    Abstract We conducted an epidemiological survey to determine the status of heterophyid fluke infections among people residing along the Boseong River, Gokseong-gun, South Korea (= Korea) from October 2011 to February 2012. Fecal specimens were collected from 115 (male 51, female 64) people and examined for intestinal helminth eggs using the Kato-Katz thick smear technique. The eggs of Metagonimus yokogawai together with other Metagonimus spp. were detected in 28 (24.3%) cases. Eleven egg positive people were treated with 10 mg/kg praziquantel followed by MgSO 4 purging in order to recover the adult flukes. Whole consecutive diarrheic stools were collected individually 4–5 times. Adult flukes recovered were 66,499 specimens (6045.4/positive case) of M. yokogawai , 343 (38.1) of Metagonimus miyatai , 3293 (299.4) of Metagonimus takahashii , 81 (20.3) of Heterophyes nocens , 6 (3.0) of Heterophyopsis continua , and 1 (1.0) of Stictodora fuscata . The results indicated that the surveyed area is a highly endemic area of metagonimiasis (three Metagonimus species) with low-grade mixed infections of 3 other heterophyid flukes. The infected people experienced variable degrees of gastrointestinal discomfort and indigestion. They consumed raw freshwater and brackish water fish, including sweetfish and mullets. It is strongly recommended that people residing in the survey area avoid eating raw fish to prevent M. yokogawai and other heterophyid infections.

    In the coprolites of 4 recently discovered Joseon mummies of Korea, we found Ascaris lumbricoides, Trichuris trichiura, Metagonimus yokogawai, Paragonimus westermani, and Clonorchis sinensis eggs. The current finding was compared with previous paleoparasitological data, and with recent national survey data from Korea. For A. lumbricoides and T. trichiura, similar patterns of infection prevalence were observed between the national survey of 1961 and our current Joseon data. Some of the trematode species (C. sinenesis and P. westermani) showed much higher infection prevalences among the Joseon Koreans than among their 1960s descendants. The present results indicate that the decrease in trematode infection rates might have begun earlier than was the case for nematode infection.

  • Comparative Morphology of Minute Intestinal Fluke Eggs That Can Occur in Human Stools in the Republic of Korea

    The egg morphology of minute intestinal flukes (MIF) that can occur as human infections in the Republic of Korea, i.e., Metagonimus yokogawai, M. miyatai, M. takahashii, Heterophyes nocens, Heterophyopsis continua, Stellantchasmus falcatus, Stictodora fuscata, Pygidiopsis summa, and Gymnophalloides seoi, was studied in comparison with Clonorchis sinensis. The adult worms were obtained from residents of endemic areas, and their intrauterine eggs were studied and measured using light microscopy; the length, width, length-width ratio (LWR), and Faust-Meleney index (FMI). Several specimens were processed for scanning electron microscopy (SEM), and before gold-coating, the uterine portion of each fluke was etched with a sharp pin in order to expose the eggs. The MIF eggs were ovoid, pyriform, or elliptical with a size range of 21-35×12-21 µm. S. fuscata eggs revealed the highest FMI (largest in the area) and lowest LWR, whereas P. summa eggs showed the lowest FMI and medium LWR. SEM revealed that G. seoi and S. fuscata had remarkably clean shell surface lacking the muskmelon-like structure which is prominent in C. sinensis eggs. In Metagonimus spp., H. continua, H. nocens, and S. falcatus eggs, minute surface ridges were recognizable though less prominent compared with C. sinensis. On the surface of P. summa eggs, thread-like curly structures were characteristically seen. The results revealed that important differential keys for MIF eggs include the length, width, area (FMI), shape of the eggs, and the extent of the muskmelon-like structure or ridges on their shell surface and operculum.

    We have previously shown that parasite eggs have been identified in the coprolites of Korean mummies. These eggs have shed light on parasitic infection patterns in Korean populations living several hundred years ago. We conducted a scanning electron microscopy (SEM) study on ancient Trichuris trichiura, Ascaris lumbricoides, Metagonimus yokogawai, Paragonimus westermani, and Gymnophalloides seoi eggs recovered from Korean mummies of the Joseon Dynasty. We anticipated that the taphonomic conditions of mummification would alter the eggs of certain species but not of others. Our SEM data show that each species of ancient egg exhibited different degrees of preservation. Thus, some of them, for example, M. yokogawai, exhibited a better preservation status than others, suggesting that they should be the first candidates considered when choosing subjects for future paleoparasitological studies.

