The etiology of MCS is poorly understood, but dysregulation of the immune system has been proposed as part of the pathophysiology. Objective To compare plasma levels of cytokines in Danish MCS individuals with a healthy, sex- and age-matched control group. Method Blood samples were obtained from 150 un-exposed MCS individuals and from 148 age- and sex-matched healthy controls. Plasma concentrations of 14 cytokines, chemokines and growth and allergen-specific IgE were measured. All participants completed a questionnaire including questions on MCS, psychological distress, morbidities and medication use at the time of the study. Although the pathophysiology of MCS remains unknown, central sensitization may be an important factor contributing to the clinical manifestations. Purpose To use quantitative sensory testing (QST) to study central hyperexcitability and Multiple aspects of central sensory processing in MCS patients without comorbid overlapping disorders and to compare the results with those among matched controls. Methods 15 MCS patients and 15 healthy matched controls underwent QST to assess the following aspects of pain: capsaicin-induced secondary punctate hyperalgesia, stimulus response function (SRF) to punctate mechanical stimuli before and after capsaicin injection, temporal summation to punctate stimuli post capsaicin injection, pressure pain thresholds, heat pain thresholds, tonic heat stimulation and conditioning pain modulation (CPM: formerly known as diffuse noxious inhibitory control or DNIC). Results The mean area of capsaicin-induced secondary punctate hyperalgesia was significantly larger in MCS patients than in controls at 5, 30 and 60 min post capsaicin injection (p = 0.01). In addition MCS patients reported higher ratings in response to punctate mechanical stimuli assessed by SRF compared with controls (p Conclusion Increased capsaicin-induced secondary punctate hyperalgesia was demonstrated in MCS patients without comorbid, overlapping disorders, suggesting facilitated central sensitization in MCS.

Various genes, especially genes of importance to the metabolism of xenobiotic compounds, have been associated with MCS, but findings are inconsistent. The purpose of this study was to investigate genetic susceptibility factors for MCS and self-reported Chemical Sensitivity in a population sample. Ninety six MCS patients and 1,207 controls from a general population divided into four severity groups of Chemical Sensitivity were genotyped for variants in the genes encoding cytochrome P450 2D6, arylamine N-acetyltransferase 2, paraoxonase 1, methylene tetrahydrofolate reductase, and the cholecystokinin 2 receptor. No hypotheses were consistently confirmed. An apparent association between number of active cytochrome P450 2D6 alleles and MCS status was not statistically significant (OR=1.2, p=0.28). Fast arylamine N-acetyltransferase 2 metaboliser status was associated with severity of Chemical Sensitivity only in the most severely affected group in the population sample (OR=3.1, p=0.04). The cholecystokinin 2 receptor allele with 21 CT repeats was associated with MCS when compared in post hoc analyses with all individuals from the population sample (p=0.02). Genetic variants in paraoxonase 1 and methylene tetrahydrofolate reductase were not associated with MSC or with self-reported Chemical Sensitivity in the population sample. Our results suggest that variants in the genes examined are of less importance to MCS than previously reported or that gene-environment interactions or significant degrees of genetic heterogeneity in MCS underlie inconsistent findings in the literature.

Metals are a well-known primary risk factor for idiopathic environmental intolerance. It is important that the medical approach designed to reduce the risk factors for MCS includes preventive strategies of metal exposure.

Multiple Chemical Sensitivity (MCS) is a chronic condition characterized by an exaggerated response to toxicants. We ascertained the prevalence of allergy to metals and toxicological aspects in MCS patients. Methods. We conducted a retrospective review of medical records of 41 patients with MCS. We performed patch testing () for dental series and did lymphocyte transformation test () for metals. We measured mercury in samples of blood (), urine (), saliva (), and scalp hair () to investigate the association between mercury levels and cases of MCS. Results. The prevalence of metal immune hyperSensitivity in a subset of 26 patients was 92.3 percent. Elevations of mercury occurred in 81.2 percent (26 of 32). The mean (±SD) in blood concentrations of mercury was μg/L; mean in urine was μg/L; mean in scalp hair was μg/g; mean in saliva was μg/L. Subgroup analyses showed that elevation of mercury levels in biological matrices were associated with mercury amalgams in patients with MCS (22 patients), compared with controls (8 patients) (odds ratio 11 : 95 percent confidence interval 1.5 to 81.6; ). Conclusions. Our data show an increased prevalence of metal allergy and elevation of mercury levels in bioindicators among patients with MCS.

