All-cause mortality by vitamin B status, using dietary (B-1, B-2, niacin, B-6) or biochemical data (erythrocyte transketolase reductase, erythrocyte glutathione reductase, plasma pyridoxal-phosphate, folate and serum B-12) was evaluated. Methods The Taiwanese Elderly Nutrition and Health Survey (1999–2000) provided 1747 participants 65 years and over. Dietary and biochemical data were collected at baseline. Survivorship was determined until December 31, 2008. Results Survivors had higher vitamin B-1 and niacin intakes and pyridoxal-phosphate and folate concentrations. Controlled for confounders, and relative to the lowest tertile of vitamin B-1 or B-6 intakes, the hazard ratios (95% confidence interval) for tertile 3 were 0.74 (0.58–0.95) and 0.74 (0.57–0.97); both p for trend values were Conclusions Higher vitamin B-1 and B-6 intakes and plasma pyridoxal-phosphate were associated with lower risk of mortality up to 10 years and could be achieved by increased dietary diversity.

Dietary diversity and food expenditure as indicators of food security in older Taiwanese.

The Elderly are often Nutritionally vulnerable, but how much of this is reflected in food quality and determined by financial status is unclear. We determined whether expenditure on dietary quality challenges food security in the aged. We used the representative Elderly Nutrition and Health Survey in Taiwan during 1999–2000 (n = 1783), and evaluated dietary quality by a Dietary Diversity Score (DDS, range: 0–6) based on a 24-h dietary recall. Monthly mean national food prices were used to estimate food expenditure. In general, it was found to cost more to achieve a greater DDS. The food expenditure of subjects whose DDS = 6 was 2.20 times greater than the DDS ⩽3 group, after controlling for covariates. Elders of lower socioeconomic status tended to choose foods which would have cost less. However, a sub-group of elders who achieve the highest DDS with limited money offer approaches to food-money management. Nutrition policy directed to food insecure groups, like the aged, could employ health promotion strategies which reduce financial barriers to healthy eating.

The Effects of Physical Activity, Body Mass Index Bmi and Waist Circumference Wc on Glucose Intolerance in Older People: A Nationwide Study from Taiwan

In this cross-sectional study, physical activity was assessed using the Modified Baecke Questionnaire for Older Adults (MBQOA). Categories of physical activity level were defined by tertiles of MBQOA scores. Glucose intolerance in subjects not previously diagnosed with diabetes was categorized according to 2003 American Diabetes Association criteria. After adjustment for potential confounders, physical activity was significantly inversely associated with the presence of undiagnosed type 2 diabetes and impaired fasting glucose (IFG) in older women. In older men, the association was less clear. BMI and WC were significantly positively associated with the presence of undiagnosed diabetes in men and were significantly associated with IFG in both sexes. In older women, undiagnosed diabetes was strongly associated with increased WC, but not with BMI. Our findings highlight that older women with low physical activity or high WC, and older men with high BMI or WC are important target populations for interventions to prevent glucose intolerance.

Therefore, we aimed to investigate the prospective relationship between uric acid and development of AF in a nationally representative cohort of Elderly people. Methods and results: A total of 1485 Elderly people (age � 65 yrs) from the Elderly Nutrition and Health Survey in Taiwan (1999e2000) were without AF on "electrocardiography" at baseline. Inci- dentAFevents(InternationalClassificationofDiseases,NinthRevision,ClinicalModification,ICD-9- CM: 427.31) were identified using data from the National Health Insurance Dataset. Hyperuricemia was defined as levels of uric acid >7.0 mg/dL in men and 6.0 mg/dL in women. A Cox proportional hazards model was used to evaluate the association between hyperuricemia and incident AF. Elderly Nutrition and Health Survey in Taiwan (1999-2000): research design, methodology and content.

A multi-staged, stratified, clustered probability sampling scheme was used in the survey. The survey population was stratified into a total of 13 strata. The four strata of “Hakka areas”, “Mountain areas”, “Eastern areas”, and “PengHu islands” were unique in their ethnicity or geographic locations. The remaining areas of Taiwan were stratified into “Northern”, “Central”, and “Southern” parts. Within these 3 strata, then each was subdivided into a further 3 strata based on population density. The household interview of the survey was arranged such that effect of seasonal variation was taken into account. A total of 1,937 persons completed the interview and 2,432 persons completed the health exam. The following data were collected: (1) Interview data : household information, basic demographics, 24 hour dietary recall, food frequency and habit, knowledge, attitudes and practice medical history, 36-item Short Form for generic health status, and physical activity. (2) Health exam data: blood sample for measurement of Nutritional biochemical indicators and complete clinical chemistry profile, urine sample for urinary electrolytes, anthropometric measurements, ECG, blood pressure, body temperature, pulmonary function, and an osteoporosis assessment. Data from the survey were analyzed using SUDAAN to adjust for the design effect and to obtain unbiased estimates of the mean, standard error and confidence intervals. Survey respondents were slightly younger compared to non-respondents; however, after weighting and adjustment with SUDAAN, the education levels and ethnicity of respondents and non-respondents were similar indicating lack of bias. We anticipate that the results of this survey will be of benefit in understanding the Nutritional status of the Elderly, the relationship between Nutrition and health, and factors influencing Elderly persons’ Nutritional status. Furthermore, this information could be used in the development of public health Nutrition policy aimed at improving the Nutrition and health of the Elderly in Taiwan.