    The Tamjin River which flows from Jangheung-gun via Gangjin-gun to the South Sea was reported to be a highly endemic area of Metagonimus yokogawai infection in 1977 and 1985. However, there were no recent studies demonstrating how much change occurred in the endemicity, in terms of prevalence and worm burden, of metagonimiasis in this river basin. Thus, a small-scale epidemiological survey was carried out on some residents along the Tamjin River basin in order to determine the current status of M. yokogawai infection. A total of 48 fecal samples were collected and examined by the Kato-Katz thick smear and formalin-ether sedimentation techniques. The egg positive rate of all helminths was 50.0%, and that of M. yokogawai was 37.5%, followed by C. sinensis 22.9% and G. seoi 4.2%. To obtain the adult flukes of M. yokogawai, 6 egg positive cases were treated with praziquantel 10 mg/kg in a single dose and purged with magnesium sulfate. A total of 5,225 adult flukes (average 871 specimens per person) of M. yokogawai were collected from their diarrheic stools. Compared with the data reported in 1977 and 1985, the individual worm burdens appeared to have decreased remarkably, although the prevalence did not decrease at all. It is suggested that the endemicity of M. yokogawai infection along the Tamjin River has been reduced. To confirm this suggestion, the status of infection in snail and fish intermediate hosts should be investigated.

    Metagonimus Katsurada, 1912 is a genus of small intestinal parasites. The genus comprises eight species, primarily from far-eastern Asia, with two exceptions reported from Europe. Metagonimus yokogawai , the most widespread species, is the main agent responsible for the intestinal disease, metagonimiasis, in Japan and some other East Asian countries. On the basis of the ratio of the size of the ventral and oral suckers, Metagonimus has traditionally been morphologically divided into two groups; however, the genus has not been extensively studied using molecular data. To reveal phylogenetic relationships within Metagonimus based on molecular data, we analyzed six of the seven species present in Asia using samples collected in central Japan. Maximum likelihood and Bayesian analyses of a combined 28S ribosomal DNA (rDNA), internal transcribed spacer 2 (ITS2), and mitochondrial cox 1 gene sequence dataset separated the six species into two well-supported clades. One clade comprised M. yokogawai , M. takahashii , M. miyatai , and M. hakubaensis , whereas the other consisted of M. otsurui and M. katsuradai . Genetic distances calculated from 28S rDNA and ITS2 nucleotide sequences and a comparison of the predicted amino acid sequences of cox 1 gene suggested that M. otsurui and M. katsuradai may have diverged recently. None of the four main morphological characters used to delimit species of Metagonimus (i.e., sucker ratio, positions of the uterus and testes, and distribution of vitelline follicles) was consistent with the distribution of species in the molecular tree.

    Metagonimus yokogawai (Katsurada, 1912) Katsurada, 1912 (Trematoda: Heterophyidae) is parasitic in the small intestine of mammals including man and birds in Far Eastern Russia, Korea, Japan, China, and Taiwan. In the present study, the metacercariae and adults of M. yokogawai were redescribed to designate a neotype of this fluke together with reviews of previous studies including study histories from the first discovery to now. We particularly, attempted to review the study histories and morphological descriptions of M. yokogawai for the species validity, and compared with the morphological characteristics and life cycles of the closely related species, Metagonimus takahashii and Metagonimus miyatai. Finally, we proposed a differential key for the 8 known Metagonimus species distributed in East Asia. Metacercariae were obtained from the body muscles of sweetfish (Plecoglossus altivelis) collected in the Asahi River at Takebe-cho, Kita-ku, Okayama City, Okayama Prefecture, Japan. Adults were recovered from the small intestine of Syrian golden hamsters, to which the metacercariae had been fed 14 days before. A neotype was selected out of the present adult specimens. The Asahi River at Takebo-cho became the type locality of M. yokogawai. In conclusion, the present review shows that M. yokogawai, M. takahashii, and M. miyatai are valid and discriminated by means of morphological characteristics.

