Fire-Setting is a type of crime that is associated with psychotic disorders.Aim: The aim of this prospective follow-up study was to investigate if Fire-Setting performed in adolescence or early adulthood was associated with future diagnoses of schizophrenia or schizoaffective disorder.Methods: The consecutive sample consisted of 111 Finnish 15–25-year old males with Fire-Setting crimes, decreed to a pre-trial forensic psychiatric examination in 1973–1998, and showing no past nor current psychosis at the time of examination. For each Firesetter, four age-, gender-, and place of birth-matched controls were randomly selected from the Central Population Register. Psychopathic traits among a consecutive sample of Finnish pretrial Fire-Setting offenders

Fire-Setting is a major source of property damage, injury, and death in many Western countries. The primary aim of this study was to evaluate psychopathic traits in a consecutive sample of Finnish male pretrial Fire-Setting offenders. Further, we wanted to investigate whether Fire-Setting recidivists show higher traits of psychopathy than one-time Firesetters and whether exclusive Firesetters show lower traits of psychopathy than those with criminal versatility.

The medical records as well as the forensic psychiatric examination statements of 90 arson recidivists referred for pretrial psychiatric assessment in Helsinki University Hospital Department of Forensic Psychiatry between 1973 and 1993 were reviewed. The most important diagnostic categories of arson recidivists were personality disorders, psychosis and mental retardation, often with comorbid alcoholism. In all, 68% of arsonists were under alcohol intoxication during the index crime. Psychotic as well as mentally retarded persons with repeated Fire-Setting behaviour were mostly "pure arsonists"- persons guilty only of arsons during their criminal careers. Arson recidivists with personality disorder, in contrast, often exhibited various types of criminal behaviour and arson appeared to be only one expression of a wide range of criminal activity. Comorbid alcoholism was apparently a more rarely observed phenomenon among pure arsonists than in "nonpure arsonists". We found only three subjects fulfilling the present diagnostic criteria for pyromania. Using the criteria of the DSM-IV-TR, pyromania must be regarded as an extremely rare phenomenon. Especially the question of substance intoxication as an exclusion criterion for pyromania should be reconsidered.

Assessment and treatment options have principally been tested in clinically referred or incarcerated children and adolescents. Little information is available about Fire Setting in normal populations and thus opportunities for screening and early intervention are not well developed. In this study, a large sample of 4- to 9-year-old children were assessed using a seven-item Fire interest history screen with other measures of antisocial behaviour, children's mental health, parenting style and quality and parenting stress. These were followed-up with parent and teacher report measures and a diagnostic interview at 12 months. The Fire history screening tool demonstrated utility in screening for early signs of Fire Setting. Prevalences of Fire interest, and match- and Fire-play were low overall but consistently higher for boys than for girls across ages. As expected, Fire Setting was associated with parental stress and a range of antisocial behaviours including conduct problems, hyperactivity, cruelty to animals and thrill-seeking temperament. In girls, it was also associated with anxiety/depression problems. Fire behaviours in children are related to broader psychopathology and family stress, and can be effectively identified in young children using a brief screening measure.

Although a link between Fire-Setting behavior and various organic brain disorders has been established, associations between Fire Setting and focal brain lesions have not yet been reported. Here, we describe the case of a 24-year-old first time arsonist who suffered Todd’s paralysis prior to the onset of a bizarre and random Fire-Setting behavior. A case of a 24-year-old man with a sudden onset of a bizarre and random Fire-Setting behavior is reported. The man, who had been arrested on felony arson charges, complained of difficulties concentrating and of recent memory disturbances with leg weakness. A video-EEG recording demonstrated a close relationship between the focal motor impairment and a clear-cut epileptic ictal discharge involving the bilateral motor cortical areas. The SPECT result was statistically analyzed by comparing with standard SPECT images obtained from our institute (easy Z-score imaging system; eZIS). eZIS revealed hypoperfusion in cingulate cortex, basal ganglia and hyperperfusion in frontal cortex,. A neuropsychological test battery revealed lower than normal scores for executive function, attention, and memory, consistent with frontal lobe dysfunction. The Fire-Setting behavior and Todd’s paralysis, together with an unremarkable performance on tests measuring executive function fifteen months prior, suggested a causal relationship between this organic brain lesion and the Fire-Setting behavior. The case describes a rare and as yet unreported association between random, impulse-driven Fire-Setting behavior and damage to the brain and suggests a disconnection of frontal lobe structures as a possible pathogenic mechanism.
