    Low inhibitory control is linked with weight gain among youth. Inhibitory problems are associated with disruption to the hypothalamic-pituitary-adrenal axis cortisol response. Increased cortisol predicts appetite and weight gain (though may be gender specific). This study hypothesized that cortisol reactivity explains the association between inhibition and weight gain while considering the moderating factors of early stressors to the hypothalamic-pituitary-adrenal axis (e.g. Prenatal-Drug Exposure) and gender. Adolescents with Prenatal-Drug Exposure (n = 76) and non-exposed comparison adolescents (NE; n = 61) completed the Conner's Continuous Performance Test and provided salivary cortisol samples. BMI z-score were measured at the initial and 12-month follow-up evaluations. A bootstrapped moderated mediation analysis was conducted to test for conditional indirect effects of cortisol reactivity. Lower inhibition was associated with increased cortisol reactivity among youth who were NE, and increased cortisol reactivity was associated with weight gain among girls. Cortisol reactivity mediated the relation between inhibition and BMI z-score change for the girls in the group who was NE. Increased cortisol reactivity may play a mechanistic role in predicting weight gain among non-Prenatally Drug-exposed girls. Cortisol reactivity may be a biomarker for targeted interventions to improve biological regulation and ultimately health risk among girls. © 2018 World Obesity Federation.

    Abstract The objective of the current study was to examine what stage of memory (encoding or retrieval) may be compromised in adolescents with a history of Prenatal Drug Exposure (PDE) and how the effects of PDE on memory ability are substantiated at the neural level. To achieve this goal, we examined memory performance and associated brain activations in adolescents with and without a history of PDE via event-related fMRI during encoding and retrieval. Consistent with previous studies, we found that PDE subjects remembered fewer items than community comparison subjects. However, there were no differences in behavior after adjusting for correct rejections (i.e., d′). Novel extensions of previous work are findings that PDE is associated with changes in brain activation during memory encoding but not during retrieval. These results suggest that less optimal memory performance often observed in adolescents with a history of PDE may result from variations in encoding rather than retrieval processes.

    Objective: To examine stress reactivity in a sample of adolescents with Prenatal Drug Exposure (PDE) by examining the consequences of PDE on stress-related adrenocortical reactivity, behavioral problems, and Drug experimentation during adolescence. Methods: Participants (76 PDE, 61 non-Drug exposed [NE]; 99% African-American; 50% male; mean age = 14.17 yr, SD = 1.17) provided a urine sample, completed a Drug use questionnaire, and provided saliva samples (later assayed for cortisol) before and after a mild laboratory stress task. Caregivers completed the Behavior Assessment System for Children, Second Edition (BASC II) and reported their relationship to the adolescent. Results: The NE group was more likely to exhibit task-related cortisol reactivity compared to the PDE group. Overall behavior problems and Drug experimentation were comparable across groups with no differences between PDE and NE groups. In unadjusted mediation analyses, cortisol reactivity mediated the association between PDE and BASC II aggression scores (95% bootstrap confidence interval [CI], 0.04–4.28), externalizing problems scores (95% bootstrap CI, 0.03–4.50), and Drug experimentation (95% bootstrap CI, 0.001–0.54). The associations remain with the inclusion of gender as a covariate but not when age is included. Conclusion: Findings support and expand current research in cortisol reactivity and PDE by demonstrating that cortisol reactivity attenuates the association between PDE and behavioral problems (aggression) and Drug experimentation. If replicated, PDE may have long-lasting effects on stress-sensitive physiological mechanisms associated with behavioral problems (aggression) and Drug experimentation in adolescence.

    Abstract The persistence of effects of Prenatal Drug Exposure (PDE) on brain functioning during adolescence is poorly understood. We explored neural activation to a visuospatial working memory (VSWM) versus a control task using functional magnetic resonance imaging (fMRI) in adolescents with PDE and a community comparison group (CC) of non-exposed adolescents. We applied graph theory metrics to resting state data using a network of nodes derived from the VSWM task activation map to further explore connectivity underlying WM functioning. Participants (ages 12–15 years) included 47 adolescents (27 PDE and 20 CC). All analyses controlled for potentially confounding differences in birth characteristics and postnatal environment. Significant group by task differences in brain activation emerged in the left middle frontal gyrus (BA 6) with the CC group, but not the PDE group, activating this region during VSWM. The PDE group deactivated the culmen, whereas the CC group activated it during the VSWM task. The CC group demonstrated a significant relation between reaction time and culmen activation, not present in the PDE group. The network analysis underlying VSWM performance showed that PDE group had lower global efficiency than the CC group and a trend level reduction in local efficiency. The network node corresponding to the BA 6 group by task interaction showed reduced nodal efficiency and fewer direct connections to other nodes in the network. These results suggest that adolescence reveals altered neural functioning related to response planning that may reflect less efficient network functioning in youth with PDE.