    Sweetfish, Plecoglossus altivelis, obtained from 18 rivers in Shizuoka Prefecture were examined for metacercarial infection of 2 flukes, Metagonimus yokogawai and Metagonimus miyatai. The infection rate and density of metacercariae in the fish were higher in eastern and western regions than in central region of the prefecture. After infection of hamsters with metacercariae derived from the scale, 98.7% of the adult worms obtained from the intestine was found to be M. miyatai. Conversely, from infection with metacercariae from the flesh, 90.0% of the worms was M. yokogawai. Since the worms had no exclusivity in the tissues, we conclude that the flukes have location preference with the former primarily preferring the scale, and the latter the flesh. Fish from two rivers located in adjacent areas in the western region had relatively a higher ratio of M. yokogawai in the scale relative to other rivers, suggesting an intraspecific genetic variation due to geographical isolation. On examination of adult worms in the hamster's intestine, M. yokogawai was mainly located towards the anterior part of the intestine, unlike M. miyatai, suggesting that in mammalian host too, the parasites have site preference.

  • Surface ultrastructure of Metagonimus takahashii metacercariae and adults

    A scanning electron microscopic study was performed on the surface ultrastructure of metacercariae and adults of Metagonimus takahashii. Metacercariae were collected from the scale of crucian carp (Carassius auratus), and adult flukes were harvested 1-4 weeks after infection to rats. In excysted metacercariae, the oral sucker had type I (numerous) and type II (seven in total) sensory papillae. Tegumental spines were dense and digitated into 5-7 points on the surface anterior to the ventral sucker, but became sparse and less digitated posteriorly toward the end of the body. In adults, seven type II sensory papillae were characteristically arranged around the lip of the oral sucker, and on the inner side of the lip four small and two large type I sensory papillae were symmetrically seen on each side (12 in total). Tegumental spines on anterior two-thirds of the body, were digitated with 9-12 tips ventrally and 8-13 tips dorsally. Sperms entering into the Laurer's canal were observed. The results show that the surface ultrastructure of M. takahashii is generally similar to those of M. yokogawai and M. miyatai except for the digitation of tegumental spines.

    Metagonimus miyatai n. sp. (Digenea: Heterophyidae) is described based on adult flukes collected from experimental dogs and hamsters fed with metacercariae encysted in the sweetfish, Plecoglossus altivelis, dace, tribolodon hakonensis and T. taczanowskii, common fat-minnow, Morocco steindachneri, pale chub, Zacco platypus, and dark chub, Zacco temmincki, and on those collected from naturally infected humans. The new species was morphologically compared with M. yokogawai and M. takahashii obtained from experimental animals fed with the sweetfish and the crucian carp, Carassius carassius, respectively. The uterine loops of M. miyatai reached near the posterior end of the body through the space between the two testes, whereas those of M. yokogawai, occupied only the space between the acetabulum and anterior border of two testes. This uterine tubule distribution was similar to that of another closely related species, M. takahashii. However, vitellaria of M. miyatai ended in front of the posterior end of the left testis, while those of M. takahashii reached the posterior end of the left testis and ran it over. By raising M. miyatai as a new species, differentiation of M. yokogawai and M. takahashii became very clear. A key to the species of the genus Metagonimus in the Far East has been proposed.

  • PCR-RFLP pattern of three kinds of Metagonimus in Korea

    We tried to compare the three kinds of Metagonimus species, M. yokogawai, Metagonimus Miyata type, and M. takahashii, which were known to be distributed in Korea with polymerase chain reaction based-restriction fragment length polymorphism (PCR-RFLP) patterns. We amplified the internal transcribed spacer 1 (ITS1) site of ribosomal RNA and mitochondrial cytochrome c oxidase I (mCOI) gene. The restriction patterns of ITS1 gene with Rsa I, Alu I and Msp I showed multiple fragmented bands of different sizes between three kinds of Metagonimus. In case of mCOI gene, Rsa I and Alu I enzymes produced differentially fragmented band patterns. According to the parsimony analysis of PCR-RFLP patterns, the estimated genetic divergence between M. yokogawai and Metagonimus Miyata type was 0.034880, between Metagonimus Miyata type and M. takahashii was 0.028098, between M. yokogawai and M. takahashii was 0.018179. It is suggested that Metagonimus Miyata type may be separate species and evolutionize at the older time than the other two species.

