanish FIQR had high internal consistency (Cronbach’s α was 0.91 and 0.95 at visits 1 and 2 respectively). The test-retest reliability was good for the FIQR total score and its function and symptoms domains (intraclass correlation coefficient (ICC > 0.70), but modest for the overall Impact domain (ICC = 0.51). Statistically significant correlations (p < 0.05) were also found between the FIQR and the FIQ scores, as well as between the FIQR scores and the remaining scales’ scores. Conclusions: The Spanish version of the FIQR has a good internal consistency and our findings support its validity for assessing Fibromyalgia patients. It might be a valid instrument to apply in clinical and investigational grounds.

Dimensionality, Reliability, and Validity of the Revised Fibromyalgia Impact Questionnaire in Two Spanish Samples

A second objective was to report on the reliability and construct validity of the FIQR in Spanish patients. Methods FIQR data from a sample of adult Fibromyalgia patients (n = 113) were analyzed using principal components analysis (PCA). Subsequently, a set of confirmatory factor analyses (CFAs) was conducted in another sample (n = 179) to analyze the goodness of fit of various factor models. FIQR reliability was assessed by computing Cronbach's alpha and coefficient H. Construct validity was evaluated by comparing the FIQR scores of participants categorized by employment status. Results According to the PCA, the FIQR structure might be described as having 1 global factor of functional impairment. Although subsequent CFAs confirmed that 1 factor accounted for the greatest proportion of common variance in the FIQR items, a confirmatory bifactor analysis indicated that the items were multidimensional because of their simultaneous significant loading on specific factors. The Cronbach's alpha values of the FIQR domains were very good (>0.80) and the H estimate for the FIQR total score was excellent (0.93). Overall, the FIQR domains were able to distinguish between patients differing in employment status (working outside the home versus on sick leave). Conclusion Our results indicate that the Spanish version of the FIQR has a complex factor structure, has excellent reliability, and shows good construct validity.

Subgrouping patients with Fibromyalgia according to the results of the Fibromyalgia Impact Questionnaire: a replication study

The purpose of the present study has been to replicate a subgroup classification proposed by de Souza et al. based in the Fibromyalgia Impact Questionnaire (FIQ) in a large sample of patients with a cultural and clinical setting different from the original one. Four hundred twenty-one patients were classified, according to the results of the visual analog FIQ subscales in type I o type II subgroups. Demographic and clinical data, as well as results of scales assessing disease’s severity, quality of life, sleep quality, anxiety and depression, were compared between the two groups. The profiles of type I and type II patients from our sample strikingly paralleled those of the original study, demonstrating the reproducibility of the classification. In our sample, 18.8% of the patients appertained to type I subgroup and 81.2% to type II subgroup. Patients from this later subgroup had higher comorbidity and received more drugs than those of the former. They were also more physically ill, with higher FIQ total scores and worse sleep quality, and more psychologically distressed, with higher levels of anxiety and depression and lower scores in the mental component summary of the Short-Form Health Questionnaire (SF-12). Our study shows that the proposed Fibromyalgia classification is reliable and easy to perform and could be applied in further studies.

A Brazilian Portuguese version of the Revised Fibromyalgia Impact Questionnaire (FIQR): a validation study.

BennettAbstract:The Fibromyalgia Impact Questionnaire (FIQ) was specifically developed to assess disease severity and functional ability in Fibromyalgia patients. In 2009, a revised version of the FIQ was published, the FIQR; this version achieved a better balance among different domains (function, overall Impact, symptoms). Here, we present the validity and reliability of the Brazilian version of the Revised Fibromyalgia Impact Questionnaire (FIQR). Female Fibromyalgia patients (n = 106) completed an online survey consisting of the Short Form 36 (SF-36) Questionnaire, the original FIQ, and the Brazilian Portuguese FIQR, which was translated by a standard method. Validity was established with correlational analyses between the FIQR, FIQ, and SF-36 items. Three domains were established for the FIQR (function, overall Impact, symptoms), and their contribution for the SF-36 subscales was also scrutinized. The Brazilian FIQR validation process showed that the questions performed in a very similar way to the original English FIQR. The new questions in the FIQR symptoms domain (memory, balance, tenderness, and environmental sensitivity) revealed a significant Impact in Fibromyalgia (FM) patients. The Brazilian Portuguese FIQR demonstrated excellent reliability, with a Cronbach’s alpha of 0.96. There was a gain on weight of the function domain and a decrease of the symptom domain, leading to a better balance among domains. The FIQR predicted a great number of SF-36 subscales, showing good convergent validity. The Brazilian Portuguese version of the FIQR was validated and found to be a reliable, easy-to-use, and score FM-specific Questionnaire that should prove useful in routine clinical practice and FM-related research.

Design After trial follow-up survey of subjects who had completed a randomized, controlled, double-blind study of noninvasive cortical stimulation therapy some 45 months previously. Setting General community. Participants Patients with FM (N=69) who participated in the previous study, 39 of whom were mailed surveys. Interventions Not applicable. Main Outcome Measure Changes in the Fibromyalgia Impact Questionnaire. Results There was a 64% survey return rate. The total Fibromyalgia Impact Questionnaire score was 52.6 at baseline, 35.7 at end of study, and 31.8 at follow-up ( P Conclusions A high percentage of patients with FM treated with noninvasive cortical stimulation continued to experience worthwhile improvement at follow-up.

Relationships between the Fibromyalgia Impact Questionnaire, tender point count, and muscle strength in female patients with Fibromyalgia: a cohort study.

Methods A total of 840 FM patients and 122 healthy subjects were evaluated between 1998 and 2005. All of the patients completed version 1 of the Fibromyalgia Impact Questionnaire (FIQ) and were assessed for tender points and knee muscle strength. All subjects underwent bilateral isokinetic knee muscle strength testing in flexion and extension. Normative knee muscle strength values were calculated from the healthy subjects, and the FM cohort was divided in 2 groups: 1) patients with normal muscle strength and 2) patients with low muscle strength (2 SDs below normal). The clinical characteristics of these 2 groups were compared. Results Significantly reduced knee muscle strength was found in 52% of the patients. There were no clinically significant differences between patients with low versus normal muscle strength. There were no clinically significant correlations between total FIQ score, tender point count, and muscle strength. Only 4.6% of the FIQ scores and 5.1% of the tender point counts were explained by muscle strength. Conclusion Significantly reduced knee muscle strength was found in more than half of the patients. Patients with subnormal muscle strength were not more symptomatic or tender than patients with normal muscle strength. There were no clinically significant correlations between FIQ, tender point count, and muscle strength; therefore, reduced knee muscle strength appears to be a common objective abnormality in FM that is independent of measurements of disease activity. The implication of this finding in regard to the clinical assessment of FM needs further study.

The objective was to develop and analyse the psychometric properties of a new composite disease-specific index (Fibromyalgia Assessment Status, FAS), a simple self-administered index that combines a patient's assessment of fatigue, sleep disturbances and pain evaluated on the basis of the 16 non-articular sites listed on the Self-Assessment Pain Scale (SAPS) in a single measure (range 0 to 10).