We previously observed significant activations in the prefrontal cortex (PFC) during olfactory stimulation using several different odorants in patients with MCS by near-infrared spectroscopy (NIRS) imaging. We also observed that the patients with MCS did not adequately distinguish non-odorant in the late stage of the repeated olfactory stimulation test. The sensory recovery of the olfactory system in the patients with MCS may process odors differently from healthy subjects after olfactory stimulation.

However, no cutoff value for Japanese subjects has yet been established. We designed this study to establish a cutoff value for Japanese subjects using QEESI^© for screening of MCS patients. Methods A questionnaire using the QEESI^© was administered to 103 MCS patients and 309 healthy control subjects matched for age and sex. QEESI^© scores of the two groups were compared using logistic regression analysis, receiver operating characteristic analysis, and the Mann–Whitney test. Results Cutoff values for Japanese subjects were determined for the Chemical Intolerance subscale (40), Symptom Severity subscale (20), and Life Impact subscale (10). The subjects whose scores exceeded the cutoff values in any two subscales accounted for 88.4% of the patients but only 14.5% of the controls. Conclusions Our results suggest that subjects meeting two out of three subscale criteria can be screened as “patients suffering from a low level of environmental Chemicals such as MCS” in Japan.

The aim of our study was to clarify the clinical characteristics of physician-diagnosed MCS patients in Japan. We analyzed patient characteristics based on the medical records of 106 patients diagnosed with MCS according to the 1999 Consensus and the Japanese diagnostic criteria for MCS. We evaluated subjective symptoms using the Quick Environment Exposure Sensitivity Inventory (QEESI © ) and compared the QEESI © scores with those of four MCS patient groups in the US reported by Miller and Prihoda [Miller, C.S., Prihoda, T.J., 1999b. A controlled comparison of symptoms and Chemical intolerances reported by Gulf War veterans, implant recipients, and persons with Multiple Chemical Sensitivity. Toxicol Ind Health 15, 386-397]. Female patients accounted for 74.0%. Most male patients were in their 30s, whereas female ages ranged widely from 10 to 65 years. Among estimated onset factors, those seen in males tended to be workplace related, while female patients showed a variety of factors. Co-morbid allergic disease was present in 84.0% of patients. A significant difference in the QEESI © score between male and female patients was found in only one item out of 10 in symptom severity and life impact. However, all 10 items in Chemical intolerance were significantly higher in females than in males. The mean QEESI © score in the patient group in our study was lower than those in any of the four self-reported patient groups in the US.

Symptoms of SHS/SBS disappear after leaving the sick house/building, while symptoms of Multiple Chemical Sensitivity (MCS) are elicited by the chance of Chemical exposure after leaving the sick house/building. To establish the concept of SHS, we propose to introduce a new classification for SHS. A total of 214 patients complaining of SHS and/or MCS were independently classified using a new classification by clinical ecologists who are experienced physicians with expert knowledge of clinical ecology and general physicians according to disease pathogenesis from clinical records. The classification is as follows: type 1 (symptoms of Chemical intoxication), type 2 (symptoms developed possibly due to Chemical exposure), type 3 (symptoms developed not because of Chemical exposure but rather because of psychological or mental factors), and type 4 (symptoms developed due to allergies or other diseases). The agreements on the classification made by clinical ecologists and general physicians reached 77.1% (Cohen's kappa = 0.631), suggesting that this new classification was both apt and accurate. Relations between SHS and allergy/MCS were also studied. The cases classified as SHS type 4 more frequently had allergic past histories than did other types. The proportion of possible MCS cases was higher in the Chemical induced SHS group (types 1 and 2) than in other types among male patients. For the universal use in clinical practice, it is necessary to prepare helpful diagnostic criteria of this SHS classification based on pathogenesis and carry our study forward all over the country.