    To evaluate the status of clonorchiasis and metagonimiasis of the inhabitants near Talchongang (River) in Chungwon-gun, Chungchongbuk-do, the stools of 67 inhabitants were examined by formalin-ether sedimentation method from August to September, 1993. Also freshwater fish caught in Talchongang were examined by slide compression method. The egg positive cases of Clonorchis sinensis and Metagonimus sp. were 22 (32.8%), and 14 (20.9%), respectively. Of 17 species of examined fish. 14 species were infected with C. sinensis and 13 species with Metagonimus sp. The adult worm collected from 2 patients after treatment with praziquantel was Metagonimus Miyata type. Also the adult worm obtained from the experimental mice infected with metacercariae from Zacco platypus was Metagonimus Miyata type. We found the highly endemic area of clonorchiasis and metagonimiasis along Talchongang.

    An epidemiological study of Metogonimus infection was undertaken along the upper reaches of the Nmhn River, with special consideration on the species (type) of the worms collected from humans. Eggs of Metogonimus spp. were detected from 15 (9.7%) of 154 people examined in Umsong-gun, and from each Infected person (5 cases) 6.015- 24,060 worms (mean 13,2331 were recovered after treatment with praziquantel (10 mg/kg). Eggs were also detected from 37 (48.1% of 77 people In Yongwol-gun, from whom (27 casesl 1-4,965 worms (mean 1,2151 were collected. The worm from Umsong-gun consisted of both Metagonimus Miyata type and Metagonimus takohashii, whereas those from Yongwol-gun consisted of only Metqsonimus Miyata type. When the uterine eggs of the two kinds and M. yokogawoi (obtained from people In Tamjin River basin) were morphologically compared, it was suggested that the egg size should be a good Indicator for discrlmination of the species or type. The source of human Infection was proved to be fresh water fleshes: 49 of 52 Znsco plntvpus examined, 6 of 8 Hemibcrbus loniroskis, 13 of 15 Pseunogobio esuinl:, 4 of 6 Odontobutis obscura intemfptn, and 17 of 18 Corqssiw carassiw were found Infected with Metosonimw metacercariae. From the results, it is concluded that the upper reaches of the Namhan River are endemic foci of Metogonimn Miyata type and M. takahashii.     Abstract:

    The infection status of digenetic trematode metacercariae (DTM) was investigated in fishes from coastal lakes in Gangwon-do, the Republic of Korea (Korea). All fishes collected in 5 lakes were examined with the artificial digestion method. More than 10 species, i.e., Metagonimus spp., Pygidiopsis summa, Centrocestus armatus, Metorchis orientalis, M. taiwanensis, Clinostomum complanatum, Echinostoma spp., Stictodora spp., Diplostomum sp. and Diplostomid No. 1. by Morita (1960), of DTM were detected in fishes from 5 coastal lakes in Gangwon-do. Metagonimus spp. metacercariae were found in 52 (41.3%) out of 126 sea rundace, Tribolodon hakonensis, from 5 lakes, and their density was 14.6 per fish infected. P. summa metacercariae were detected in 48 (84.2%) out of 57 mullets from 5 lakes, and their density was 316 per fish infected. C. armatus metacercariae were detected in 7 (14.6%) T. hakonensis and 3 (15.0%) Tridentiger brevispinis from Hyang-ho, and 5 (19.2%) Acanthogobius flavimanus from Gyeongpo-ho. Stictodora spp. metacercariae were found in 4 fish species, i.e., Tridentiger obscurus, Tridentiger trigonocephalus, Chelon haematocheilus, and Acanthogobius lactipes, from Gyeongpo-ho. Total 15 C. complanatum metacercariae were detected in 2 (9.1%) crucian carp, Carassius auratus, from Songji-ho. M. taiwanensis metacercariae were found in T. hakonensis from Hyang-ho and Gyeongpo-ho and in Pseudorasbora parva from Gyeongpo-ho. Total 11 M. orientalis metacercariae were detected in 3 (6.3%) T. hakonensis from Hyang-ho. From the above results, it was confirmed that various species of DTM are infected in fishes from coastal lakes in Gangwon-do, Korea.