    Prenatal Exposure to recreational Drugs impairs motor and cognitive development; however it is currently unknown whether visual brain areas are affected. To address this question, we investigated the effect of Prenatal Drug Exposure on global motion perception, a behavioural measure of processing within the dorsal extrastriate visual cortex that is thought to be particularly vulnerable to abnormal neurodevelopment. Global motion perception was measured in one hundred and forty-five 4.5-year-old children who had been exposed to different combinations of methamphetamine, alcohol, nicotine and marijuana prior to birth and 25 unexposed children. Self-reported Drug use by the mothers was verified by meconium analysis. We found that global motion perception was impaired by Prenatal Exposure to alcohol and improved significantly by Exposure to marijuana. Exposure to both Drugs Prenatally had no effect. Other visual functions such as habitual visual acuity and stereoacuity were not affected by Drug Exposure. Prenatal Exposure to methamphetamine did not influence visual function. Our results demonstrate that Prenatal Drug Exposure can influence a behavioural measure of visual development, but that the effects are dependent on the specific Drugs used during pregnancy.

    OBJECTIVE:: High-risk environments characterized by familial substance use, poverty, inadequate parental monitoring, and violence Exposure are associated with an increased propensity for adolescents to engage in risk-taking behaviors (e.g., substance use, sexual behavior, and delinquency). However, additional factors such as Drug Exposure in utero and deficits in inhibitory control among Drug-exposed youth may further influence the likelihood that adolescents in high-risk environments will engage in risk-taking behavior. This study examined the influence of Prenatal substance Exposure, inhibitory control, and sociodemographic/environmental risk factors on risk-taking behaviors in a large cohort of adolescents with and without Prenatal cocaine Exposure (PCE). METHOD:: Risk-taking behavior (delinquency, substance use, and sexual activity) was assessed in 963 adolescents (433 cocaine-exposed, 530 nonexposed) at 15 years of age. RESULTS:: Prenatal cocaine Exposure predicted later arrests and early onset of sexual behavior in controlled analyses. Associations were partially mediated, however, by adolescent inhibitory control problems. PCE was not associated with substance use at this age. In addition, male gender, low parental involvement, and violence Exposure were associated with greater odds of engaging in risk-taking behavior across the observed domains. CONCLUSIONS:: Study findings substantiate concern regarding the association between Prenatal substance Exposure and related risk factors and the long-term outcomes of exposed youth. Access to the appropriate social, educational, and medical services is essential in preventing and intervening with risk-taking behaviors and the potential consequences (e.g., adverse health outcomes and incarceration), especially among high-risk adolescent youth and their families.     The purpose of this article was to review follow up studies of children with Prenatal Drug Exposure from preschool through adolescence. Specifically, the authors focus on the effects of Prenatal Exposure to cocaine, methamphetamine, and opiates on behavior and development. The largest number of studies have examined cocaine-exposed children. The authors identified 42 studies that suggest that there are unique effects of Prenatal cocaine Exposure on 4- to 13-year-old children, particularly in the areas of behavior problems, attention, language, and cognition. In addition, studies make reasonable attempts to control for possible confounding factors. Systematic research on the long-term effects of Prenatal methamphetamine Exposure is just beginning but seems to be showing similar effects to that of cocaine. The literature on the on the long-term effects of children with Prenatal opiate Exposure is more substantial than the methamphetamine literature but it is still relatively sparse and surprising in that there is little recent work. Thus, there are no studies on the current concerns with opiates used for prescription mediation. There is a growing literature using neuroimaging techniques to study the effects of Prenatal Drug Exposure that holds promise for understanding brain/behavior relationships. In addition to pharmacological and teratogenic effects, Drugs can also be viewed from a Prenatal stressor model. The author discuss this "fetal origins" approach that involves fetal programming and the neuroendocrine system and the potential implications for adolescent brain and behavioral development.

    Sleep data were collected by maternal report in a prospective longitudinal follow up of cocaine-exposed and unexposed children. There were 139 participants: 23 with no Prenatal Drug Exposure, 55 exposed to cocaine alone or in combination with other Drugs, and 61 exposed to Drugs other than cocaine. Characteristics differed between Exposure groups including birth size, caretaker changes, maternal socioeconomic status, and postnatal Drug use. Compared to those with no Drug Exposure, children with Prenatal Drug Exposure other than cocaine experienced greater sleep problems (p = .026). Prenatal nicotine Exposure was a unique predictor of sleep problems (p = .048). Early sleep problems predicted later sleep problems (all ps < .01). Together, these preliminary findings suggest possible neurotoxic sleep effects that persist over time. Larger studies, however, need to be conducted that better control for potential postnatal confounding factors.