International Scholarly Research Notices, 2011Co-Authors: Pamela Reed Gibson, Amanda LindbergAbstract:Ninety physicians practicing in the state of Virginia USA completed a mail survey regarding Multiple Chemical Sensitivity (MCS). Survey questions addressed demographics; familiarity with MCS; etiology; overlapping conditions; accommodations made for patients and practices regarding evaluation, treatment, and referral. A little over half of respondents were familiar with MCS. Under a third had received any medical training regarding Chemical Sensitivity, only 7% were “very satisfied” with their knowledge, and 6% had a treatment protocol for the condition. Participants cited a range of etiologies and overlapping conditions including asthma, Reactive Airway Dysfunction Syndrome (RADS), Sick Building Syndrome (SBS), Chronic Fatigues Syndrome (CFS), and Fibromyalgia. Physicians infrequently considered Chemicals as a cause of illness when seeing new patients. Evaluation techniques included interviews, blood work, immune profiles, and allergy testing. Interventions recommended included Chemical avoidance, alterations in the home environment, diet restrictions, the use of air filters, and referrals to outside specialists.

We used a phenomenological perspective and planned to focus on the impact of MCS on relationships, but found that difficulty with accessing safe spaces at times prevented relationships by limiting access to personal interactions. Thus, persons with MCS either lost or were unable to cultivate new relationships as a result of lack of spatial access. Others' lack of understanding and refusal to make accommodations at times denied spatial access to those with MCS. In this way, relationships (relationality) and spatial access (spatiality) interacted with one another to keep persons isolated. In this paper, we describe informants' detailed experience of living with Chemical intolerance in a culture where Chemical exposures are commonplace.

Sickness-related dysfunction in persons with self-reported Multiple Chemical Sensitivity at four levels of severity

VogelAbstract:AIM: To examine quality of life outcome for persons who self-report Chemical Sensitivity, often referred to as Multiple Chemical Sensitivity. BACKGROUND: Multiple Chemical Sensitivity is poorly understood with few providers specialising in its treatment. This lack of treatment and the ubiquity of Chemicals engender severe life impacts such as job loss, financial loss, social isolation and even homelessness for persons who experience these sensitivities. DESIGN: Survey. METHOD: We examined Chemical incitants, symptoms and sickness-related behavioural dysfunction as measured by the Sickness Impact Profile in 254 persons self-identified with Multiple Chemical Sensitivity. RESULTS: Chemicals rated as causing the most symptomatology in respondents were pesticide, formaldehyde, fresh paint, new carpet, diesel exhaust, perfume and air fresheners. The five highest rated symptoms in this sample were tiredness/lethargy, difficulty concentrating, muscle aches, memory difficulties and long-term fatigue. Overall mean Sickness Impact Profile score was 25.25%, showing serious impairment, with the most serious dysfunction in the categories of work (55.36%), alertness behaviour (53.45%) and recreation and pastimes (45.20%). CONCLUSION: Multiple Chemical Sensitivity is an important health care issue because it often includes serious dysfunction, is poorly understood by providers and poses extensive financial and treatment obstacles for those who experience it. RELEVANCE TO CLINICAL PRACTICE: Persons with Multiple Chemical Sensitivity seek medical treatment in a variety of contexts and informed providers can both avoid iatrogenic harm due to medical exposures and provide any possible treatment for the Chemical sensitivities. Understanding the impact of the health condition is crucial to communicate with and treat persons who experience the sensitivities.

It is sometimes associated with electromagnetic Sensitivity, and both conditions are most likely engendered by environmental exposures. Prevalence studies suggest that Chemical Sensitivity affects 12–15% of the US population and that it crosses age, socio‐economic status and racial lines. Electromagnetic Sensitivity is less understood, but affects a portion of those with Chemical Sensitivity. Persons with these environmental sensitivities often experience difficulties with health care, housing and work. Workplace barriers result in significant disability, with ensuing financial loss and isolation for the worker. We examined work accommodations for 100 persons with self‐reported MCS and its relation to life satisfaction. Results are discussed in terms of accommodating invisible disabilities that challenge industrial capitalism.

Authors then examined responses for themes, which they discuss within the framework of critical theory. Emergent themes included loss of a stable, familiar personality, loss of self-positioning, emotional suppression to meet others’ expectations, redesigning the planned life, forced growth, struggling with support, discovering the spiritual self, and identity reconsolidation. The authors compare findings with published works on adjustment to chronic illness and other delegitimized illnesses, find them to be fairly congruent, and then discuss problems regarding cultural acceptance of MCS as a condition caused by Chemical exposure.