    Abstract Inflammatory bowel disease (IBD) is an inflammatory disease caused by the activity of effector immune cells, such as the overproduction of inflammatory cytokines. Helminth immunomodulation in the host has been shown to have therapeutic implications in IBD. In the present study, we investigated whether Metagonimus miyatai infection could ameliorate inflammatory diseases. Mice were infected with M. miyatai, and colitis was then induced through oral administration of dextran sulfate sodium (DSS). Weight loss, stool consistency, gross bleeding, colon length, and tissue inflammation were assessed by macroscopic and microscopic examinations. In addition, regulatory cytokine expression was observed in colon tissue by reverse transcription polymerase chain reaction. The results showed that M. miyatai infection decreased the clinical severity of DSS-induced colitis, including weight loss, bloody diarrhea, shortening of the colon, and colon tissue damage in mice (p

    Abstract:

    Helminth infection can alleviate immune-mediated disorders such as allergies and autoimmune diseases, by altering the gut microbiome. However, changes in gut microbiome due to intestinal trematodes remain unelucidated. Here, we evaluated the changes in the gut microbiome of ICR mice infected with Metagonimus yokogawai, a hypo-virulent intestinal trematode. Four weeks after infection, mouse cecal content was analyzed by 16S rRNA amplicon analysis. Although there was no apparent difference in species richness and diversity, the microbiome composition was different in the infected and control groups. Furthermore, several Lactobacillus species with known immunomodulatory role in immune-mediated diseases were increased in the infected group.

    Abstract:

    The infection status of zoonotic trematode metacercariae was investigated in a total of 2,293 freshwater fish collected from 11 rivers or streams in 9 administrative regions of Gangwon-do, Korea for 5 years (2009-2013). All fish were collected by netting methods and examined using the artificial digestion methods. Clonorchis sinensis metacercariae were detected in 4 fish species, i.e., Pungtungia herzi, Squalidus japonicus coreanus, Acheilognathus rhombeus, and Ladislabia taczanowskii, from only Hantangang in Cheorwon-gun. Metagonimus spp. metacercariae were found in 1,154 (50.3%) fish and their average number per infected fish was 55.8. Among the positive fish species, especially Tribolodon hakonensis from Namdaecheon in Yangyang-gun and Plecoglossus altivelis from Osipcheon in Samcheok-si were most heavily infected. Centrocestus armatus metacercariae were detected in 611 (26.7%) fish and the average metacercarial burden per infected fish was 1,032. Two chub species, Zacco platypus and Zacco temminckii were highly and heavily infected with C. armatus metacercariae in almost all regions surveyed. Echinostoma spp. metacercariae were also found in 24 fish from a few localities, but their numbers per fish infected were very low. From the above results, it is confirmed that the metacercariae of intestinal flukes, especially Metagonimus spp. and C. armatus, were heavily infected, while C. sinensis metacercariae were rarely found in fish from Gangwon-do, Korea.

    Abstract:

    The present study was performed to determine the current infecion status of Metagonimus yokogawai metacercariae in sweetfish, Plecoglossus altivelis, collected from several streams in eastern and southern coastal areas of Korea. The sweetfish collected were artificially digested with pepsin-HCl solution and examined under a stereomicroscope in August and September, 2007. Out of 145 sweetfish collected from 10 streams in eastern coasts (Gangwon-do and Gye-ongsangbuk-do), 88 (60.7%) were infected with Metagonimus metacercariae. The average metacercarial density was 61 per infected fish. Among 141 sweetfish collected from 10 streams in southern coasts (Gyeongsangnam-do, Ulsan Metropolitan city, and Jeollanam-do), 140 (99.3%) were infected with Metagonimus metacercariae, and their average density was 949 per infected fish. The present study confirmed that M. yokogawai metacercariae are still prevalent in the sweetfish from several eastern and southern coastal localities. The prevalence and metacercarial density were much higher in the sweetfish from streams of southern coasts than in those of eastern coasts. Therefore, attention should be paid to this small fluke infection, and consumption of raw sweetfish naturally produced in these areas should be prohibited.
