METHODS: Between 1995 and 2004, 59 neurologically intact men suffering from anejaculation underwent multiple Electroejaculations. Sperm quality of the first and the second ejaculates was compared. RESULTS: A significant difference of 0.33 ± 0.16 ml in the volume of the antegrade portion was found (P = 0.023). The results showed no significant difference in the concentration, motility, count and total motile count of the antegrade ejaculates. In retrograde ejaculates there were no significant differences in the count, motility and total motile count. Neither was there any difference in the total count and the total motile count of both fractions. CONCLUSIONS: Electroejaculation is a reliable method for semen procurement in men suffering from anejaculation. Since semen parameters are consistent, repeated procedures are not justified for improving the sperm quality in anejaculatory, neurologically intact men.

However, sperm in these ejaculates often exhibit low motility. The aetiology of asthenozoospermia remains uncertain. It may be related to primary low-grade motility of sperm from anejaculatory men or to detrimental effects of the Electroejaculation process itself. The aim of this study was to determine whether the procedure of Electroejaculation alters the quality of the semen by comparing the characteristics of the electroejaculates with those of the normal ejaculates of the same patients. Methods Nine men suffering from ejaculation disorders underwent 14 procedures of Electroejaculation. The sperm concentration and motility of the 14 samples from Electroejaculation were compared with 20 collected at a later date after successful ejaculation by the same patients. Results The mean +/- SD concentration of the electroejaculates was (52.3 +/- 42.1)x10(6)/ml and of the natural samples (67.4 +/- 38.1)x10(6)/ml. No statistical difference was found between the two groups. The mean percentage of sperm showing progressive motility following Electroejaculation was 23.1 +/- 18.8% and 36.3 +/- 16.7% after spontaneous ejaculation. No statistical difference was found between the groups (P = 0.082). Likewise, the quality of sperm motility was similar after both methods of procurement. Conclusions In our small-sized study group (nine men with ejaculation disorders), the concentration and the motility of sperm obtained by Electroejaculation were not significantly different from sperm obtained naturally.

Intervention(s): Nocturnal emission, Electroejaculation, sperm cryopreservation, and assisted reproduction technologies. Main Outcome Measure(s): Semen analysis, intracytoplasmic sperm injection (ICSI), fertilization rates. Result(s): In four patients, the concentration and motility of sperm obtained from freeze-thawed nocturnal emission were decreased compared with sperm from Electroejaculation. Fertilization rates after ICSI using the nocturnal emission sperm were relatively low (45%). One clinical pregnancy was achieved after intrauterine insemination. Conclusion(s): The quality of sperm from nocturnal emissions is variable, but it can be used in assisted reproduction procedures to avoid aggressive procedures such as Electroejaculation or testis biopsy.

Electroejaculation and assisted fertility in men with psychogenic anejaculation

Design Retrospective clinical study. Setting In Vitro Fertilization Unit, Bikur Cholim Hospital, Jerusalem, Israel. Patients Twenty men with psychogenic anejaculation who underwent 55 sessions of Electroejaculation and their spouses. Interventions Electroejaculation, assisted reproduction technologies. Main Outcome Measures Semen analysis, IVF, intracytoplasmic injection (ICSI), fertilization rates, and pregnancy rates. Results In all patients, sperm density and motility rates were unsatisfactory (98±127 × 10 6 with 14.6%±15% motility in the antegrade portions and 42±42 X 10 6 with 9.7%±15.6% motility in the retrograde samples). Intrauterine inseminations performed in eight couples did not result in a pregnancy. Four couples underwent IVF-ET treatments. Two pregnancies were achieved with overall success rates of 22% per cycle. Five couples were treated using the ICSI procedure. Although good quality embryos were transferred, none of the treatments resulted in a pregnancy. Conclusions Psychogenic failure to ejaculate may be treated by Electroejaculation. However, the average motility of the sperm obtained is diminished. The combination of Electroejaculation with IVF, including the ICSI procedure, should improve chances of fertilization and pregnancy in these cases.

Blood and semen samples were collected after transrectal Electroejaculation and antisperm antibodies in serum and on the surface of motile spermatozoa were measured using the direct and indirect immunobead binding test. Five men (31%) were found positive for antisperm antibodies. The majority of antibodies were directed against the sperm heads. Surface antibodies were mainly IgA isotype whereas serum antibodies were IgG isotype. These results suggest that psychogenic anejaculation might be associated with increased incidence of antisperm autoimmunity.

Electroejaculation in combination with intracytoplasmic sperm injection in patients with psychogenic anejaculation results in lower fertilization rates

Design: Retrospective clinical study. Setting: In Vitro Fertilization Unit, Bikur Cholim Hospital, Jerusalem, Israel. Patient(s): Seven men with psychogenic anejaculation who underwent 16 sessions of Electroejaculation in combination with ICSI. Intervention(s): Electroejaculation, ICSI. Main Outcome Measure(s): Semen analysis, ICSI, fertilization rates. Result(s): All patients had poor sperm motility. One hundred forty-seven oocytes were injected, with a fertilization rate of 27% (39/142). One ongoing pregnancy was achieved. Conclusion(s): Sperm obtained by Electroejaculation have low motility and reduced fertilization potential. Nevertheless, ICSI should be offered to improve the possibility of successful pregnancy.