    Based on the estimates of the 2007 National Survey on Drug Use and Health (NSDUH), 9.5% of women of child-bearing age (ages 15–44 years)—almost six million—use illicit Drugs annually (SAMHSA 2007). A lower proportion of use is estimated among pregnant women (5.2%), but this rate is high enough to make illicit Drug use during pregnancy a major public health concern. However, among 15- to 17-year-olds, the rate of illicit Drug use during pregnancy is 22.6%, higher than the rate of use (13.3%) in those not pregnant in same age range (SAMHSA 2007). The commonly used illicit substances by women of child-bearing age include marijuana and hashish, cocaine, heroin, hallucinogens, and psychotherapeutic agents including pain relievers, tranquilizers, stimulants, and sedatives. Of interest is the increasing nonmedical use of prescription medications; rate of use of pain relievers among women of child-bearing age is 2.6%, higher than the reported rates of cocaine (1.0%) and heroin (0.1%) use. The use of hallucinogens and inhalants is more common among teens (SAMHSA 2007). The rates and types of substance used vary among different sociodemographic characteristics such as marital status, level of education, employment status, method of hospital payment, and ethnicity (NIH 1996). In 2006–2007, the annual rate of Drug use during pregnancy was highest among non-Hispanic white women (6.2%), slightly higher than in the non- Hispanic African American women (5.7%). An earlier survey on Drug use during pregnancy found that the rate of cocaine use was highest among African Americans (4.5%) and in the those older than 25 years of age, whereas the younger age group had the highest rate of marijuana use (3.5%) (NIH 1996). In utero Exposure to illicit substances may be associated with adverse effects on the developing brain through various mechanisms, with resultant physical and or cognitive and behavioral abnormalities. This chapter focuses on cognitive and behavioral outcomes in childhood and adolescence following Prenatal illicit Drug Exposure. However, it is essential to realize that most women who use Drugs are polyDrug users; that is, they also use tobacco and/or alcohol, or a combination of other illicit Drugs.

    Objective To examine the relationship between early parenting stress and later child behavior in a high-risk sample and measure the effect of Drug Exposure on the relationship between parenting stress and child behavior. Methods A subset of child-caregiver dyads (n = 607) were selected from the Maternal Lifestyle Study (MLS), which is a large sample of children (n = 1,388) with Prenatal cocaine Exposure and a comparison sample unexposed to cocaine. Of the 607 dyads, 221 were Prenatally exposed to cocaine and 386 were unexposed to cocaine. Selection was based on the presence of a stable caregiver at 4 and 36 months with no evidence of change in caregiver between those time points. Results Parenting stress at 4 months significantly predicted child externalizing behavior at 36 months. These relations were unaffected by cocaine Exposure suggesting the relationship between parenting stress and behavioral outcome exists for high-risk children regardless of Drug Exposure history. Conclusions These results extend the findings of the relationship between parenting stress and child behavior to a sample of high-risk children with Prenatal Drug Exposure. Implications for outcome and treatment are discussed.

    OBJECTIVE:: High-risk environments characterized by familial substance use, poverty, inadequate parental monitoring, and violence Exposure are associated with an increased propensity for adolescents to engage in risk-taking behaviors (e.g., substance use, sexual behavior, and delinquency). However, additional factors such as Drug Exposure in utero and deficits in inhibitory control among Drug-exposed youth may further influence the likelihood that adolescents in high-risk environments will engage in risk-taking behavior. This study examined the influence of Prenatal substance Exposure, inhibitory control, and sociodemographic/environmental risk factors on risk-taking behaviors in a large cohort of adolescents with and without Prenatal cocaine Exposure (PCE). METHOD:: Risk-taking behavior (delinquency, substance use, and sexual activity) was assessed in 963 adolescents (433 cocaine-exposed, 530 nonexposed) at 15 years of age. RESULTS:: Prenatal cocaine Exposure predicted later arrests and early onset of sexual behavior in controlled analyses. Associations were partially mediated, however, by adolescent inhibitory control problems. PCE was not associated with substance use at this age. In addition, male gender, low parental involvement, and violence Exposure were associated with greater odds of engaging in risk-taking behavior across the observed domains. CONCLUSIONS:: Study findings substantiate concern regarding the association between Prenatal substance Exposure and related risk factors and the long-term outcomes of exposed youth. Access to the appropriate social, educational, and medical services is essential in preventing and intervening with risk-taking behaviors and the potential consequences (e.g., adverse health outcomes and incarceration), especially among high-risk adolescent youth and their families.     Abstract:

    To determine the factors that would increase the likelihood of outcomes: low birth weight (LBW), preterm births and intrauterine growth restriction (IUGR). Secondary data analysis from a multi-center study. Risk factors for each outcome were derived from logistic regression models. Odds ratios (OR), 95% confidence intervals, and population-attributable risk proportions (PAR%) were estimated. Prenatal cocaine Exposure increased the likelihood of LBW (OR: 3.59), prematurity (OR: 1.25), and IUGR (OR: 2.24). Tobacco, but not marijuana, significantly influenced these outcomes. Alcohol had an effect on LBW and IUGR. Etiologic fractions (PAR%) attributable to tobacco for LBW, prematurity, and IUGR were 5.57, 3.66, and 13.79%, respectively. With additional Drug Exposure including cocaine, estimated summary PAR% increased to 7.20% (LBW), 5.68% (prematurity), and 17.96% (IUGR). Disease burden for each outcome increases with each added Drug Exposure; however, etiologic fraction attributable to tobacco is greater than for cocaine.