Evaluation of the USDA Elderly Nutrition Demonstration Volume I: Evaluation Findings

To identify effective strategies for increasing participation among this population, the U.S. Department of Agriculture funded the Elderly Nutrition Demonstrations between 2002 and 2004—six projects aimed at testing ways to increase FSP participation among eligible Elderly individuals. This report presents findings from Mathematica’s evaluation of the demonstrations. The results suggest that a variety of approaches can be effective in increasing participation among the Elderly, including efforts to simplify the application process, to provide individual assistance in completing the application, and to make FSP benefits more appealing to the Elderly.

Evaluation of the USDA Elderly Nutrition Demonstrations Volume II, Demonstration Summaries

To identify effective strategies for raising participation among this population, USDA designed three models, each using different techniques to reduce the barriers that seniors face in FSP participation. The techniques involve reducing the time and effort of applying for benefits, aiding seniors in navigating the application process, and giving seniors the option of receiving commodity packages instead of getting benefits through electronic benefits transfer cards. The models were tested as county demonstrations in six States between 2002 and 2004. This report presents the findings of the in-depth process analysis component of an evaluation of the demonstrations. Each of the demonstrations was examined individually for overall design and implementation.

Food Stamp Program Elderly Nutrition Demonstrations: Interim Report on Elderly Participation Patterns
Mathematica Policy Research Reports, 2004Co-Authors: Scott CodyAbstract:To raise participation in the Food Stamp Program by low-income Elderly people, USDA implemented the Elderly Nutrition Demonstration in Arizona, Connecticut, Florida, Maine, Michigan, and North Carolina in 2002. Each demonstration used one of three strategies to increase participation: (1) simplifying eligibility requirements for those applying for food stamps, (2) directly assisting applicants with completing the application process, and (3) offering applicants the option of receiving packages of commodities each month instead of getting benefits through an electronic benefits transfer card. Preliminary analysis indicates that participation rose substantially after the demonstrations started. There is also some evidence that the demonstrations attract those eligible for relatively low benefits, particularly in Maine and North Carolina, where a large number of people eligible for a $10 benefit are applying.

The Interrelationship of Nutritional Risk Factors, Indicators of Nutritional Risk, and Severity of Disability Among Home-Delivered Meal Participants

SharkeyAbstract:Purpose: This study examines the direct and indirect relationships between individual components of Nutritional risk and increased severity of disability among a large and diverse sample of homebound older adults. Design and Methods: Using routinely collected Nutrition and function data, structural equation modeling of recursive and nonrecursive models examined the interrelationships of Nutritional risk factors, indicators of Nutritional risk, and disability severity among 1,010 home-delivered meals program participants in Wake County, NC. Results: The equally good fit for both the recursive and nonrecursive structural models revealed that specific Nutritional risk factors were directly and indirectly associated with indicators of Nutritional risk and increased severity of disability. The nonrecursive model also revealed significant reciprocal associations of increased disability with unintended weight change and medication use. Implications: The findings from this study acknowledge aspects of the complex direct and indirect relationships between Nutrition and function among homebound older persons. This knowledge will help service providers with the development of effective Elderly Nutrition programs with Nutritional and functional status outcomes.

Use of Telephone-Administered Survey for Identifying Nutritional Risk Indicators Among Community-Living Older Adults in Rural Areas

HainesAbstract:The ability of Elderly Nutrition Programs, especially in rural areas, to address increasing needs for disparate program services is linked to determining which older people are most likely to be at Nutritional risk and what constitutes the potential risk. The purpose of this study is to characterize Nutritional risk factors in a probability sample of rural elders (n = 152) through telephone interviews using a 67-question modified version of the Nutrition Screening Initiative's Level I and II screens. Participants reported high levels of unintentional weight loss, medication use, multiple health conditions, and depression. This study demonstrates that comprehensive information on general health, eating habits, living environment, and functional status can be collected from rural elders by telephone.