However, some sperm characteristics are better when semen is collected with EE than with TUMASG. As anesthesia reduces the response to stressors, the advantages of TUMASG may be reduced in anesthetized animals, and thus, TUMASG may be even more advantageous in conscious animals. Therefore, the aim of the present study was to compare the stress response and the characteristics of the sperm collected with TUMASG and EE in conscious goat bucks. Semen was collected in 10 bucks with both procedures. During each procedure, the time required for ejaculation, the number of electric pulses applied and the number of vocalizations were recorded. Rectal temperature, heart rate, serum cortisol concentration, biochemical and hematological parameters were measured before and after each procedure. However, quality of collected sperm with TUMASG is less than that collected by Electroejaculation. Utilisation of TUMASG may not, however, induce strong muscular contractions, so administration of oxytocin before the procedure may improve sperm quality. Therefore, two studies were designed to determine if administration of oxytocin before TUMASG improves the procedure and quality of the collected semen. The first study was performed with anaesthetised mouflons, and the second with non-anaesthetised Gabon bucks. Time length, number of electric pulses applied and numbers of vocalizations were recorded, and sperm characteristics were analysed. The number of electrical pulses and sperm characteristics did not differ with oxytocin administration in mouflons. However, oxytocin administration decreased time length (P=0.02) and tended to decrease the number of electric pulses needed for induction of ejaculation (P=0.08) in bucks. The number of vocalizations and sperm variables did not differ when the two procedures were used. In conclusion, oxytocin administration before TUMASG shortened the procedure and tended to decrease the number of electrical pulses needed to induce ejaculation in conscious bucks, thus, decreasing welfare concerns for the animals in which TUMASG is used. Therefore, its routine inclusion in this species should be considered.

A sperm sample was collected by Electroejaculation from a mature giant panda and subjected to five freezing–thawing cycles. Although repeated freezing–thawing negatively affected (P 60% of the sperm cells in both treatments showed acrosome integrity even after the fifth freezing cycle. In fresh semen, the sperm head length was 4.7 μm, the head width 3.6 μm, area 14.3 μm2 and perimeter length 14.1 μm. The present results suggest that giant panda sperm trends to be resistant to repeated freezing–thawing, even without DGC selection.

In mouflons, the physiological responses (heart and respiratory rate, rectal temperature, cortisol, creatine kinase, potassium and glucose concentrations) changed similarly with both procedures. The TUMASG procedure was faster than EE in mouflons (21.7 ± 1.4 vs. 12.4 ± 1.2 minutes, P < 0.01). In ibexes, respiratory rate, cortisol and creatine kinase concentration changes were greater with EE than with TUMASG (final respiratory rate: 62.7 ± 5.5 vs. 38.1 ± 5.6 breaths/min [P < 0.05]; final cortisol: 51.4 ± 5.1 vs. 25.3 ± 5.6 ng/mL [P < 0.001]; and final creatine kinase: 300.9 ± 99.9 vs. 87.1 ± 16.9 U/L [P < 0.001]). Electroejaculation provided better results in some sperm parameters (mouflons: sperm score: 3.4 ± 0.3 vs. 2.6 ± 0.2 [P < 0.01]; total number of sperm ejaculated: 982.4 ± 299 vs. 710.0 ± 542.2 [P < 0.05]; ibexes: sperm with progressive motility: 47.7 ± 6.2 vs. 20.5 ± 8.3 [P < 0.05]). The transrectal ultrasound-guided massage of the accessory sex glands appears to be an alternative technique to collect sperm from wild ruminants, reducing the need for electrical stimuli and thus decreasing the undesired responses of EE in the more sensitive species. On the other hand, better fresh sperm may be collected with EE. However, TUMASG provides practical advantages in animal welfare, firstly in these wild species more sensible to stress management and capture myopathy.

A sperm sample was collected by Electroejaculation from a mature giant panda and subjected to five freezing–thawing cycles. Although repeated freezing–thawing negatively affected (P 60% of the sperm cells in both treatments showed acrosome integrity even after the fifth freezing cycle. In fresh semen, the sperm head length was 4.7 μm, the head width 3.6 μm, area 14.3 μm2 and perimeter length 14.1 μm. The present results suggest that giant panda sperm trends to be resistant to repeated freezing–thawing, even without DGC selection.

After TUMASG, just one to three electrical pulses were required for ejaculation to occur in five of the six animals studied; one ejaculated after TUMASG alone. Transrectal, ultrasound-guided massage of the accessory sex glands would therefore appear to be useful in obtaining sperm samples from this species, requiring few subsequent electrical Electroejaculation stimuli and sometimes none at all. After thawing, the membrane integrity (assessed by nigrosin-eosin staining) of sperm extended with TTG was greater than that of sperm extended with TCG (P < 0.05). The total percentage of sperm showing an intact acrosome, as assessed by fluorescein isothiocyanate-conjugated peanut (Arachis hypogea) agglutinin, was also higher in the TTG-extended sperm (P < 0.05), and the percentage of dead sperm with a damaged acrosome was lower (P < 0.05). No differences were seen between TCG and TTG in terms of apoptotic manifestations (DNA damage, caspase activity, mitochondrial membrane potential, and plasmalemma stability). Therefore, TTG appears to be a better extender than TCG for cryopreserving aoudad sperm.

Body temperature, heart, and respiratory rates, as well as a number of plasma biochemical variables were also recorded prior to and during anesthesia. The detomidine plus ketamine protocols induced bradycardia and increased respiratory rate. However, the tiletamine/zolazepam protocol did not affect heart and respiratory rates. None of the three protocols caused a substantial change in rectal temperature, yet all protocols caused a significant increase in plasma glucose levels.
